Core Psychiatry Training in South East Scotland

These Locum Appointment for Training posts will be recognised training posts on the South East of Scotland Core Psychiatric training scheme. They are available for 6 months. As such, any person appointed as a LAT on the scheme will have access to all the training opportunities described below for the 6 month time limit of the post. The posts which are advertised are:

1) General Adult Psychiatry at Straheden Hospital with Dr Gandhi and Dr Dasgupta

2) Old Age Psychiatry at St John’s Hospital with Dr Lindsay

3) Intellectual Disability Psychiatry at , with Dr Alkanti

The South East of Scotland Deanery covers a wide geographical area and placements are available within the following Health Board Regions: * Lothian (including East, Mid and West Lothian) * Borders * Fife.

Core training takes place over 3 years in order to reach competencies to apply for Advanced Training.

During Core Training, trainees will normally undertake six 6 month posts, although longer (12 month) placements are arranged for General Adult Psychiatry posts. It would be expected that trainees would work in two or more hospital bases during Core training to gain a breadth of experience. Normally the first year will be spent in one hospital, with rotation to other hospitals in the second and third years of training based on the educational needs of the trainee.

Trainees will spend a minimum of 12 months each in General Adult Psychiatry, 6 months in Old Age Psychiatry and a further 6 months in subspecialty placement (Child or Learning Disability Psychiatry). Remaining placements may include Addictions, Forensic, Liaison, Psychotherapy, or Rehabilitation Psychiatry, depending on the trainee's requirements.

Trainees are expected to participate in the half day Psychiatry Curriculum Course at the Royal Edinburgh Hospital in preparation for their Royal College examinations, to undertake audit (or research) projects and to contribute to local teaching. Teaching sessions for trainees, including journal club presentations, take place on Wednesday mornings.

Trainees are also expected to attend the Psychotherapy Foundation Course, which takes place on Wednesday afternoons. There is also a Special Lecture Series, which takes place on Wednesday afternoons during term time. Research, audit, teaching and management opportunities South East Scotland offers a diverse and rewarding training in Core Psychiatry, with access to a large range of specialties during the three year scheme, and supervision by an enthusiastic group of trainers. There is also excellent academic support for trainees, and several trainees have recently progressed to academic career streams towards the end of their core years. South East Scotland also has very high pass rates for both the two MRCPsych written papers and the CASC clinical exam. Since August 2014 we have been able to create a brand new core training post which has 4 dedicated sessions for research. This point is well supported by the local academic department, and the aim is for this arrangement to lead to the production of a piece of published research. Where is the training delivered? This is a post in General Adult Psychiatry based at Stratheden Hospital and providing services to two busy GP practices in Glenrothes (North Glen Medical Practice and Markinch Health Centre). The duties include both outpatient and inpatient work. The doctor would be a member of multidisciplinary team and would work closely with other health professionals when managing patients. The doctor would be expected to participate in a day time Stratheden Hospital duty rota with other junior doctors covering for emergencies on site. There would also be cross cover between junior doctors in Glenrothes sector. The doctor would be expected to join a Fife-wide out of hours duty rota. First and Second Year trainees will be expected to attend the weekly Psychiatry Curriculum Course teaching on Wednesday mornings. They will also be able to attend regular journal club and case conference meetings. Psychotherapy case based discussion groups will be held on Wednesday afternoons.

Core Objectives

Trainees will be expected to demonstrate achievement of the 19 competencies set out in the 2013 Competency Based Curriculum for Core Specialist Training in Psychiatry (see appendix for list of Learning Outcomes). West Lothian has a population of 172,000 with 23,500 people aged over 65 years (General Register Office for Scotland). This is a busy hospital based post with opportunity to see a range of patients and for working in a variety of clinical settings. 1 – Introduction: This post sits within the Fife Learning Disability Service and is managed by the Fife-wide Division of the Integrated Health & Social Care Partnership (one of three operational management units within the Fife Integrated Health & Social Care Partnership, the others being the West and East Divisions). The Region of Fife is bounded in the north by the Firth of Tay, in the east by the North Sea and in the south by the Firth of Forth. The Region spans an area of 130,700 hectares and has a population of approximately 370,000. There is a highly developed agricultural part in east and north-east Fife, and in the west there is an extensive cross-section of highly skilled and scientifically orientated industry. The largest towns are Dunfermline, Kirkcaldy and Glenrothes. The cathedral city of St Andrews is the seat of Scotland’s oldest university and the UK’s third oldest university. Fife is an area of considerable scenic and historical interest. The usual range of sporting facilities are available locally including golf, swimming, fishing, curling, football, rugby, sailing, motor sport and gliding. The Cairngorm Mountains are within easy reach providing access to skiing, mountaineering, orienteering and salmon fishing. Wide ranges of cultural activities are available in Fife, and in the cultural centres of Edinburgh and Glasgow. The main urban and leisure centres of central Scotland are within easy reach and there are excellent air, rail and motorway links to the major centres in the UK. Edinburgh Airport is within easy reach by road. Fife has a rising birth rate and overall population increase particularly in the west. This is mainly due to the expansion of businesses around Edinburgh and an increasing commuter belt.

2 – Organisation details:

NHS Fife NHS Fife provides primary, community, mental health and hospital services. Mr Paul Hawkins is Chief Executive and Dr Christopher McKenna is Medical Director. Dr Seonaid McCallum is Associate Medical Director in the Fife Integrated Health & Social Care Partnership and Dr Robert Cargill Associate Medical Director in the NHS Fife Acute Services Division. The NHS Fife Board is situated at Hayfield House, Victoria Hospital, Kirkcaldy. The Director of Public Health is Dona Milne. Fife Health & Social Care Partnership The NHS Fife Learning Disability Service is operationally managed within the Fife-Wide Division of the Fife Health and Social Care Partnership following integration of Health and Social Care services. Mr Michael Kellett is Director of the Fife Health and Social Care Partnership and Ms Julie Paterson is Divisional General Manager of the Fife-Wide Division. 3 – Local Working Arrangements: Adult Learning Disability Services in Fife The Learning Disability Service in NHS Fife has undergone a period of significant change following the completion of the discharge programme from the traditionally long stay inpatient model of care to the development of community based services supported by a new Assessment and Treatment Facility and a local forensic unit. There are 12 mixed sex assessment and treatment beds (Mayfield) and 8 non secure male forensic beds (Levendale) for Fife patients with Learning Disability. In addition there are 10 low secure male forensic beds (Daleview) for patients with Learning Disability from the health boards in the Managed Care Network (MCN) area (Fife, Lothian, Forth Valley and Borders). There is also a 2 bedded unit (Tayview) which currently supports Fife patients with forensic needs and challenging behaviour. All these services are based on the Lynebank Hospital site. The NHS Fife Learning Disability Service has a lead clinician and medical care is currently provided by consultants (4.2 wte) and two specialty doctors (1.6 wte), with support from a core psychiatry trainee and a GP trainee, and intermittently a higher trainee in Psychiatry of Intellectual Disability, delivering the services to Lynebank Hospital and the community in Fife. Mrs Margaret Pirie is Service Manager of the NHS Fife Learning Disability Service. The service has traditionally been an adult based service but has managed within its caseload a number of children with a Learning Disability. There are also close working relationships with colleagues within both Social Work and various care agencies and the Care Programme Approach is widely used to ensure consistency of care. All substantive Consultant Psychiatry staff in the department are employed as part of the South East Scotland Learning Disability Managed Care Network (MCN) hosted by NHS Lothian, which allows for sharing of resources, peer support and Continuing Professional Development for medical staff in the service. The MCN was set up to share resources regarding those adults with a learning disability with additional Forensic problems, ‘challenging behaviour’ and Autism Spectrum Disorder. The focus of the Fife Learning Disability Service is on the mental health, behavioural and forensic needs of the patients although provision of an appropriate level of physical healthcare to inpatients is also included in the service.

General Community Learning Disability Services for Adults The general Learning Disability Service in Fife in the community is divided into three sectors – West Fife, Central Fife (Glenrothes and Kirkcaldy) and East Fife (including Levenmouth). Each sector has a multidisciplinary Community Learning Disability Team comprising professionals working in psychiatry, nursing, psychology, occupational therapy, speech and language therapy, physiotherapy, dietetics, podiatry and pharmacy.

Each Community Learning Disability Team provides specialist, high quality healthcare services for people with a learning disability. In providing such specialist care, each team aims to:

 Deliver effective, specialist healthcare using evidence based practices  Support General Practitioners and Primary Healthcare teams to identify and meet the health needs of people with learning disability  Work with Community Health Partnerships and Social Services to provide co- ordinated services  Facilitate access to primary and secondary healthcare services  Encourage and facilitate a healthy lifestyle  Provide education and advice to clients, their families, carers and other professionals.

The Fife Learning Disability Service also employs a number of senior specialist nurses in the fields of Challenging Behaviour, Acute Services and Primary Care Liaison, Epilepsy and Child to Adult Transition.

Forensic Learning Disability Services The Fife Forensic Learning Disability Service (FFLDS) provides a Fife-wide service to adults with Learning Disability and forensic issues. The FFLDS is multi-agency and multidisciplinary and forms part of the NHS Fife Learning Disability Service. The service includes professionals working in psychiatry, nursing, psychology, occupational therapy, speech and language therapy, physiotherapy, dietetics and podiatry. The service liaises closely with social work and social care services providing support to service users (currently employed by Fife Council and Richmond Fellowship Scotland).

The FFLDS objectives include:

 Assessment and management of risk with promotion of defensible decision making in forensic contexts  Provision of treatment, support and care to individuals with learning disability and forensic needs, reflecting the nature of risks identified  Promotion of individual rights, social inclusion and quality of life whilst recognising the importance of community safety  Promotion of inter-agency and inter-disciplinary co-operation in the management of risk

The FFLDS aims to achieve these objectives via:

 Comprehensive risk assessment, risk management and review  Commissioning, supporting and monitoring care packages  Therapeutic interventions (including offence focussed interventions)  Identifying, supporting and monitoring relevant structured occupational and leisure activities  Consultancy and advice  Liaison with other agencies including via Multi-Agency Public Protection Arrangements

Services for Children with a Learning Disability in NHS Fife Services employ a ‘tiered stepped approach’ to service provision allowing children to be referred to Specialist Services when needed and ensuring professionals work together in providing co-ordinated care for children. There is a Core Community Team which accepts referrals for children with Learning Disability and additional persistent social, behavioural, mood and developmental difficulties which are causing concern and interfering with daily living/ access to usual activities. The team can also provide advice and additional/intensive consultation, assessment, intervention and support for those children and adolescents with more complex needs and/or challenging behaviours.

Description of Post The LAS on attachment with the Learning Disability Service in Fife will work in the following areas: 1. Mayfield Ward, a 12-bedded Acute Assessment and Treatment Unit for adult patients with a learning disability and additional mental health problems and/or challenging behaviour, under the clinical supervision of Dr John Russell, Consultant Psychiatrist.

2. The Central Fife Community Team for People with Learning Disability (CTPLD) under the clinical supervision of Dr Swaraj Alkanti, Consultant Psychiatrist.

3. Within the Fife Learning Disability Service, cross cover for CT and Specialty Doctor colleagues on leave / study leave.

4. The LAS will be on call on the CT/GPST/FY2 shift rota in Fife as a member of the Unscheduled Care Assessment Team for psychiatry, which involves cover outside normal working hours of 9am to 5pm Monday to Friday, and also cover for emergency psychiatry assessments in normal working hours (9am to 5pm Monday to Friday) on a 1 in 19 basis.

The LAS will have a mobile phone and access to a computer. He/she will also have dedicated secretarial support and digital dictation is available for administrative tasks.

Core Objectives There is opportunity to demonstrate achievement of the 19 competencies set out in the 2013 Competency Based Curriculum for Core Specialist Training in Psychiatry (see appendix for list of Learning Outcomes). Achievement of these competencies can be demonstrated by use of Work Place Based assessments and supervisor overview.

Range of Experience Available 1) The LAS will gain experience in the assessment, treatment and management of mental health problems (and ‘challenging behaviour’) in adults with mild, moderate and severe degrees of intellectual disability.

2) The LAS will gain experience working in a range of clinical settings i.e. in an acute inpatient ward (Mayfield Ward) and the community (in clinics as well as home visits). The LAS will also be expected to provide cross cover for Levendale, Daleview and Tayview wards at Lynebank Hospital when other non-consultant grade staff are away.

3) The LAS will also be able to gain experience in the assessment of capacity as well as of Autism, ADHD and personality issues.

4) The LAS will also have opportunity to gain experience in specialist areas of working with adults with a Learning Disability such as in forensic problems and epilepsy.

5) There will be the opportunity for the LAS to gain experience in working with Social Work colleagues as well as GPs, families, carers and a range of care providers.

6) The LAS will have the opportunity enhance their formulation skills and to work systemically.

7) There will also be opportunities to gain more understanding of the use of the Scottish Mental Health and Adults with Incapacity Acts in adults with a learning disability.

8) The LAS will also have opportunity to gain knowledge of the use of Adult Support and Protection Guidelines and legislation.

9) The LAS will be encouraged to participate in MDT meetings and also gain knowledge of the roles of the various members of the LD Team – e.g. LD Nursing, Psychology, SaLT and OT.

On Call Commitment: Participation as 1 of 19 doctors to a mixed shift and on call rota as duty psychiatry doctor to Fife Unscheduled Care Assessment Team (UCAT), covering all psychiatry specialties. This involves Fife-wide cover providing emergency psychiatry assessment input as part of UCAT and cover to psychiatric hospitals in Fife. Other Rota Commitments: Cross cover for CT/ Specialty Doctors when on-call/rest periods/annual leave/ study leave.
