The Dementia Information Prescription includes a list of key services, organisations and contact telephone numbers for Cherwell Oxfordshire where you may be able to access information and support. The role of the LPA may be to make decisions about your health and personal welfare that are in your best interests such as: where to live; the provision of day-to-day care; medical treatment and / or managing your financial affairs.
