Blood pressure was 120/60 mmHg. A 12-lead electrocardiogram (ECG) revealed inverted T-waves in anterior and inferior leads (Fig. 1). Cardiac troponin Ic was 12 ng/mL (normal range 0—0.04 ng/mL), creatinine phosphokinase: 529 IU/L (normal range 33—145 IU/L), and myoglobin: 894 ␮g/L (normal range 10—92 ␮g/L). Determi- nation of arterial blood gases at room air revealed: pH: 7.42; PaO2: 85.4 mmHg; PaCO2: 38.3 mmHg; HCO3: 24.3 mmol/L; and SaO2: 97%. Thoracic scanner ruled out a pneumothorax or a pneumomediastinum but revealed a right perihi- lar bullous dystrophy (Fig. 2). Brain magnetic resonance imaging showed a right fronto-parietal ischemic process (Fig. 3). Discussion While diving, divers present relatively stable pulmonary vol- umes, but during ascent, the compressed gas in their lungs expands and the pulmonary volumes increase as the ambi- ent pressure falls. If intrapulmonary gas is prevented from escaping through the airways by gas trapping, excessive transpulmonary pressures will result and rupture can occur in the pulmonary capillaries. Escaping gas can enter pul- monary veins (which transit the left heart leading to arterial gas embolism). Air bubbles are then released in the sys- temic circulating blood flow and can produce acute cerebral and coronary arterial embolic manifestations. Pulmonary diseases predisposing to gas trapping (bullous lung disease, asthma) may favor the occurrence of pulmonary barotrau- mas [2]. In this case, the perihilar bullous dystrophy and rapid ascent may have precipitated the accident. In the absence of a patent foramen ovale, another likely mechanism to explain arterial gas embolism is the opening of pulmonary arteriovenous shunts by accumulation of nitrogen bubbles in the capillary bed site, causing tran- sient pulmonary arterial hypertension, leading to arterial bubbles. However, in this case, paradoxical gas embolism caused by decompression illness remains unlikely. In summary, myocardial ischemia can be a complication of scuba diving, but does not always reveal a significant obstructive coronary artery disease. A coronary angiogram must be performed in this situation, but must not delay hyperbaric therapy. Recent studies have determined that discursive communication is insufficient in expressing one’s pain. In particular, Elaine Scarry maintains that pain destroys language and that its victims must rely on the vocabulary of other cultural spheres in order to express their pain. The problem is that neither Scarry nor any other Western pain scholar can provide an alternative to discursive language to express pain. This study claims that both artists must work beyond their own cultural registers in order to give their pain a language. In the process of expressing their suffering, Duras and Kahlo subvert traditional literary and artistic conventions. Through challenging literary and artistic forms, they begin to re-think and ultimately re-define the way their readers and viewers understand feminine subjectivity, colonial and wartime occupation, personal tragedy, the female body, Christianity and Western hegemony.

The incidence of MEB has been shown to vary between treatment centers and patients. This study was aimed to determine which patients are at high risk of MEB. Materials and methods: Prospective study including all the patients treated in a multiplace HBO2 chamber between January and December 2005. Scoring of MEB before and after HBO2 by otoscopy was performed using the Haines and Harris classification. Results: We included 130 patients: 53 Males, 37.5 ± 20.5 years old; 76% were treated for CO poisoning, 11% for iatrogenic gas embolism, 12% for decompression sickness and 4% for necrotizing soft tissue infection. 13% were intubated. MEB occurred in 13.6% of the patients (12.4% of the conscious and 24.4% of the intubated patients, p=0.26). Risk factors for MEB were: repetitive treatments and difficulties with pressure equalization. There was no influence of age, sex or mechanical ventilation on the occurrence of MEB. Conclusions: MEB induced by HBO2 occurred in 13.6% of the patients.

Methods: Thirty-six consecutive patients with ARF treated by NPPV were matched for diagnosis, age within 5 years, sex, preoperative radiochemotherapy and Charlson co-morbidity index with 36 patients who received conventional treatment (control group). Results: NPPV was associated with a lower reintubation rate (nine versus 23 patients; P = 0·008), lower frequency of acute respiratory distress syndrome (eight versus 19 patients; P = 0·015), and a reduction in intensive care stay (mean(s.d.) 14(13) versus 22(18) days; P = 0·034). Anastomotic leakage was less common in patients receiving NPPV (two versus ten; P = 0·027). These patients also showed a greater improvement in gas exchange in the first 3 days after onset of ARF (P = 0·013). Conclusion: The use of NPPV for the treatment of postoperative ARF may decrease the incidence of endotracheal intubation and related complications, without increasing the risk of anastomotic leakage after oesophagectomy.

    吉田:Aerosiniusitis ―An Experience Of Treatmentof Two Cases― 71 CASE REPORT AEROSINIUSITIS ―AN EXPERIENCE OF TREATMENT OF TWO CASES― Yasuyuki YOSHIDA Department of ENT, Saint Marguerite Hospital (Received: May 9, 2002) Abstract Aerosinusitis is not a rare entity of the disease, but not usually encountered at the general practice of ENT. We have experienced three operations of two cases of this condition which occurred in the fighter pilots: the intercep- tor F-4EJ and F-15 J, of the Japan Air Self Defence Force. The symptoms of maxillary pain of the affected side dur- ing the landing approach and purulent nasal discharge thereafter was noted in all case, as described in the litera- ture. Pre-and postoperative pressure trial was done to all cases. The operation done was coventiona maxilloeth- moidectomy under local anesthesia and, during the operation, cicatric closure of the fontanelle, which was surgi- cally open was found in all cases and the counter opening was made at the inferior meatus. Recurrence was not noted thus far observed. Jet-engined aircraft which attains high performance brings great altitude changes therefore, pilots or crews and passengers are exposed to great barometric changes, so that it inevitably raises the incidence for aerosinusitis. But the incidence of the passenger aircraft for aerosinusitis is controlled to the minimum because the comfortabil- ity of the air travel is one of their important factors; on the other hand, it is not the first importance for the mission- oriented tatical aircraft. The pressurization is lowered so that it is considered the incidence of these aircraft for aerosinusitis is relatively high as compaired with the former.
