Instructions for filling out the questionnaire: The form should tab you through all of the fields. There are several fields that are required, which are defined by an * . At the top of the first page of the form, there are buttons for ‘Print Form’, ‘Submit by Email’ and ‘Reset Form’. There is also a ‘Submit Form’ button at the top right of the Acrobat toolbar, both ‘Submit’ buttons do the same thing. If you need to start over, click on the ‘Reset Form’ button to remove all of the current information. BEFORE YOU CLICK THE ‘Submit’ BUTTON: When the questionnaire is complete, you can print a copy for your records. Make sure you print before you select the submit button, as it may close the questionnaire. To submit the questionnaire, click either ‘Submit’ button and an email should be created with the questionnaire attached and the return email address entered. It may also automatically send the email for some people. If you are unsure whether it sent or not, check your email ‘sent’ box.
