Choosing Your Hospital Choosing your hospital Central Lancashire Primary Care Trust For most medical conditions, you can now choose where and when to have your treatment. This booklet explains more about choosing your hospital. You will also find information about the hospitals you can choose from. If you and your GP decide that you need to see a specialist for more treatment, you can now choose where and when to have your treatment from a list of hospitals or clinics. Why has patient choice been introduced? Research has shown that patients want to be more involved in making decisions and choosing their healthcare. Most of the patients who are offered a choice of hospital consider the experience to be positive and valuable. The NHS is changing to give you more choice and flexibility in how you are treated. Your choices Your local choices are included in this booklet. If you do not want to receive your treatment at a local hospital, your GP will be able to tell you about your choices of other hospitals across England. As well as the hospitals listed in this booklet, your GP may be able to suggest community-based services, such as GPs with Special Interests or community clinics. Or, if your GP decides to refer you to a Clinical Assessment Service and you then need to go to hospital, you will be able to choose from this list of hospitals. General information 1 Making your choice In choosing where to have your outpatient appointment, you will be choosing where to receive your full treatment, if you need it. This will include your initial outpatient appointment and any other appointments for treatment (for example, inpatient care) or aftercare. However, if you are not happy with your chosen hospital after your outpatient appointment, your GP can make you an appointment at another hospital. If you need very specialised care, you may need to be referred to another hospital. You may also be transferred to another specialty if the hospital doctor feels that this is more appropriate for your condition. 2 General information How to use this booklet This booklet includes information about your choice of hospitals. Use the ‘Where can I have my treatment?’ table on page 4 to find out which hospitals offer your treatment. You can then find information about individual hospitals. This starts on page 7. You can also find out how your hospital scores against a number of measures, some of which are patients’ views. This information starts on page 50. You will also find other information in this booklet to help you make your choice. You can use the 'Contents list' at the beginning of the booklet to find this information. You can also find more information on www.nhs.uk Your local library can help you use the internet to search for more information on choosing your hospital. See the 'Where can I find more information and support?' page later in this booklet for some website addresses that you can use. General information 3 Where can I have my treatment? Ask him or her what’s available in your local area. He or she will also advise you if the service you need is not listed. Specialist services include maxillofacial, gastric (stomach) and liver surgery, chest medicine and sleep disorders, rheumatology, stroke, diabetes, kidney dialysis, rehabilitation and joint replacement units. A new chemotherapy centre opens in 2007. Day surgery and outpatient clinics are also held at Walton Hospital. IN the first part of this paper we observed that the two counties, as they are now combined by the Registrar General, to form what is termed the " North Western division" of England and Wales, have an area of 1,874,000 acres. This is about 2928 square miles. And as England and Wales contain about 57,800 square miles, our own district comprises about |j| one-twentieth part of that area.     Ormskirk Town Centre Strategy 2015-2020 FOREWORD PARTNERSHIPS Ormskirk is a vibrant and prosperous market town which has been serving the people of West Lancashire This strategy is endorsed by the following key partners who agree to work together to deliver the Ormskirk and beyond for over 700 years. We are proud of Ormskirk’s unique historic character, long established Town Centre Strategy and Action Plan alongside West Lancashire Borough Council. market and of the increasingly successful Edge Hill University. Ormskirk has continued to remain vibrant despite the economic downturn demonstrating the strength and importance of the town centre to the local area and beyond. Through the development and implementation of this strategy, I hope that Ormskirk town centre will continue to thrive, serving both the people of West Lancashire as well as visitors to the town. This strategy seeks to build upon some of the good work which has been done previously, whilst adding fresh impetus and vigour as we look to the future. We all know that town centres are changing and now is the time to act to ensure that the town changes with the times. It is my hope that this strategy can help Ormskirk adapt to meet the requirements of these changing times. Although Ormskirk is doing well, through the implementation of actions contained within this strategy and with the support of the public and stakeholders, the town centre will go from strength to strength attracting new businesses and both local residents and visitors from further afield.     REGULATION 28: REPORT TO PREVENT FUTURE DEATHS REGULATION 28 REPORT TO PREVENT FUTURE DEATHS THIS REPORT IS BEING SENT TO: East Lancashire Hospitals NHS Trust 1 CORONER I am Rachel Galloway, assistant coroner for the area of Blackburn, Hyndburn and Ribble Valley. 2 CORONER’S LEGAL POWERS I make this report under paragraph 7, Schedule 5, of the Coroners and Justice Act 2009 and regulations 28 and 29 of the Coroners (Investigations) Regulations 2013 3 INVESTIGATION and INQUEST On the 28th September 2016 an inquest was opened into the death of Patrick Clifford. Evidence was heard at inquest on the 22nd March 2017. The inquest was adjourned part heard on the 22nd March 2017 and completed on the 11th October 2017. A narrative conclusion was left: Mr Clifford fell backwards following a faint in the toilet on Ward B4 at the Royal Blackburn Hospital on the 19th March 2016. He suffered a fractured acetabulum as a result. This was treated conservatively. His condition deteriorated over time. He developed pneumonia due to immobility and heart failure and passed away on the 18th September 2016 at Springhill care home in Accrington. 4 CIRCUMSTANCES OF THE DEATH Mr Clifford suffered a fall in the toilet at Royal Blackburn Hospital where he was a patient on the 19th March 2016. No risk assessment had been carried out but I found that the fall would not have been prevented had a risk assessment taken place, as there was no requirement for Mr Clifford to be supervised in the toilet due to his level of his mobility.
