CA Student 2021 Prom & Post Prom Party Agreement May 1 (Please read through this form before signing) • The Prom/Post Prom Party is for all Junior and Senior students. • All students attending the Prom/Post Prom Party MUST complete a Party Agreement form. • ALL Non-CA students must provide a completed agreement form to attend and a regular dance contract. • The above-mentioned forms MUST be turned in at the time you purchase your Prom/Post Prom Party ticket. Tickets will not be sold to anyone without these signed agreements. • Prom/Post-prom tickets will be on sale March 22-24 during lunch or see Mrs. Sell by April 9. • If you need another copy of this form, it can be found on the CA website under Current Students/Forms. • Prom dress code is as follows: Dresses should be made of solid, opaque material from the bust to the fingertip length of the skirt. Sheer, see-through and translucent materials are not acceptable for this main part of the dress. The backline of the dress must start at least 5 inches above the buttocks. Any slits in the dress must not be higher than fingertip length. Excessive cleavage may not be shown. Baggy pants that leave underwear exposed will not be allowed. Any student who fails to adhere to these guidelines will be sent home to change before being admitted into Prom or Grand March. • Post Prom Party dress code is as follows: regular school day dress code applies (Girls - no tank/halter tops and shorts must be acceptable length. Boys - no tank tops, and pants/shorts must cover undergarments). Shoes must be worn at all times. We, (student and parent/guardian) agree to the following: 1. I understand that I must arrive at CA by 5:45pm. I understand that failure to show up on time will result in a phone call to parents. 2. I understand that if any prescriptions or over the counter medications are found, they will be placed in a bag marked with my name and will be available from a chaperone during the evening if needed. 3. I agree to leave all outside food, beverages, toothpaste, and mouthwash at home and understand that should they be found they will be removed and discarded. 4. I understand that school rules apply. 5. I understand that ANY unidentifiable, illegal, and/or banned items found in bags will be confiscated and identified with my name; administration will be notified, my parents will be contacted, and I will not be allowed to attend the party. 6. I have read and understand and will adhere to the Prom and Post Prom Dress code as outlined above. 7. I understand that parents/guardians will be contacted immediately if there are any serious problems. 8. I understand that Concordia Academy and chaperones are not responsible for student injury or loss/damage of property. If a student is injured, the parent/guardian will be contacted. If parent/guardian cannot be reached, by signing the attached form, parents/guardians’ consent to treatment being provided by the nearest emergency medical center.
