Please note: SILVER SNEAKERS members need to renew as well. Membership IS required to participate at the Mentor Senior Center. All memberships expire Dec 31 of each year regardless of when the membership began. You may re- new in person at the desk, on the phone with a credit card or through the mail with a check or credit card. Anyone 55 or older can join the Senior Center with full privileges. $7 residents, $10 non-residents FREE for Silver Sneakers. MEMBERSHIP RENEWAL EXPRESS Want to renew quickly? Stop in to the Membership Renewal Express from Jan 2 – 24 9 am – 3 pm in the lobby. Submit payment and completed renewal form (available at the front desk & on page 16) for quick and easy renewal at this dedicated work station. Want to avoid the lines? Simply complete the renewal form and submit credit card info or check for payment. Residents and nonresidents alike are welcome to join the Mentor Senior Center. Anyone 55 or older can join the Senior Center with full privileges. CONFIRMATION AND CANCELLATIONS A receipt and/or course confirmation will be emailed to you following registration. If a course is can- celed by the City of Mentor, you will be notified by phone or email. If the City cancels a course, we welcome you to transfer to another class within the same season, or receive a full refund. It takes two to three weeks to process a refund check. Credit card payees will receive credit on their account with- in a few days. COURSE REFUNDS Please choose your classes carefully. Full refunds will only be granted if a class is cancelled by the City of Mentor. There is a $10 administrative fee for classes cancelled or changed by the customer. Refunds will only be granted for medical reasons and will require a doctor’s note. No refunds will be granted less than 1 week prior to the start of the program. Participants assume risk of change in per- sonal affairs or health. OVERNIGHT BUS TRIP REFUNDS Please choose your classes carefully. Full refunds will only be granted if a trip is cancelled by the City of Mentor. An administrative fee of $25 will be assessed per person for any changes made to trip registrations. Changes will include participant cancellations, refund requests, transfers and/or room changes. No changes will be made/granted after the advertised deadline. Participants assume risk of change in personal affairs or health. For overnight trips, travel insurance is available and recommend- ed.
