Post Full time Consultant in Anaesthesia with a special interest in cardiothoracic anaesthesia at Imperial College Healthcare NHS Trust. 2. Location This post will be based at The Hammersmith Hospital campus of Imperial College NHS Trust. 3. Accountability 3.1 In respect of the consultant contract, the post holder will be professionally accountable to the Divisional Director who is responsible for appraisal of the post holder. 3.2 : The post holder will have professional responsibility to the Medical Director and Director of Research and Development for clinical governance and research undertaken within the hospital. 4. Job Purpose 4.1 : To provide clinical services in Anaesthesia in accordance with clinical and legal requirements and in keeping with the international role of the Trust. 4.2 : This clinical role is divided between attending theatre and peri-operative assessment and post- operative care on the cardiac intensive care unit as part of the team based approach. Background to the Post The Hammersmith Hospital receives national and international referrals for diagnostic and therapeutic evaluations. Specialist surgical services based at the Hammersmith are Hepatobiliary, Cardiothoracic and Gynaecology including the West London referral centre for oncology. The Hammersmith, already a centre for renal failure surgery and renal transplantation expanded in 2005 to become the sole provider of these services in West London, coinciding with the completion of the renal medicine block with a new intensive care unit and is now the largest unit in Europe. Hammersmith Hospital provides regional services in hematology and bone marrow transplantation, a supra-regional service for hormone assay all are supported by the wide range of cutting edge imaging technology in the UK. Procedures requiring anesthesia are also performed in the X-ray department, MRI Suite, Cardiac catheter lab and the In Vitro Fertilization Unit, the largest NHS fertility unit in the UK. The Intensive Care Unit has ten beds and caters for an extensive range of clinical specialties including complex medical problems, specialized postoperative care and obstetric intensive care. Cardiac surgery has expanded with the merger of the St. Mary’s unit and patients are managed postoperatively in a newly designated 16-bedded cardiothoracic intensive care unit. The Department of Anaesthesia incorporates theatres, intensive care, day surgical services, obstetric anaesthesia, acute and chronic pain management and resuscitation. The Divisional Director, supported by the Clinical Director, manages the clinical Directorates within the Division. There is a consultant-led Acute Pain Service at Hammersmith Hospital. 4 Resuscitation Officers In August 2017 The critical care outreach & resuscitation team expanded their services to Hammersmith Hospital. The change will align the critical care outreach & resuscitation services cross-site, currently with clinical cover from 08.00 - 20.00 Monday to Friday. The cross-site service will continue to be covered by SNPs out of hours and at weekends. The team aim to: Deliver resuscitation training, as well as in-house Trust study days, and resuscitation equipment Follow up patients discharged from a Critical Care environments Respond to acute deterioration, patients with NEWS 7 or above, and emergencies The surgical specialities based on the Hammersmith site are Cardiothoracic surgery, Renal failure surgery (including transplantation), Urology and Gynaecology including major pelvic oncology, Endocrine and Hepatobillary surgery. Anaesthesia is also provided for catheter and angio suites, endoscopy and IVF units. There is a busy 12-bedded general intensive care unit run by a dedicated team of consultant intensivists working very closely with the anaesthetic department. The cardiac intensive care is separate and staffed by consultant anaesthetist/intensivists. List allocations change on a weekly basis, all our staff including the consultant body work flexibly depending on service needs. 3. Key Result Areas, Main Duties and Responsibilities 3.1 Provide High Quality Care to Patients in anaesthetics The post holder must be medically qualified and maintain GMC specialist registration. To develop and maintain the competencies required to carry out the duties required of the post. December 2018 2 To ensure prompt attendance at agreed direct clinical care Programmed Activities. To ensure patients are involved in decisions about their care and to respond to their views . December 2018 3 3.2 Research, Teaching and Training Where possible to collaborate with academic and clinical colleagues to enhance the Trust’s translational research portfolio, at all times meeting the full requirements of Research Governance. To provide high quality teaching to medical undergraduates and members of other health care professions as required by the Divisional Director / Clinical Director. To act as educational supervisor and appraiser as delegated by the Divisional Director / Clinical Director to ensure external accreditation of training post. 3.3 Performance Management To work with medical, nursing and managerial colleagues to ensure high performance in the following areas: Clinical efficiency e.g. LOS reductions, reducing cancelled operations and DNA rates. Quality of outcomes e.g. infection control targets, reducing re-admission rates Financial management e.g. identification, implementation and achievement of cost improvement programmes and participating in efforts to ensure services are provided cost effectively e.g. managing locum agency spend, monitoring and managing the drug budget to target, ensuring accuracy of clinical data for the team Operational efficiency e.g. day-case rates, waiting list activity and demand management. 3.4 Medical Staff Management . To work with colleagues to ensure junior doctors’ hours are compliant in line with EWTD and New Deal. To ensure that adequate systems and procedures are in place to control and monitor leave for junior medical staff and to ensure that there is appropriate cover within the clinical areas, including on-call commitments . To participation in the recruitment of junior medical staff as delegated by the Divisional Director / Clinical Director. To participate in team objective setting as part of the annual job planning cycle. To be responsible for the annual appraisal of all doctors in training, Trust doctors and non- consultant grades as delegated by the Divisional Director / Clinical Director/General Manager. 3.5 Governance . To review clinical outcomes in designated area using external benchmarking data where appropriate, to identify and advise variances to the Divisional Director / Clinical Director. Participate in clinical audit, incident reporting and analysis and to ensure resulting actions are implemented. To work closely with the Directorate, Patient and Public Involvement panels in relation to clinical and services developments as delegated by the Divisional Director / Clinical Director. Participate in ensuring NICE requirements are reviewed and implemented and monitored in the speciality areas. To ensure clinical guidelines and protocols are adhered to by junior medical staff and updated on a regular basis. To keep fully informed about best practice in the speciality areas and ensure implications for practice changes are discussed with the Divisional Director / Clinical Director. To role model good practice for infection control to all members of the multidisciplinary team. 3.6 Strategy and Business Planning . To participate in the business planning and objective setting process for the directorate and Trust where appropriate. To represent the Trust at appropriate clinical networks/other external clinical meetings, as delegated by the Divisional Director / Clinical Director. 3.7 Leadership and Team Working . To demonstrate excellent leadership skills with regard to individual performance, clinical teams, the Trust and when participating in national initiatives. To work collaboratively with all members of the multi-disciplinary team and Imperial College as required. To chair regular meetings for the specialties. To resolve conflict and difficult situations through negotiation and discussion, involving appropriate parties. Adhere to Trust/departmental guidelines on leave including reporting absence.
