Any information that is not included or left blank will affect your application and may cause it to be rejected. ❖ Factors that will be considered are academics, financial need, extra curricular activities, character, and career objectives. ❖ Only applications from students attending a high school in Dane County will be accepted for consideration. ❖ You must be in your senior year of high school and graduating in 2019 ❖ All information submitted with your application is subject to verification. Any information found to be false or misleading might cause your application to be rejected.

Information about unique personal or financial circumstances may also be added. The Personal Statement must not exceed two pages.

❑ Two Letters of Recommendation written by adults who are unrelated to you.
