We are seeking new consultant colleagues to join our friendly team of consultant radiologists based at Victoria Hospital/Queen Margaret Hospital.

You will have the opportunity to influence the future of the service in NHS Fife, alongside the Lothian component of this post.

You should be proficient in cross sectional imaging and development of sub-specialist interests will be encouraged as part of our commitment to innovation and excellence. We would particularly value candidates with expertise in colorectal, lung, urology, gynae, head and neck or musculoskeletal imaging.

There is a strong emphasis on providing a sustainable work/life balance and we are keen to consider flexible working options. The posts include a commitment to 50 % of a 1 in 10 out of hours rota. Overnight CT scans are outsourced to a ‘night-hawk’ service.

NHS Fife has a rolling plan for equipment replacement. Present imaging facilities include: two 1.5T MRI scanners, Three CT scanners (128 slice), three fluoroscopy suites, 6 (non obstetric) ultrasound scanners, one state-of-the-art intervention room and separate units for breast and nuclear medicine.

Postgraduate and undergraduate teaching is strongly encouraged, with established links with St Andrews, Edinburgh and Dundee Universities. We have a strong commitment to radiology trainees from the South East Scotland training programme. NHS Fife has a proactive Research and Development team who are committed to supporting research with established links with St Andrews and Dundee Universities.

You must be eligible for inclusion in the Specialist Register or be within 6 months of your expected CCST, at the time of interview.

JOB DESCRIPTION

THE POST

Introduction

The advertised posts will be 50:50 split posts, shared with NHS Lothian. The Fife component will based at the Victoria Hospital, which is easily reached by car via the A92 form Edinburgh and is within walking distance from the train station with onward connections to Edinburgh and beyond. There may be a requirement to work at the Queen Margaret Hospital, Dunfermline. This is also easily accessed by car from the A92 and across the road from the railway station.

These are replacement posts. You will have a strong background in general cross- sectional imaging although opportunities exist to develop, or continue with the practice of, a wide range of sub-specialist interests. All subspecialty interests would be considered, but we would particularly value expertise in GI, Chest, GU, gynae, head and neck or musculoskeletal imaging.

Definition

Whole time Consultant Radiologist, split 50:50 with Lothian. The Fife component is based at Victoria Hospital though you may be required to work at Queen Margaret Hospital, Dunfermline. This post entails responsibility for providing, with consultant colleagues, a comprehensive Radiology service to NHS Fife.

Clinical Staff: Radiology is part of the Women’s, Children’s and Support Services Directorate. The Clinical Lead for Radiology is Dr Katharine Jamieson, Consultant Radiologist. In addition to our substantive radiology consultants, NHS Fife has networked arrangements with interventional colleagues in NHS Tayside and colleagues in NHS Lothian. The national PACS system allows significant regional collaboration and we have established a regional reporting network, SERRIS. We have also developed a number of advanced radiography roles including reporting radiographers and sonographers.

Support Facilities (Offices/Secretary):

Shared office accommodation will be provided on both sites, with additional reporting hubs. The department predominantly uses VR. You will have support from departmental personal assistants on both sites as well as by the PACS teams.

DUTIES AND RESPONSIBILITIES

a) Clinical

You will provide radiological interpretations for CT, MRI, ultrasound and plain film examinations. Opportunities exist for fluoroscopy and some interventional radiology if desired. Core “general work” is shared by all radiologists but you will have the opportunity to develop your sub-specialist interests in a number of clinical areas. These include, but are not limited to head and neck, GI, GU, gynae, chest and musculoskeletal imaging. We have excellent relationships with our medical staff in Fife and you will also liaise with them on your duty session in regards to discussing cases/ arranging examinations. You will be encouraged to attend and support some of the wide variety of MDT meetings according to your clinical interests.

b) Managerial

You will be expected to work with local managers and professional colleagues in the efficient running of services and will share with consultant colleagues in the medical contribution to management.

c) Clinical Audit, Data management and Clinical Governance

Clinical audit is important, both in regard to quality improvement of the department and personal revalidation, and as such you will be encouraged to contribute to the audit and clinical governance activities within the department.

d) Leave

Annual leave entitlement is as laid out under terms & conditions of the new consultant contract. Leave entitlement will be calculated on a pro rata basis for those working part-time.

e) Research

Opportunities for research exist internally; in addition there are possibilities to cultivate research links with the University of St Andrews.

f) Teaching

- 4 - We are part of the South East Scotland Radiology rotation and regularly have radiology trainees joining us from there. We also enjoy teaching students from the Universities of Edinburgh, Dundee and St Andrews on a weekly basis, as well as junior medical staff based in Fife. You will be encouraged to play an active role in teaching all these groups.

g) Continuous professional development You will be entitled to 10 days of study leave per year to allow you to fulfill your CPD needs as outlined by the Royal College of Radiologists.

Job Plan Review

The Job Plan included is indicative and still to be finalised. In particular, you may be asked to consider extra programmed activities of direct clinical care. The level of participation in on-call will be agreed at job planning and appropriate PAs agreed (either as extra programmed activities or in exchange for other DCC activity). Your job plan will be subject to review once a year by you, your colleagues in radiology and the Lead Clinician/Clinical Director, as laid out in the terms & conditions of the new consultant contract.

In the event of any significant changes in the circumstances affecting this job plan, it is agreed that it will be renegotiated jointly in collaboration with consultant colleagues.

Cover for Consultant Colleagues

Annual/Study Leave - You will be required to provide emergency cover for consultant colleagues during absence on annual or study leave. If for any reason such deputising is not practicable, the Acute Division of NHS Fife undertakes to authorise immediate consultant locum cover.

GENERAL PROVISIONS

You will be expected to work with local managers and professional colleagues in the efficient running of services and will share with Consultant colleagues in the medical contribution to management. Subject to the provision of the Terms and Conditions, you are expected to observe the Division’s agreed policies and procedures, drawn up in consultation with the profession on clinical matters, and to follow the standing orders and financial instruction of NHS Fife. In particular, where you manage employees, you will be expected to follow the local and national employment and HR policies and procedures. You will be expected to make sure that there are adequate arrangements for hospital staff involved in the care of your patients to be able to contact you when necessary.

*This timetable represents the type of work we would expect you to undertake. The precise timetable will be discussed to take into account departmental. This would include the possibility of extending the job by up to 2 EPAs if mutually acceptable.

Out of hours

The current on-call rota is 1:10. This post will cover 50% of the full time on call commitment. This is worked as predictable evening cover between 5 and 8 pm weekdays and 9 am and 5 pm weekend days. We currently outsource on call CT to an external company between 8 pm and 9 am Monday – Friday and 5 pm to 9 am Saturday and Sunday. “On call” cover for these periods, for IT failure, carries a 3% supplement. There is a secure network to allow reporting of scans from home. Out of hours interventional radiology is performed in Ninewells Hospital, Dundee.

- 9 - TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

GRADE AND SALARY Consultant £ 84,984 - £112,925 per annum (pro rata)

New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.

HOURS OF DUTY 50% of full time 20.00

SUPERANNUATION You have the option to join the NHS Superannuation Scheme, to participate in the State Earnings Related Pension Scheme or to take out a Personal Pension. Employee’s contributions to the NHS Scheme are Tiered based on your earnings and the employers contribution equates to 13.5 % of salary. Employees in the NHS Scheme are “Contracted-out” of the State Earnings Related Pension Scheme and pay a lower rate of National Insurance contributions. Employees who choose to participate in the State Earnings Related Pension Scheme pay the higher rate of National Insurance contribution. A Stakeholder Pension is also available. A Personal Pension is a private arrangement agreed with the pension provider that will be an organisation such as a Bank, Building Society or Insurance Company.

REMOVAL EXPENSES Assistance with removal and associated expenses may be given and should be discussed and agreed prior to appointment.

NHS Fife Board Headquarters

NHS Fife Board Headquarters is situated at Hayfield House, Victoria Hospital, Kirkcaldy. Strategic planning is an area function performed by the Board’s headquarters staff. The Director of Public Health is Dr Dona Milne.

Health Education

The Department of Health Education supports all health educators in the community, including medical staff, and runs the Board’s Resource Centre and Library.

PROVIDERS OF HEALTH CARE IN FIFE

Health care in Fife is provided by one Acute Service and three Health and Social Care Partnerships

Postgraduate Medical Education

There is a Fife Area Postgraduate Medical Education Committee with an Associate Director of Medical Education, Professor Morwenna Wood, in post. There are two Foundation Tutors, Dr S Mowbray and Mr A Ballantyne. Postgraduate Medical Centres are located at Victoria Hospital, Kirkcaldy and Queen Margaret Hospital, Dunfermline; both offer study and library facilities. There is an extensive programme of postgraduate courses, lectures and clinical meetings.

Further library facilities are provided in other hospitals and there is ready access to library facilities and postgraduate activities in Edinburgh. Many units maintain links with the University of Edinburgh and are involved in the Rotations Training Schemes at Edinburgh University, Vocational Training Scheme for General Practitioners and Rotational Training Scheme for Specialist Registrars (with NHS Lothian University Hospitals).

Undergraduate Medical Education

Fife has an ongoing Undergraduate Medical Education Programme in partnership with the Universities of Edinburgh and Dundee. Approximately 600 medical students attend for clinical attachment, in various specialties, throughout the academic year.

- 13 - Short clinical attachments and site visits in medicine, including elderly medicine, surgery, paediatrics and obstetrics are also organised for medical students from the University of St Andrews.

Kirkcaldy and Levenmouth Community Attachment Scheme (KLCAS) will bring 160 medical undergraduate students to the Randolph Wemyss Memorial Hospital from the University of St Andrews.

HOSPITALS

The area

Fife Hospitals serve a population of approximately 380,000. District General Hospital Services are provided at Dunfermline (Queen Margaret Hospital) and Kirkcaldy (Victoria Hospital). All acute inpatient services are provided on one site, the Victoria Hospital in Kirkcaldy. Queen Margaret Hospital, (QMH), is a dedicated Day Surgery and ambulatory care centre (diagnostic and treatment centre), along with day hospital, in-patient rehabilitation, intermediate care and continuing care beds. The nearest teaching hospitals providing regional services are in Edinburgh (Western General Hospital and Royal Infirmary) and Dundee (Ninewells Hospital).

Victoria Hospital

Victoria Hospital (VHK) is a large fully equipped District General Hospital with a major new build which opened in January 2012.

The hospital has approximately 600 beds and provides a full range of general hospital services to the residents of Fife. There is a critical care area consisting of an Intensive Care Unit and both Medical and Surgical High Dependency Units. Fully equipped endoscopy rooms operate throughout the week. The hospital includes the regional Obstetric Unit for the whole of Fife.

There is an excellent Radiology Service. In addition to general radiology there are facilities for digital screening, ultrasound, radioisotope scanning and DEXA scanning. Specialist Vascular Interventional facilities for nephrostomies, angiography and embolisation are available. This service is delivered by specialist interventional radiologists from Tayside and local consultants with an interest in drainages and biopsies. Modern multislice CT and 1.5T MRI .

d) Radiotherapy/Oncology

Oncology services are provided from the Western General Hospital, Edinburgh. Clinical Oncologists with special interests in most cancers hold large outpatient clinics each week. e) Theatres

There are 8 main theatre suites within the new build, including a dedicated fully staffed 24 hour emergency theatre. f) Library

As part of a well-equipped medical education centre. g) Regional Laboratory Service

The Fife Regional Laboratory is on site. The department provides a comprehensive service in haematology, immunology, histopathology, cytology, microbiology and clinical chemistry on a Fife-wide basis. The Microbiology Department provides an overall microbiological surveillance system throughout the region.

Queen Margaret Hospital

The hospital is now a fully equipped ambulatory and day care centre providing day surgery services for the whole of Fife. A full range of out-patient clinics are provided for the residents of West Fife. There are two fully equipped endoscopy rooms. In addition there are a range of elderly medicine services including a 30 place day hospital (supporting rapid community assessment, specialist clinics, early supported discharge and rehabilitation), a 60 bed assessment and rehabilitation unit, and continuing care beds.

(c) Radiology There is a full range of radiology services available, including a spiral 128 slice CT scanner and an MRI suite opened in 2008. The MRI scanner is currently being replaced. Mammography, fluroscopy and ultrasound scanning are also on site.

As part of a well-equipped medical education centre.

St. Andrews Community Hospital

This local hospital provides rehabilitation, transitional and long term care beds and outpatient facilities for North East Fife. Clinics provided are:

A theatre and recovery area is available for Endoscopy and Local Anaesthetics only.

Adamson Hospital, Cupar

This small friendly local hospital provides a range of surgical outpatient and day care services, as well as community GP and NHS long term care beds. St Andrews Community

- 16 - Hospital and have an outpatient dermatology service provided by NHS Tayside.

Cameron Hospital, near Windygates, Leven

This hospital is situated at Windygates, approximately 5 miles from VHK. The Sir George Sharp Unit houses the Fife Rehabilitation Service, which provides specialist rehabilitation for under 65s. There is a 12-bedded stroke rehabilitation unit, along with beds for both elderly general rehabilitation and interim care.

Other Hospitals

Other hospitals include , near Cupar and Whyteman’s Brae Hospital, Kirkcaldy, which provide mental health services. Randolph Wemyss Memorial Hospital provides day hospital facilities, as well as community based rehabilitation and 16 NHS long term beds. There are also GP beds at Glenrothes Hospital.
