Consultant Psychiatrist Old Age Psychiatry

Mental Health Services Fife Psychiatric Services The NHS Fife Mental Health Service is a Fife wide service delivering the full range of inpatient and community care and treatment for patients of all ages experiencing mental illness and a range of mental health problems, including alcohol and drug dependency. The service is managed and governed within and by the Fife Wide Division of Fife Health & Social Care Partnership.

It is a large service which provides secondary, tertiary and specialist services, and links with primary care and the Acute Division. Services providing inpatient and community general adult and older adult psychiatric care are organised geographically and those providing general and specialist community services for children and young people, and for adults with addiction problems are organised functionally.

Older Adult Services Older Adult services are provided across 6 community sectors which largely align to the new GP clusters, with inpatient and outpatient care being led by the sector consultant. Inpatient services are based across two hospital sites (, Dunfermline and , Cupar) with both acute admission and specialist dementia beds available on both sites.  Dunfermline and West Fife Sectors – based in Queen Margaret Hospital for community and inpatient services  Sector – based in Whyteman’s Brae Hospital, Kirkcaldy with inpatients in Queen Margaret Hospital.  Glenrothes Sector – based in Whyteman’s Brae Hospital with inpatients in Stratheden Hospital.  North East Fifes Sector – based in Stratheden Hospital

All Older Adult sector services are supported by Intensive Home Treatment teams and also by the newly developed Care Home Liaison service. Inpatient wards are supported by the Medicine of the Elderly Hospital@Home team to provide physical health interventions where needed and there is active collaboration with Medicine of the Elderly on issues of joint interest such as Frailty.

Supporting Services 1. Community Psychiatric Nursing Services in Older Adult and General Adult services - teams work on a sector basis and are being redeveloped to work in a CMHT model. 2. Dementia Post Diagnostic Support services are available across Fife to provide key worker support and signposting for people with a new diagnosis of dementia and their carers.

3. Department of Clinical Psychology, led by the Area Head of Psychology Services, has bases at Stratheden, Whyteman's Brae and Lynebank hospitals and works in parallel to Mental Health services. 4. The Fife-wide Occupational Therapy Service (Mental Health) provides a service to acute, rehabilitation and community patients for both over and under 65 year old patients. The service includes assessment of functional capacity and deficit, therapeutic activity, specific evidence based interventions and discharge planning. 5. Social Work services for older adults are aligned geographically, as well as providing a specific hospital discharge service. There is a separate Fife -wide specialist MHO team. NHS Fife and Fife Council areas are co-terminus. 6. Specialist Pharmacy Services. Specialist mental health pharmacy managers work closely with clinicians, provide information to patients and carers and work on local protocols.

General Adult Services The Adult Psychiatric Service in Fife is divided into three sectors, with inpatient and out patient care being delivered by the sector consultant. In-patient services are at Whyteman’s Brae Hospital, Queen Margaret Hospital, and at Stratheden Hospital. There are three general adult acute wards, one on each site.

Specialist Services Unscheduled Care Assessment Team This is a 24 hour nurse lead team with medical input from trainee doctors and speciality grade doctors within hours (consultant supervision within sectors) and on call medical psychiatric doctors out of hours. It is a Fife wide service and is based at Whytemans Brae Hospital. It provides urgent assessments, management and signposting for unscheduled presentations including A&E at the Victoria Hospital Kirkcaldy and has close links with the sector teams.

Development plans  Mental Health Services are working towards increasing services in the community and reducing dependence on beds, along with plans to redevelop and update inpatient wards.  Clinical leads posts have recently been established with an aim of enhancing medical input into the Mental Health Strategic Team and service development.  In General Adult Psychiatry work is currently underway to further develop and streamline sector CMHTs particular emphasis on care planning, relapse prevention, patient and carer empowerment in addition to looking at the interfaces between different aspects of mental health services, primary care and the voluntary sector.  A new IPCU on the Stratheden site opened in 2016.  Dementia Friendly Communities work – multi agency work including Fife Council, NHS Fife and Alzheimer’s Scotland with a very successful project initially based in Glenrothes and now being rolled out across the whole of Fife.  Fife Health and Social Care Partnership is developing a series of community resources including Health and Wellbeing resources “the Well” and community hubs for co-working and information sharing between community services including District Nursing, Medicine of the Elderly, Older Adults Psychiatry and Social Work.

South East Scotland Psychiatric Training Scheme Fife is part of the South East of Scotland Psychiatric Training scheme. Trainees rotate throughout the whole region, between Lothian, The Borders and Fife. Trainees spend one half day each week during the academic year attending the MRCPsych course in Edinburgh and an additional half day of postgraduate training in Fife. They also attend psychotherapy training. There are GPSTs and FY2s on rotation with the service also. In total there are 19 trainee doctor posts. There are also frequently Higher Trainees in both Old Age and General Adult Psychiatry based in Fife.

Arrangments For Medical Representation There is an active Local Negotiating Committee with Primary Care Division representation.

Duties of the post The post became vacant with the retirement of a long serving consultant.

The post holder will provide community and inpatient cover for the Dunfermline area within west Fife, serving a population of approximately 12000 people over 65 years old. The post is based at Queen Margaret Hospital, Dunfermline and is supported by CPN, Day Hospital, Intensive Home Treatment and Care Home Liaison teams and works alongside the Consultant for the second West Fife sector. The West Fife sectors are supported by a further part time Consultant (0.5WTE), a full time Speciality Grade Doctor and a part time (0.8WTE) Speciality Grade Doctor. West Fife services are also supported by Clinical Psychology, Occupational Therapy, Dementia Post Diagnostic Support and Specialist Pharmacy services.

Queen Margaret Hospital (QMH) in Dunfermline is the main service base in this West Fife sector for both Adult and Old Age Services. It has one general adult acute admission ward, one older adult admission ward and one older adult interim care ward. Developments ongoing within the service include an aim for the Care Home Liaison Service in the West to have dedicated Consultant time, which would include not only cover of patients in West Fife Care Homes, but also for the Older Adult Transitional Care Ward (Ward 4 QMH).

The post holder will be responsible for the supervision of a junior doctor, with opportunities to become a trainer for Higher Trainees in Old Age Psychiatry and to offer special interest placements for Higher Trainees in Medicine of the Elderly.

The post holder will be expected to be involved in audit and clinical governance and will have the opportunity to have a clear role in shaping further service development, as well support to develop a special interest to further augment the whole service. There are also opportunities to become involved in teaching for medical students from Edinburgh, Dundee and St Andrews Universities, with the possibility of an Honorary Teaching Fellow role. There are opportunities to become involved in research with St Andrews University and to support Clinical Trials (particularly in Dementia)based in both Edinburgh and Dundee.

Principal Duties  Undertake the statutory duties require of an Approved Medical Practitioner (AMP) under the terms of the Mental Health (Care and Treatment) (Scotland) Act 2003.  Undertake clinical assessment, diagnosis and management of patients with mental disorders, in community, inpatient and day hospital settings, for one catchment area in West Fife.

 Work with Consultant colleagues to co-ordinate care of patients with other clinical teams in older adult psychiatry including the Intensive Home Treatment and Care Home Liaison Teams.  Attend and participate in Care Programme Approach meetings, Mental Health Tribunal Hearings, Team meetings, Care Home Liaison meetings, , and any other relevant clinical and managerial meetings.  Responsibility for the education, training and supervision of any aligned non- consultant grade medical staff, trainee medical staff and medical students. The post holder will be expected to have attended or to attend the educational supervisor course.  In line with the seniority of the post, provide appropriate clinical leadership within the service and work flexibly with colleagues of all professional disciplines to achieve the best standards of care possible.  Participate fully in the Fife wide psychiatric consultant on-call rota. This is a non-resident rota on a 1:17 nominal basis. and includes out of hours cover for CAMHS issues. Out of hours first response is provided by a dedicated mental health Unscheduled Care Team (nurse led with a rostered on duty trainee doctor). There is a Primary Care Emergency Service (PCES) and an MHO on-call rota. Out of hours cover for Learning Disability patients is provided by a separate LD regional rota.  In conjunction with consultant colleagues, provide a share of cover for colleagues on annual, public holiday, parental and study leave.  Liaise effectively with social work services, including Mental Health Officers (MHO’s) and with relevant non-statutory service providers.  Work collaboratively with management and professional colleagues in taking forward relevant service developments, performance and health improvement work. There are no management duties associated with this post but the post holder is expected to co-operate with and support the Clinical Services Managers in meeting the objectives of the service. Any specific management duties will be agreed through discussion.  Provide, when required, teaching to multidisciplinary staff at various stages of their training and to other groups for in-service training.  The above list of duties is not exhaustive and a significant amount of work will be generated by the patients’ needs and demands. Accordingly, the post holder is expected to exercise a high degree of initiative, independence, personal organisation and planning skills in order to meet the demands of the post.  In addition to the duties outlined above, you may occasionally be required to undertake other duties for limited periods.  In view of the ongoing changes in the Mental Health Service, the job description will be subject to review after 12 months to allow flexibility for both the successful applicant and the managers planning service delivery changes.

Terms and Condtions of employment The post holder will have a contract of employment with NHS Fife. The terms and conditions of service relating to employment are those currently set out in the Hospital Medical & Dental Staff and Doctors in Public Health Medicine and the Community Health for the Consultant Grade and the General Whitley Council Conditions of Service.

Absence The post holder should make requests for annual leave and reports of sickness absence to the clinical lead & management secretary. The successful candidate is expected to contribute to management tasks.

Professional support Professional support is available through NHS Fife Medical Director and/or Director of Public Health with whom the appointee is entitled to discuss any grievance relating to their employment, or to gain advice and/or support following a failure to agree a job plan at review. NHS Fife Board act as final arbiters of disputes over job plans which cannot be resolved at local level.

Employment details All NHS Fife appointments are subject to a satisfactory medical report and a PVG check.

Superannuation The appointment is superannuable unless you opt out of the NHS Superannuation Scheme. Staff who choose not to be in the scheme will automatically contribute to the State Earnings Related Pension Scheme (SERPS) unless they are excluded from doing so or have arranged a Personal Pension Plan.

Annual Leave The annual leave entitlement for consultant staff is six weeks and 10 public holidays per annum on a pro rata basis.

Programmed Activities The post will offer a maximum of 2 SPAs. 1 is guaranteed for CPD the other will be negotiated taking into account duties such as appraiser, trainer or other agreed specific tasks.

Salary Salary Scale: £80,653 - £107,170 per annum. Progression through seniority points will be in accordance with the provisions of Section 5 of the Terms and Conditions of service.

Other duties In addition to the duties outlined above, you may occasionally be required to undertake other duties for limited periods.

Notice period The employment is subject to three months’ notice (or longer) on either side.
