Compared to other germ cell tumors in the brain, bifocal tumors are poorly understood and have a bad prognosis. In this paper we explore features that predict which children will have good outcomes and which will not. This is important for the research community because it can help physicians decide what type of radiation treatment is best to treat these children. Our study shows that bifocal tumors have a unique appearance on magnetic resonance imaging (MRI) compared to other germ cell tumors. Children with bifocal tumors are more likely to be male, have tumors that come back sooner, and cause death sooner. -

He was an 82-year-old man who presented to the emergency department with weakness and shortness of breath. Fifteen years earlier, he had been diagnosed with prostate cancer , underwent chemotherapy, and was in remission for 10 years. Eighteen months ago, he developed a recurrence and began chemotherapy again but, because of his worsening renal condition, stopped the chemotherapy about 4 months ago. On admission, he was found to have chronic kidney disease, stage 5, and sepsis. Abdominal CT was negative for genitourinary mass. His condition deteriorated rapidly and he developed bradycardia and then pulseless electrical activity (PEA). He went into cardiac arrest for 30 minutes without return of pulse and remained in PEA. His poor prognosis was explained to his family and, per the family’s wishes, resuscitation was stopped, and the patient died. Autopsy revealed bilateral dilated renal pelvis, trabeculated urinary bladder and enlarged prostate. No gross evidence of metastasis was identified in lymph nodes or bone. However, the openings of the ureters revealed papillary masses involving the distal ureters bilaterally, but not involving the bladder. Microscopic examination of the masses revealed atypical tumor cells with highly pleomorphic features. -

Rank,6 LeeAnn Baldridge,1and Liang Cheng1, 2 Abstract Purpose: Carcinoids are neuroendocrine tumors and most frequently occur within tissues derived from the embryonic gut.These tumors can occur in any organ site but are rare in the testis. The cell type giving rise to testicular carcinoid is unknown.We hypothesized that testicular carci- noid may have a germ cell origin. Experimental Design: We describe our analysis of protein and genetic markers of germ cell neoplasia, using immunohistochemistry and fluorescence in situ hybridization, in four testicular carcinoid tumors. Results: All four cases of testicular carcinoid tumor arose in a background of mature teratoma. Isochromosome 12p was identified in carcinoid tumor cells in all four samples. 12p overrepresen- tation was also observed in three cases. Isochromosome 12p and 12p overrepresentation were present in cells of coexisting mature teratoma in three cases. Carcinoid tumors showed strong immunoreactivity for synaptophysin and chromogranin, but no immunoreactivity for OCT4, CD30, c-kit,TTF-1, and CDX2. Membranous and cytoplasmic staining for h-catenin was detected in three cases. Conclusion: Our findings suggest that testicular carcinoid represents a phenotypic expression of testicular teratoma and is of germ cell origin. Testicular carcinoid tumor is rare. It was originally reported in Materials and Methods 1954 by Simon (1) who described it as part of a cystic teratoma, and additional cases have been subsequently reported. All Patients. Sarcomatoid renal cell carcinoma represents a high grade dedifferentiation of Renal cell carcinoma. This phenotype can occur in all subtypes of renal cell carcinomas, including clear cell, papillary, chromophob e, and collecting duct carcinoma. Although prognosis in sarcomatoid renal cell carcinoma is known to be extremely poor, little is known about the clinicopathologic information about them. Methods and Methodology: The population of this retrospective study consisted of all patients who underwent surgery for Renal cell carcinoma between January 2010 and February 2013 in the Department of Pathology, Sri Ramachandra university and Research Institute .A total of 64 renal cell carcinoma cases were diagnosed among which 9 had sarcomatoid changes. All 9 cases were reassessed and the diagnosis was confirmed on the basis of morphologic features. Complete baseline and follow -up data were available for analysis for all 9 patients. Conclusion: Present study showed that s arcomatoid changes were seen more commonly in younger age group with male preponderance and ne crosis was a consistent feature. Sarcomatoid areas constitute 1-25% in majority of cases which makes extensive sampling an important measure. Majority of renal cell carcinomas with sarcomatoid changes ha ve a high grade and high stage. -

Rare Sarcomatoid Carcinoma of the Liver in a Patient with No History of Hepatocellular Carcinoma: a Case Report
Rare sarcomatoid carcinoma of the liver in a patient with no history of hepatocellular carcinoma: a case report Abstract Sarcomatoid carcinoma is a rare malignant tumor of unknown pathogenesis characterized by poorly differentiated carcinoma tissue containing sarcoma-like differentiation of either spindle or giant cell and rarely occurs in the gastrointestinal tract and hepatobiliary-pancreatic system.1 Primary hepatic sarcomatoid carcinoma accounts for only 0.2 % of primary malignant liver tumors, and 1.8% of all surgically resected hepatocellular carcinomas.2 The majority of hepatic sarcomatoid carcinoma cases appear to occur simultaneously with hepatocellular or cholangiocellular carcinoma.3 The preferred treatment for hepatic sarcomatoid carcinoma is surgical resection and the overall prognosis is poor.4 This case depicts a 62-year-old female who underwent initial resection of a cavernous hemangioma in 2010. Seven years after her initial diagnosis, she developed what was initially felt to be local recurrence of the hemangioma but additional diagnostic workup with a liver biopsy confirmed primary hepatic sarcomatoid carcinoma. Patient had a complicated gastrointestinal history and underwent surgical resection of a cavernous hemangioma in 2010. Six years later, she developed abdominal fullness with right upper quadrant pain and an abdominal ultrasound at that time suggested hemangioma recurrence. In 2017, she underwent laparoscopy with unroofing of the hemangioma, drainage of an old organizing hematoma, removal of debris, and placement of an omental patch. -
