Case Management Services

Case Management Services 2018

Our case managers are trained and caring professionals who are knowledgeable about issues related to aging and about community resources available for older adults. Case Management is a collaborative process of assessment, planning, coordination, evaluation, and advocacy for options and services to meet an individual’s needs. Some individuals are looking for short-term information, while others benefit from on-going case manage - ment.

In 2018, the Case Managers had 3784 direct and indirect contacts assisting older adults with services and resources. They spent 1500 hours providing Case Management services.

Frequent information requested and provided by the Case Managers in 2018 was on Alzheimer’s and Dementia resources, energy assistance, grocery delivery, housing options, home care agencies, meals on wheels, lifeline, prescription drug coverage, senior center lunches/programs, and transportation resources.

The case managers are responsible for the approval of home delivered meals. They conduct home visits to assess if the client is eligible to receive the home delivered meals and complete the required paperwork. In 2018 17,127 meals on wheels were delivered to seniors in our area.

The Case Managers provided one-on-one Medicare Part D counseling to 188 individuals during Medicare Part D open enrollment, October 15 – December 7, 2018. The Case Managers assisted 44 people with enrolling in a new Medicare Part D plans. This service, saved individuals a total amount of $36,043.67!

In addition to these services, our case managers have taken lead roles with the dementia friendly committee, the caregiver support group and the Parkinson’s support group. They continue their education on resources for older adults through trainings provided by the Area Agency on Aging.

They have also been vital in promoting the activities, lunch, and transportation the Senior Center offers. They look forward to continuing to help older adults in our community in 2019!

Case Management Services Case Management assistance is available to individuals 60 and older. We provide information, resources, and support for older adults and their families. Some individuals are looking for short-term information, while others benefit from ongoing case management support.

Trips & Excursions

TSI Transportation

Daily bus service is available 9:30 a.m. to 2:30 p.m. This service provides transportation to the Senior Center for meals and other services, to use the Village Center fitness area/walking track and for local shop - ping. We can request a bus with wheelchair lift if a passenger needs it.

The cost is $1 round trip to the Senior Center and $2 round trip for in-town shopping. Reservations must be made one day prior to ride by calling 849-8385.

Federal Government Shut Down – How it’s impacting older adults Food Share - Those currently enrolled in the Food Share program will see their full February benefits loaded on to their Quest card as early as January 20. Recipients are encouraged to check their benefit balance on or after this date since they will not receive any written notice. They will then not receive any additional funds during the month of February.

If the government shut down continues there is no guarantee that the March benefits will be dispersed.

If the shutdown continues if could eventually affect Section 8 housing assistance.

Home Energy Plus Program An Energy Assistance representative will be at the Senior Center from 9:00 a.m. to 4:00 p.m. on Thursday, April 11, 2019.

Households whose total gross income for the three months immediately preceding the month of the application are at or below the amount for their household size (see income guidelines below). Households that are responsible for paying heating costs directly to a fuel or utility company (must have an active utility account). Households whose heating costs are included in their rent may qualify. The Senior Center Patio, which over looks beauti - ful Village Park, has 136, 24” by 24” tiles. Your purchase of a whole or half engraved tile will be a lasting symbol of your generosity to your community and support of the Senior Center. You may wish to have your name, the name of a loved one, family or business engraved on a tile. The cost of a full 24” by 24” tile is $500. A half tile is $300. For an addi - tional cost, a logo may be placed on the tile.
