Fee: $725 To enter the lottery for the CIT program please indicate below all sessions you are available to attend. We will notify you of your session by Sept. 10, 2021. The lottery is designed to give all campers an equal opportunity. Must apply by August 31, 2021. O June 26- July 2 □ CIT1 June 5-18 □ CIT2 June 19-July 2 □ CIT3 July 3-16 □ CIT4 July 17-30 PARENT/GUARDIAN AUTHORIZATION While Camp Ocoee will make every attempt to provide reasonable accommodations for mentally and physically challenged children , the camp will not accept children that are (1) of danger to Payment themselves, (2) of danger to others, or (3) a disruption to the normal activities making it unreasonably difficult for other children to enjoy camp programs. Any of the above reasons A $100 deposit is required for each session. will be grounds for dismissal from camp. A parent/guardian must discuss with the director any special conditions or circumst ances involving their child. This must be completed prior to registration so that we can advise you as to whether we can make a reasonable accommodation for your child. I approve this registration and certify that the camper is capable of an overnight camp experience. I grant permission for t he camper to participate in all planned camp activities Balance Due Date for all sessions: May 13, 2022 including out-of-camp trips by van or bus, hiking, horseback riding and/or boating. In case of an accident or illness, the YMCA is authorized to secure emergency medical treatment. Prudent attempts will be made to contact the camper’s parent immediately. I hereby give my permission to the medical personn el selected by the Camp Director to order X-rays, routine tests and treatment; to release any records necessary for insurance purposes; and to provide or arrange necessary rel ated transportation for my child. Account #_________________________ Exp Date__________ REFUND POLICY: I understand that $50 of my deposit is a registration fee that is nonrefundable under any circumstances. Notification after May 13, 2022 will result in the loss of the $100 deposit (per session, per camper). Date ______________ Cancellations made less than 2 weeks prior to the start of your respective session will forfeit all previous payments. Cancellations Summer 2022 Fee Increase To ensure that we have the most qualified role models leading our campers, we are increasing our weekly fee. Currently we are paying one of the lowest weekly rates compared to other YMCA camps ($185 per week). We are increasing our base weekly pay to $250 per week. After summer 2022 we will go back to our annual inflation increase. Notes: Extended Care: If you are unable to arrive by 10:30am (ET) on Check-out day. We are happy to care for your children until 12:30pm (ET) for an additional $20. □ Extended Care $20 Discounts: Deduct $40 for each additional child (2nd, 3rd, 4th) from the same household who is registered for a summer session. Deduct $40 for each additional week of camp an individual camper is attending Transportation: If your child is flying in and needs transportation to/from the Chattanooga airport (CHA) then arrangements M UST be made at least 2 weeks in advance with the camp office. Please indicate by checking here: □ $30 Check-In time for all sessions is 1:00-2:30pm (EST) on Sunday- Checkin is staggered based on the parent's last name. A-L will check in between 1pm-1:45pm, while M-Z will check in between 1:45pm-2:30pm. Check-out time for all sessions is 9:30-10:30am (EST) on Saturday.
