How to Apply:
o Select an eligible Energy Star® smart thermostat model from the list on reverse page o Purchase the thermostat and have a licensed HVAC contractor install the unit between March 1, 2018 and December 15, 2018 (summited to MID no later the December 15th) o Complete the application – application must be signed by the person whose name appears on your MID bill o Attach an itemized “paid in full” copy of receipt for the thermostat and the installation o Mail application package to MID – applications must be received by MID on or before December 15, 2018 to qualify MID Energy Services Department PO Box 4060 Modesto, CA 95342
