Instructions for filling out the questionnaire:

The form should tab you through all of the fields. There are several fields that are required, which are defined by an * .

At the top of the first page of the form, there are buttons for ‘Print Form’, ‘Submit by Email’ and ‘Reset Form’. There is also a ‘Submit Form’ button at the top right of the Acrobat toolbar, both ‘Submit’ buttons do the same thing.

If you need to start over, click on the ‘Reset Form’ button to remove all of the current information.

BEFORE YOU CLICK THE ‘Submit’ BUTTON: When the questionnaire is complete, you can print a copy for your records. Make sure you print before you select the submit button, as it may close the questionnaire.

To submit the questionnaire, click either ‘Submit’ button and an email should be created with the questionnaire attached and the return email address entered. It may also automatically send the email for some people. If you are unsure whether it sent or not, check your email ‘sent’ box.

If you haven’t scheduled your consultation yet, now is the time to do so. During your consultation we will answer all of your floral questions, discuss your day vision, and help you stretch your budget to ensure that you have the flowers you’ve been dreaming about. If you have no idea what flowers you want for your wedding—don’t worry, we can help you with that as well! If you decide to have A Floral Affair provide any or all of the floral services for your wedding a $100 non-refundable deposit is required to reserve your wedding date. The remaining balance is due no later than four weeks prior to the delivery date. We look forward to designing your wedding flowers and taking part in your special day. Thank you!
