The service is managed and governed within and by the Fife Wide Division of Fife Health & Social Care Partnership. It is a large service which provides secondary, tertiary and specialist services, and links with primary care and the Acute Division. Services providing inpatient and community general adult and older adult psychiatric care are organised geographically and those providing general and specialist community services for children and young people, and for adults with addiction problems are organised functionally. Older Adult Services Older Adult services are provided across 6 community sectors which largely align to the new GP clusters, with inpatient and outpatient care being led by the sector consultant. Inpatient services are based across two hospital sites (Queen Margaret Hospital, Dunfermline and Stratheden Hospital, Cupar) with both acute admission and specialist dementia beds available on both sites. Dunfermline and West Fife Sectors – based in Queen Margaret Hospital for community and inpatient services Kirkcaldy Sector – based in Whyteman’s Brae Hospital, Kirkcaldy with inpatients in Queen Margaret Hospital. Glenrothes Sector – based in Whyteman’s Brae Hospital with inpatients in Stratheden Hospital. North East Fifes Sector – based in Stratheden Hospital All Older Adult sector services are supported by Intensive Home Treatment teams and also by the newly developed Care Home Liaison service. Inpatient wards are supported by the Medicine of the Elderly Hospital@Home team to provide physical health interventions where needed and there is active collaboration with Medicine of the Elderly on issues of joint interest such as Frailty. Supporting Services 1. Community Psychiatric Nursing Services in Older Adult and General Adult services - teams work on a sector basis and are being redeveloped to work in a CMHT model. 2. Dementia Post Diagnostic Support services are available across Fife to provide key worker support and signposting for people with a new diagnosis of dementia and their carers. 3. Department of Clinical Psychology, led by the Area Head of Psychology Services, has bases at Stratheden, Whyteman's Brae and Lynebank hospitals and works in parallel to Mental Health services. 4. The Fife-wide Occupational Therapy Service (Mental Health) provides a service to acute, rehabilitation and community patients for both over and under 65 year old patients. The service includes assessment of functional capacity and deficit, therapeutic activity, specific evidence based interventions and discharge planning. 5. Social Work services for older adults are aligned geographically, as well as providing a specific hospital discharge service. There is a separate Fife -wide specialist MHO team. NHS Fife and Fife Council areas are co-terminus. 6. Specialist Pharmacy Services. Specialist mental health pharmacy managers work closely with clinicians, provide information to patients and carers and work on local protocols. General Adult Services The Adult Psychiatric Service in Fife is divided into three sectors, with inpatient and out patient care being delivered by the sector consultant. In-patient services are at Whyteman’s Brae Hospital, Queen Margaret Hospital, and at Stratheden Hospital. There are three general adult acute wards, one on each site. Central Fife Sector Kirkcaldy & Levenmouth areas - based in Whyteman’s Brae Hospital, Kirkcaldy Glenrothes area - inpatients based in Stratheden Hospital, Cupar, Day Hospital and Gemini Team based in Whyteman’s Brae Hospital, Kirkcaldy North East Sector- Based in Stratheden Hospital, Cupar West Sector - Based in Queen Margaret Hospital, Dunfermline Specialist Services Unscheduled Care Assessment Team This is a 24 hour nurse lead team with medical input from trainee doctors and speciality grade doctors within hours (consultant supervision within sectors) and on call medical psychiatric doctors out of hours. It is a Fife wide service and is based at Whytemans Brae Hospital. It provides urgent assessments, management and signposting for unscheduled presentations including A&E at the Victoria Hospital Kirkcaldy and has close links with the sector teams. Old Age Psychiatry Whyteman’s Brae Hospital (Day Hospital only), Kirkcaldy Stratheden Hospital, Cupar Queen Margaret Hospital, Dunfermline Rehabilitation Stratheden Hospital,Cupar (Fife wide). Intensive Psychiatric Care Unit Stratheden Hospital, Cupar (Fife wide). Addictions Cameron Hospital, Windygates, Leven. Whyteman’s Brae Hospital, Kirkcaldy Lynebank Hospital, Dunfermline Child and Adolescent Playfield House, Stratheden Hospital, Cupar (Fife wide). Learning Disabilities Lynebank hospital, Dunfermline (Fife wide). Forensic Forensic Community Mental Health Team, Lynebank Hospital, Dunfermline Radernie Low Secure Unit, Stratheden Hospital, Cupar Development plans Mental Health Services are working towards increasing services in the community and reducing dependence on beds, along with plans to redevelop and update inpatient wards. Clinical leads posts have recently been established with an aim of enhancing medical input into the Mental Health Strategic Team and service development. In General Adult Psychiatry work is currently underway to further develop and streamline sector CMHTs particular emphasis on care planning, relapse prevention, patient and carer empowerment in addition to looking at the interfaces between different aspects of mental health services, primary care and the voluntary sector. A new IPCU on the Stratheden site opened in 2016. Dementia Friendly Communities work – multi agency work including Fife Council, NHS Fife and Alzheimer’s Scotland with a very successful project initially based in Glenrothes and now being rolled out across the whole of Fife. Fife Health and Social Care Partnership is developing a series of community resources including Health and Wellbeing resources “the Well” and community hubs for co-working and information sharing between community services including District Nursing, Medicine of the Elderly, Older Adults Psychiatry and Social Work. South East Scotland Psychiatric Training Scheme Fife is part of the South East of Scotland Psychiatric Training scheme. Trainees rotate throughout the whole region, between Lothian, The Borders and Fife. Trainees spend one half day each week during the academic year attending the MRCPsych course in Edinburgh and an additional half day of postgraduate training in Fife. They also attend psychotherapy training. There are GPSTs and FY2s on rotation with the service also. In total there are 19 trainee doctor posts. There are also frequently Higher Trainees in both Old Age and General Adult Psychiatry based in Fife. Arrangments For Medical Representation There is an active Local Negotiating Committee with Primary Care Division representation. All consultants are members of Fife Division of Psychiatry.
