Ermöglicht wird dies durch eine flexible Verknüpfung aufeinanderfolgender Behandlungsabschnitte. Das Verfahren stellt im Interesse der Patienten die gezielte und unmittelbar weiterführende Behandlung gesundheitlicher Einschränkungen im ambulanten oder stationären Rahmen einer medizinischen Rehabilitation sicher. Die Überleitung von der stationären Krankenhausversorgung in die Anschlussrehabilitation kann dabei ohne vorherige Einbindung der Leistungsträger durch das Krankenhaus gemeinsam mit dem Patienten geplant und organisiert werden. Unter Berücksichtigung der medizinisch-technologischen Entwicklung und im Einklang mit hohen Qualitätsstandards gewährleistet der so beschleunigte und entbürokratisierte Zugang zur medizinischen Rehabilitation eine nahtlose Versorgung der
