1. Introduction1

Infectious and chronic lung diseases account for a large share of the burden of morbidity and mortality in Vietnam, including 8.7 percent of total disability-adjusted life years (DALYs) and nearly 12 percent of all deaths in 2017.2 Among all diseases, tuberculosis (TB) ranks tenth in terms of the number of deaths caused and twelfth in terms of the total years of life lost3. Multidrug-resistant TB (MDR-TB) is on the rise; in 2018, it was estimated that there were 8,600 cases in the country.4 Vietnam aims to eliminate TB by 20305 but still faces many challenges in accurately and rapidly diagnosing people with TB. Case detection is quite weak: in 2018, the health system detected 102,171 cases of TB out of an estimated 174,000 cases.6 In the same year, 3,187 cases of laboratory confirmed MDR- and XDR-TB were detected. Only 20 percent of tuberculosis cases were tested with rapid diagnostics at the time of diagnosis. Treatment is also not sufficiently effective: Among TB cases registered for care in 2017, the treatment success rate was 92 percent (including new and relapsed cases) and in 2018 the case fatality rate was estimated at 7.6 percent7. Lack of capacity for differential diagnosis, particularly between acute respiratory infections and TB, likely contributes to delays in treatment. Testing of presumptive cases of TB using GeneXpert, included to detect MDR-TB strains, is a cornerstone of the Vietnam Ministry of Health’s (MOH) strategy to eliminate TB. The GeneXpert test, which is performed by a GeneXpert machine, is a molecular test that diagnoses TB by detecting the presence of TB bacteria and tests for resistance to the drug Rifampicin. This procedure uses unprocessed sputum samples, produces results within two hours (which is substantially faster than drug susceptibility testing), and requires no additional technical equipment, which makes it especially suitable in contexts with limited laboratory capacity.8 Per the MOH’s strategy documents, Vietnam aims to test all presumptive cases of TB with GeneXpert by 2020, by increasing the testing capacity at district levels.9 The MOH’s TB program has and intends to continue to mobilize external financing, both loans and grants, from development partners (including the Global Fund and the World Bank) to procure a sufficient number of GeneXpert machines and cartridges to reach these goals. With TB diagnostics, including GeneXpert testing, having recently been included in the social health insurance benefit

To ensure that resources used to purchase GeneXpert machines are put to their most efficient use, especially considering the substantial cost of this equipment, the placement of future machines should be such that it maximizes the share of the unmet need for TB testing that is addressed by the investment. Optimization of GeneXpert placement in Vietnam was previously carried out on a smaller scale, for 13 provinces, to inform the placement of 35 GeneXpert machines that will be purchased as part of a new World Bank-financed project (loan)10, 11. This study extends that analysis to the national level, including all of Vietnam’s provinces. The objective of this national-level analysis is to provide guidance to the Government of Vietnam and its development partners in deciding where future GeneXpert machines should be located to best achieve Vietnam’s goal of testing all presumptive TB cases by GeneXpert.

2. Methods

The approach is to conduct an optimization analysis to estimate where to deploy new GeneXpert machines to best meet the demand for TB diagnostic testing across all districts in Vietnam. The solution depends on the geographical distribution of district-level health facilities to which GeneXpert machines can be allocated, the geographical distribution of TB diagnostic testing needs across all districts in Vietnam, the population’s ability to travel to health facilities to obtain a TB test, and the testing capacity of GeneXpert machines in the various facilities. The data and modelling approach are described below.

2.1 Data

Geographical distribution of health facilities potentially eligible for GeneXpert machines The potential district-level locations for new GeneXpert machines were identified using an unpublished health facility database12 which includes information on geographical location (GPS coordinates) and type of facility (e.g. hospital, district health center). Almost all districts have a district health center, which is generally combined with a district hospital, but in some localities is a separate facility. Vietnam has 711 districts, of which 12 districts are islands and have been excluded for that reason, leaving 699 districts. Of these remaining 699 districts, 121 districts already have facilities with one or more GeneXpert machine(s) and are therefore not eligible for a new machine.

However, the existing GeneXpert capacity in these districts was considered in the optimization, as these facilities could perform more tests without requiring new machines. This would in turn reduce the need for new equipment in nearby districts. As such, the capacity of facilities with one or more existing GeneXpert machine(s) was first

The remaining 578 districts with no GeneXpert machine at any facility were considered eligible to receive one new GeneXpert machine per district. In each of the 578 eligible districts, a single candidate facility, generally the district health center, was prioritized to receive one new GeneXpert machine per district in priority order ( Figure 1).

Geographical distribution of the need of diagnostic TB testing Information is needed to estimate the number of TB tests that must be performed in each district in order to find all individuals with TB who are still undiagnosed in each district. This estimate has many parts to it: First, for each of the 578 districts which have candidate facilities to potentially receive a new GeneXpert machine, the following information was used to estimate the number of individuals who are still undiagnosed in each district: • Location of the districts, obtained from the Database of Global Administrative Areas (GADM)13; 14 • Population size, defined as 푁푖 people in district 푖, obtained from the 2014 Intercensal Population Survey estimates, with corrections made to address district splits, merges, and renaming that has subsequently taken place (and has been recorded in various legal documents);

• TB detection rates for 2018, 푟푖, from the Vietnam National Tuberculosis Control Program (NTP) at the National Lung Hospital under the Ministry of Health, with provincial detection rates applied equally to every district within each province;

• Estimated patient diagnostic rate (PDR) for adults aged 15 years and older, 푝푖 with regional rates applied equally to every district within each region15; 푑 • Estimated number of individuals diagnosed with TB in 2018 in each district 푁푖 = 푁푖 ⋅ 푟푖; 푢 푢 푁푖 1− 푝푖 • Estimated number of individuals with TB who are still undiagnosed in each district 푁푖 from 푑 = 푁푖 푝푖 (assumes that the proportion of people diagnosed and undiagnosed with TB remained equal to the last reported PDR).

Second, the number of tests required per positive TB diagnosis was estimated. It was calculated by the NTP, using the 2017 TB prevalence survey as a baseline with corrections applied to account for improvements in detection through the use of GeneXpert testing. This resulted in an estimated 푐 = 22 GeneXpert tests required to detect one positive case of TB.

Finally, the TB diagnostic testing demand (i.e. number of tests required) for each district was estimated as: 퐷푖 = 푢 푐 ⋅ 푁푖 . The geographic distribution of the demand for TB testing is shown in Figure 2.

Accessibility of health facilities (i.e. travel time) The share of the need for TB testing that can be met depends also on the ability of people in need of TB testing to travel to facilities, considering the road network and infrastructure. The time to travel from each district to each health facility was estimated by taking the (geographic) centroid of each district and using Open Source Routing Machine (OSRM)16, 17 to calculate the driving time between the centroid of the district and the health facility’s GPS coordinates. OSRM estimates the best route and travel time between locations given the road network assuming an average speed. These driving times gave an indication of the relative accessibility of health facilities.

The accessibility function for the proportion of people in district 푖 who can access facility 푗, depending on travel time indicator 푡푖푗 as given by OSRM, is as follows:

Depending on travel time 푡푖푗 from district 푖 to facility 푗, only 푓(푡푖푗) ⋅ 퐷푖 of the district’s demand 퐷푖 can be met by facility 푗.

TB diagnostic testing capacity of existing and new GeneXpert machines GeneXpert machines are seldom able to be used at their maximum technical capacity because machine capacity in practice also depends on the capacity of facilities and their staff to effectively use and maintain GeneXpert machines, how many machines each facility could feasibly operate, and the testing sensitivity of machines when placed in a certain health facility given the operations of that facility.

For existing machines, the diagnostic testing capacity of existing machines, both at maximum capacity and in practice (actual use), is based on a recent report by Nguyen et al.18

For new machines, it is assumed that they will be GeneXpert IV machines19. The GeneXpert IV machine contains four modules which can each be run an average of three times per day for approximately 270 days per year, i.e. 5 days per week for 6 hours per day20. This number was then discounted by 20% to account for module failure rate, repair time, insufficiency in cartridge supply, etc. Therefore, the maximum testing capacity 훼 of each new GeneXpert machine was set to 훼 = 4 ⋅ 3 ⋅ 270 ⋅ 0.8 = 2,592 tests per year. To account for GeneXpert machines being used only at a proportion of their full capacity, the maximum testing capacity 훼 is scaled by a factor 훽 ∈ [0,1]: 훽 = 1 if GeneXpert machines operate at their theoretical maximum capacity and 훽 < 1if GeneXpert machines operate below maximum capacity. Based on the collected data, it is possible to determine the actual average capacity as observed in practice before running the optimization. This derived value of 훽 is then used for all machines throughout the optimization.

2.2 Optimization analysis

We modelled the GeneXpert allocation problem as a linear integer optimization problem that used the data described above to calculate the optimal allocation of 푀 GeneXpert machines that will maximize the total satisfied demand for diagnostic TB testing.

We conducted this optimization analysis with a ‘Gurobi 8’ algorithm21 in Python 3.722.

2.3 Caveats and data limitations

Because district-level population estimates were used to inform the model, the distances between people needing to be tested and health facilities were calculated from the geographic centroid of each district. An alternative would have been to generate commune-level population estimates by distributing the district population to each commune according to the commune’s population share from the 2009 census. However, due to splitting, merging, and renaming of communes since 2009, it was not possible to map the 2009 census population sizes to the 2019 communes. New, up-to-date commune-level estimates will be available following the completion of the 2019 census data processing.

There were an additional 5,647 TB cases detected at central hospitals. As these cases may have been hard to detect or may have been XDR-TB cases with the province of origin unknown, it was not possible to assign these cases to a given province. As such they have been excluded from this assessment of need for GeneXpert testing and as a result the need for GeneXpert testing in Vietnam may have been slightly underestimated in this study.

In addition, the optimization results are sensitive to assumptions made about the willingness of people to travel to a health facility given the travel time and the capacity of new GeneXpert machines (both maximum and likely actual). The analysis is also sensitive to the assumption that individuals will travel to the nearest district-level facility with a GeneXpert machine in order to be tested. It may be the case that individuals travel to the most convenient or preferred facility – which may be at the commune-level or district-level – and then their sputum samples are sent to the district-facility through a network of sputum referral and testing (as has been implemented in some locations of Vietnam23). This alternative practice was not modelled. However, it could be modelled using an accessibility function analogous to the one used in this study, providing better accessibility between commune-level and district-level health facilities.

3. Findings

3.1 TB testing capacity of existing GeneXpert machines

The number of tests performed across Vietnam in 2018 with existing GeneXpert machines amounts to 1.3% of the total estimated need for TB diagnostic tests. According to current utilization rates, existing GeneXpert machines are being used substantially below their maximum capacity. We speculate that the low utilization results are due to low testing cartridge stocks and limited trained human resources, leading to rationing testing to detecting MDR-TB in TB-positive cases rather than implementing the protocols calling for widespread

GeneXpert testing for all presumed TB cases. Also, some existing machines are not yet being used at all because they were only recently acquired.

It is estimated that if existing machines were used at their maximum testing capacity, then the current number and distribution of GeneXpert equipment could meet up to 30% of the total TB diagnostic testing demand in Vietnam. Figure 3 shows the share of the total TB testing demand in each district that is met, if current machines are utilized at their current and maximum capacity, respectively.

3.2 Optimal allocation of new GeneXpert machines

With the current number of machines only able to meet 30% of the estimated need for TB diagnostic testing, even if operated at maximum capacity, new machines are needed and, in the interests of efficiency and access, should be optimally allocated.

There are 175 existing GeneXpert machines located in 125 health facilities in 121 districts across Vietnam. For this analysis, it is assumed that these existing machines cannot be moved. The capacity utilization of these existing machines was first maximized (to a maximum of 2,592 tests which were assumed could be performed per machine per year) such that the additional tests could be conducted using machines not operating at maximum capacity, before successively allocating any new machines. Of the remaining 587 non-island districts without a GeneXpert machine at any facility in the district, it is assumed that only one new machine per eligible district would be allocated to each of the single health facilities prioritized in each eligible district. The optimization generated a list of candidate facilities in order of prioritization for new GeneXpert investments as shown in Table 1 of the Annex.

If GeneXpert units are invested in order of priority, the number of additional tests that could be performed per machine would follow the curve in Figure 4. The x-axis shows the number of GeneXpert units added to the health system in order of decreasing priority of candidate facilities (one facility per district that doesn’t already have any facility with a GeneXpert machine in that district) as determined by this optimization exercise. The y-axis shows the additional total TB tests that could be performed per machine (one data point per machine), which decline as more machines are put in place in priority order to satisfy demand, leading to surplus capacity. The plateau at the top left shows that the first 136 new machines are expected to achieve near maximal capacity (2,592 tests per year) of annual tests per year above 2,500. For the next 63 machines, the additional TB tests that are estimated to be performed falls off sharply with the number of extra annual tests dropping from 2,500 to 2,000 tests per year per machine. For the next 304 machines added, the total additional annual tests that are anticipated to be performed decline from 2,000 to 500 tests per year. After the 502nd GeneXpert unit, total additional tests per unit per year are expected to fall below 500 tests per year.

The results of allocating different theoretical number of machines – 200, 300, 400 and 500 – according to this prioritized ranking of facilities, and with the assumption that they will operate at maximal capacity 훽 = 1, are illustrated in Error! Reference source not found.. These allocations yield a 48%, 66%, 79% and 81% coverage of the estimated need for TB testing, respectively.

However, as discussed in section 3.1 and shown Figure 3, current average utilization rates of the existing GeneXpert machines are at only 23% of their maximum capacity, with 103,146 tests being conducted per year

9 10 across Vietnam as opposed to the estimated 455,420 tests per year that should be possible with the current distribution of GeneXpert equipment under our maximum capacity assumptions.

To account for this, the above computation was repeated with reduced utilization (훽 = 0.23). The results, shown in Figure 6, starkly illustrate how the difference between an optimal allocation of machines and a random allocation of machines changes when machines are assumed to be operated only at current utilization rates as opposed to maximum utilization rates. At maximum capacity, the optimal allocation outperforms the random allocation, regardless of how many machines are allocated. However, at current capacity, there was only a marginal difference between the optimal and random allocations.

This suggests that, unless the rate of use of existing and new GeneXpert machines for TB diagnostic testing is substantially increased, there is little benefit to optimizing the allocation of the machines: overall testing will increase by deploying new machines to any district which does not have a GeneXpert machine. This is because very few machines would reach their full capacity.

4. Conclusion and recommendations

This analysis shows that, when machines are used at full capacity, geographically optimizing the location of additional GeneXpert machines results in a substantially higher share of TB testing need being met than would otherwise be the case. Geographical optimization, thus, results in a more efficient investment decision by getting more value-for-money out of each GeneXpert machine purchased.

Given the decision to purchase a specific number of new machines, the analysis provides guidance on where such machines should be placed. Machines should be placed in the priority order shown in Table 1 in the Annex. While

10 11 the model optimized the placement of machines (given a particular number of new machines to be placed) rather than the number of machines needed, the drop-off in the number of tests performed per new machines also provides some guidance in thinking about how many machines should be purchased. Within the optimization, the capacity of the 125 facilities that already have a GeneXpert machine was maximized to a total of 2,592 tests per machine per year, with the number of additional tests recommended for new GeneXpert machine allocation remaining above 2,500 for 136 additional machines. Beyond the 304th new machine, the modeled capacity to conduct extra TB tests falls below 500 each year.

However, at current rates of utilization, there is little difference in the share of TB testing need that is met when new machines are allocated optimally than when they are allocated randomly. Therefore, to fully realize the benefits of investing in new GeneXpert machines (and the optimization of their placement), the utilization (capacity) of both existing and new machines should be increased.

Increasing the utilization (capacity) of GeneXpert machines will, in turn, require complementary investments and interventions. First, interventions are needed to improve case-finding. This includes additional training of health workers to improve their skills and activities related to case-finding and referral, as well as interventions to stimulate the demand for testing, e.g. enhanced awareness campaigns. Second, on the supply-side, it needs to be ensured that sufficient cartridges are available, that warranties are purchased for new machines, that there is reliable internet connectivity (for results to be uploaded and stored in national electronic TB registries), and that workers that trained in the operation of the GeneXpert machines are available at facilities. Effective implementation of policies that allow recurrent expenditures associated with GeneXpert testing to be fully covered by social health insurance will help to ensure the funding needed for this. Taken together, these demand- side and supply-side measures will help to reduce the large gap between current and potential diagnostic testing levels, regardless of how many new GeneXpert machines are purchased.

This analysis is not without its limitations. Data and methodological limitations were discussed in detail in section 2.3. One limitation is that travel time had to be calculated using the distance from the geographic centroid of each district to the health facility since only district-level population estimates were available. Also, the various assumptions made by the model – including about people’s willingness to travel various distances to facilities, the maximum and actual testing capacity of new machines, and that people (rather than sputum samples) will be referred for GeneXpert testing – may not in fact, hold in practice.

It could also be argued that by choosing an optimization function that focuses on the unmet need for TB testing, the analysis ignores the many other types of diseases and clinical conditions24 that can be diagnosed using a GeneXpert machine (if those disease-specific cartridges were supplied) and for which the geographical distribution may be different from TB. If these other diseases and conditions were taken into consideration, then the optimal placement of GeneXpert might be different. However, right now, in government health facilities, TB is the only disease for which GeneXpert testing is currently mandated (and financed) and, therefore, optimization for this single disease seems appropriate.

As Vietnam seeks to makes progress towards its goal of having all presumptive TB cases tested by GeneXpert at the district level, the need for TB testing will increase substantially. Geographic optimization of the placement of new machines will help to ensure that resources are used more efficiently. Therefore, it is recommended that the placement of new GeneXpert machines follow the prioritization that emerges from this study. However, for the benefits of optimization of the placement of new machines to be beneficial, increasing the utilization capacity of both new and existing machines closer to the maximum is needed. In this regard, it is further recommended that any new GeneXpert machines, whether financed by government or by donors, be accompanied by a budget
