NEW CPT CODE PROCESS: Individual components of this assay have been redefined and the initial FISH probe code will now be included in the orderable Mayo Test ID. The orderable test will carry the interpretation and corresponding FISH probe CPT code:  88291-Interpretation and report  88271x2-DNA probe, each; first probe set (formerly reflex code _PB1)

As before, the following CPT codes will be applied as appropriate based on the number of probes applied and analysis performed as indicated in the Additional Test Report:  88271x2 – DNA probe, each; each additional probe set (if appropriate)  88271x3-DNA probe, each; coverage for sets containing 5 probes (if appropriate)  88271x2-DNA probe, each; coverage for sets containing 4 probes (if appropriate)  88271 x 1-DNA probe, each; coverage for sets containing 3 probes (if appropriate)  88274 w/modifier 52-Interphase in situ hybridization, <25 cells, each probe set (if appropriate)  88274-Interphase in situ hybridization, 25 to 99 cells, each probe set (if appropriate)  88275-Interphase in situ hybridization, 100 to 300 cells, each probe set (if appropriate)

The Additional Test Report should be utilized to identify the correct CPT codes and charges for the reflex testing performed.
