Rev. 02/15 STATE OF DEPARTMENT OF TRANSPORTATION AND DEVELOPMENT SUPPLEMENT TO OVERSIZE/OVERWEIGHT PERMIT

1. GENERAL This special permit must be carried with the vehicle using same and must be available at all times for inspection by proper authorities. This permit is subject to revocation or cancellation at any time. The applicant assumes responsibility for and obligates himself to pay for any damages caused to highways, roads, bridges, structures or any other state-owned property while using this permit. Issuance of this permit is not assurance or warranty by the State or DOTD that the highways, roads, bridges and structures are capable of carrying the vehicle and load for which this permit is issued; nor shall issuance stop the State or said department from any claim which may arise for damage to its property. The applicant hereby accepts this permit at his own risk. The applicant agrees to defend, indemnify and hold harmless the Department and its duly appointed agents and employees from and against any and all claims, suits, liabilities, losses, damages, cost or expense including attorney fees sustained by reason of the exercise of the permit, whether or not the same may have been caused by negligence of the Department, its agent or employees. When required, by the permit, the vehicle and load for which the permit is issued shall be accompanied by a proper escort, State Police or otherwise, all at the expense of the user: and such other conditions or requirements as are herein imposed by the Secretary shall be complied with. This permit is issued pursuant to R.S. 32:380 thru 32:389 which is made part hereof by reference and of which applicant takes cognizance. By using this permit, permittee certifies that the information supplied by him contained in his application for this is correct; that he made application to induce the issuance of the permit; that he fully understands all the provisions and requirements thereof and of said R.S. 32:380 thru 32:389 and that he accepts all conditions and assumes all of the obligations imposed thereby. 2. TIME RESTRICTIONS Hours of operation are from sunrise to sunset, as determined by the National Weather Service. Unless otherwise excluded on the permit, vehicles and loads requiring a permit are prohibited from traveling at night, in inclement weather, and on certain designated holidays. The primary concern is the safety of the motoring public. Vehicles and loads traveling during inclement weather or at night must be equipped with all necessary safety lights. Permit loads may not cross any bridge spanning the River in the parishes of Orleans and Jefferson or be within 2 miles of such bridge from 6:30 – 9:00 a.m. and from 3:30 – 6:00 p.m., Monday through Friday. 3. SPEED LIMITS All permit loads are not to exceed 55 MPH. 4. INTERSTATE MOVEMENTS 5. Vehicles and loads over 16 feet wide may not travel on Interstate highways in Louisiana. 6. Vehicles and loads with Agronomic/Horticultural Permits (except Sugar Cane-Beginning August 1, 2012, the secretary shall not issue an annual Agronomic/Horticulture special permit pursuant to the provisions of this Section to any owner or operator of a vehicle hauling sugarcane who has not added an additional single axle on the sugarcane trailer for a total of six axles for the vehicle and trailer combination. R.S. 32:387.7 Act 462. The axle must be equipped with dual mounted tires lowered to the ground and functioning as a load bearing axle when hauling sugarcane.), Cotton Module Permits, Forest Management Equipment Permits, Forest Product Permits, House Movers Equipment Permits, Oil Field Equipment Permits, Refuse Permits, Refuse Waste Permits, Pleasure Craft Permits, Solid Waste Permits, and Timber Cutting/logging Equipment Permits are prohibited from traveling on Interstate highways. Oversize loads over 12 feet wide may not travel on the Interstate system through Shreveport, Monroe, Lake Charles, Baton Rouge, and from 7:00 – 9:00 a.m. and from 3:30 – 5:30 p.m., Monday through Friday. The restricted areas are Shreveport: from Interstate 220 on east side of Bossier City to Interstate 220 on west side of Shreveport. Monroe: Interstate 20 from the eastern city limits of Monroe to the western city limits of West Monroe. Lake Charles: All of Interstate 210. from LA 397 on the east to LA 108 at Westlake on the west. Baton Rouge: All of . Interstate 10 from Bluebonnet Boulevard on the east to the on the west. from O’Neal on the east to the junction with Interstate 10 on the west. New Orleans: All of . Interstate 10 from Paris Road on the east to Williams Boulevard on the west. This is not to include U.S. 90 – Business from its intersection with Interstate 10 in New Orleans westbound. 7. INCLEMENT WEATHER Vehicles and loads requiring a permit are prohibited from traveling during weather which is physically severe- such as extremely heavy rain, heavy fog, icy road conditions, heavy snow, or any continuous condition which creates low visibility for drivers or hazardous driving conditions. 8. PRIVATE ESCORTS Private escorts are required for all vehicles and loads: Over 12 feet wide and up to 16 feet wide Over 90 feet long and up to 125 feet long (State Police escorts are required for loads exceeding these dimensions) 9. RED WARNING FLAGS Flags not less than 18 inches square, are required for vehicles and loads which exceed the legal width. 10. WARNING SIGNS Vehicles and loads exceeding 10 feet in width, legal length or the legal rear end overhang must display signs with the wording “OVERSIZE LOAD”. All warning signs must be at least 7 feet long and 18 inches high. The background must be yellow and the letters black. Letters must be at least 10 inches high with a 1 inch brush stroke. “THIS FORM MUST ACCOMPANY ALL WIRE SERVICE, E-MAILED, FAXED, AND INTERNET PRINTED PERMITS.” LOUISIANA

PERMIT RESTRICTIONS

General - Permits are valid for one to three days. In general, vehicles and loads requiring a permit are prohibited from traveling at night, in inclement weather, and on certain designated holidays. Travel is permitted daylight hours only, unless time restrictions apply.

Serial numbers must be provided for construction equipment such as drag lines, dozers, back hoes, being moved on an oversize/overweight permit.

Vehicles must be registered for the highest gross weight which is 88,000 lbs. for 6 axles and 80,000 for 5 axles.

Insurance — Liability bond or certificate of insurance which insures any damage to state property must be posted at the truck permit office before permit can be issued. Bond amounts are $100,000 for a single trip, or $250,000 annually.

Weekend/Night Travel – In general, vehicles and loads requiring a permit are prohibited from traveling at night, in inclement weather, and on certain designated holidays.

Holiday – Permitted loads are prohibited from traveling on Saturday September 3, Sunday September 4, and Monday September 5, 2011 in observance of Labor Day.

Inclement Weather - Hours of operation are from sunrise to sunset, as determined by the National Weather Service. Vehicles and loads requiring a permit are prohibited from traveling during weather which is physically severe- such as extremely heavy rain, heavy fog, icy road conditions, heavy snow, or any continuous condition which creates low visibility for drivers or hazardous driving conditions.

Permit loads may not cross any bridge spanning the Mississippi River in the New Orleans area or be within 2 miles of such bridge from 6:30 – 9:00 a.m. and from 3:30 – 6:00 p.m., Monday through Friday.

Permit Information Required — Tractor and trailer information; description and serial number of load; gross weight; axle weights; overall dimensions; origin and destination; mileage and routes to be traveled; company customer number.

LA-3 12/11 Notwithstanding any other provision of law to the contrary, any combination vehicle with a gross weight greater than 120,000 pounds, but not in excess of 254,000 pounds shall be authorized a maximum tandem axle weight of 45,000 pounds and a maximum steering axle weight of 13,000 pounds, provided the spread between axle groups is a minimum of 12 feet and the spread between tires in a group is a minimum of 4 feet.

Note: Loads exceeding 232,000 pounds, but not greater than 254,000 pounds, shall be allowed statewide movement on the Department of Transportation & Development selected and approved routes, the majority of which are interstate highways only. However, those portions of their route from the load's origin to the National Highway System and that portion from the National Highway System to its destination shall be subject to the structural evaluation provided for in this schedule.

ESCORTS AND SIGNS

Labeling — Two signs with the wording ―OVERSIZE LOAD‖ are to be displayed at the front and rear of vehicles over 10' in width or exceed legal length or overhang. The sign must have black lettering (10" high, 1.5" brush stroke) on a yellow background, and must have dimensions of at least 7' x 1.5'.

Red/fluorescent orange flags, 18" square, must be mounted on all four corners and all extremities.

Escort Vehicles and Personnel — Escort vehicles must have the name and address of the escort company on each side of the vehicle, and must be registered annually ($10 fee for certification issuance). Escorts must carry proof of insurance and proof of valid driver’s license. Amber warning lights must be mounted at a minimum of 60" above the roadway. Escort vehicles must carry a 10 lb. BC dry or equivalent chemical type fire extinguisher, four 15-minute burning flares, and two fluorescent orange hand held flags.

Escort vehicles must have two flags placed on each side of the warning light sign.

A single escort must accompany vehicles over 12' in width and up to 16' on two-lane roads. The escort must lead on two-lane roads, follow on four-lane roads. A single lead escort must accompany vehicles over 12' in width but no wider than 16' on multilane roads. Also, a single rear escort must accompany a vehicle with dimensions over 90' in length but no longer than 125' in length.

State police escorts required for loads over 16' wide and 125' in length.

Travel through metropolitan areas may require city or parish permits and/or escorts.

Certification – Pilot Car Escort Certification is not currently required.

FINES

Posted Bridges — If a vehicle or combination of vehicles is observed crossing a bridge which has regulatory weight limits and the vehicle or combination of vehicles exceeds the posted weight limit on the bridge, the penalty will be: $100.

Escort Vehicles — If operating as an escort vehicle when required by OVERSIZE/OVERWEIGHT PERMIT - unable to pass required inspection or transporting load without an escort when one is required: the penalty will be: $100.00 and load impounded until proper escort is obtained.

Overdimension Only — Vehicles and divisible loads exceeding the legal limitations are impounded until the fine has been paid and the load has been made legal. Legal load violations are $100, and twin trailer violations are $100. In both cases, operators must make the load legal, and are still fined anyway.

Overweight Fines — The amount assessed for an overweight penalty will be for the violation with the greatest dollar value — whether based on gross vehicle weight, axle weight(s), or bridge formula. A $10.00 penalty will be assessed for each lesser violation(s).

If a vehicle and divisible load exceeds the legal limitations or DOTD regulations for axle weight or gross vehicle weight, these schedules will be used:

This chart is for over axle and over gross. The larger penalty is imposed with $10.00 for lesser of the infraction.

2. If a vehicle exceeds the legal maximum gross weight but not the legal maximum axle weight, the Overweight Penalty Schedule is used to assess the penalty.

3. If a vehicle does not exceed the legal maximum gross weight, but exceeds the legal maximum axle weights for any axle or axle groups, the penalties are then computed separately for each axle or group. The sum amount of which is the penalty assessed according to the Overweight Penalty Schedule.

4. If a vehicle exceeds both the legal maximum gross weight and the legal maximum axle weight, both penalties are computed from the Over Gross Weight Schedule, and the greater of the two is assessed as the penalty. When two (2) or more single axle or axle groups are overweight, the sum of the penalties of the overweight axles is compared to the penalty on the Overweight Penalty Schedule, and the greater of the two will be assessed. A $10.00 penalty is assessed for the lesser violation.

5. If the vehicle has a permit but is over the gross weight allowed by the permit, the fine is assessed from this schedule:
