The Nagaland Health Project (NHP) with support from World Bank aims to improve management and delivery of health services and increase their utilization by communities in targeted locations in Nagaland without duplicating existing government programs. The project will target 177 health facilities and 500 villages across the state over the six even-year project duration. The project will accomplish these objectives through implementation of two components: 1) Community action for health and nutrition 2) Health system development. Community Action for Health and Nutrition: Under this component, activities to support community engagement and management of health services will intend to contribute to improved delivery of local health services. This component will also aim to increase community awareness and knowledge to increase utilization of health services and engage in positive health behaviors. Capacity-building activities for the Village Health committees will also be undertaken. This component will also support increased representation by women and foster the involvement of women's self-help groups. Health System Development Under the second component, support will be provided for improvements in the management and delivery of health services, which includes targeted and system-wide investments. This will include investments in off-grid electricity solutions and water supply and sanitation in targeted health facilities and consultancy service for Nagaland Medical College, Kohima. The project will also support investment in information and communications technology (ICT) systems to improve management and delivery of health services. The project will also contribute to addressing health human resource constraints, including improving human resource management and supporting implementation of a health human resource strategy for the state that is currently under development. Strengthening nursing and para-medical schools/colleges is one the areas of support for quality improvement of health care by improving the skills of human resource in health. The project will support development of key state-level components of the health system intended to improve the management and effectiveness of government health services in Nagaland. Development of the health supply chain management system will include improving planning and ordering, procurement, quality assurance, stock management, distribution, storage, and reporting and information systems. Along with required policy and process improvements, including adoption of standard operating procedures, investments will encompass training, equipment and warehouse improvements. Methodology: Technical assessment to understand the Supply Chain Management System of the health sector in Nagaland was done by PHFI. The project also conducted a field assessment to all targeted project sites to understand the existing system of inventory management and demand and forecasting pattern. It was observed that out of the 177 project sites, only about 5% of the health units were maintaining stock register and none of the health unit had a register for assets. No record on indenting of requirement by the health units to the district warehouse was found. Some health facilities responded that the indenting was done verbal over telephone and if the medicines were available in the district warehouse, the health facility was asked to come and collect the supply from the district warehouse. Given the existing scenario, it was decided that standard format for stock register, asset/inventory register, indenting form, monthly consumption report needed to be developed and the personnel should be trained on logistics and supply chain management system. The formats was developed using existing template and formulae for maximum stock, minimum stock, lead time stock, buffet stock was formulated. The training modules was developed in consultation with Deputy Director Store and approved by the Principal Director. Training: 2017-18: 1. Orientation cum training on Supply Chain Management System- CMS, CMO, MS, NHM, NSACS: The first training on Supply Chain Management System was held at the Conference hall of Directorate Of Health And Family Welfare on November 14-15, 2017. A total of 44 personnel from Central Medical Store, 11 District Hospitals, 11 CMO, State Mental Health Institution and NSACS were trained. The Principal Director, Director (health Services), SPO (NHM), SPO (NVDCP), Nagaland Health Project Officials also attended the training. The CMO establishments and District Hospitals personnel/Store Keepers were directed to record the inventory of their respective offices/facilities in the asset register and submit the data within 2 months. Soft copy of all the training materials and formats were given to the participants. 2. Supply Chain Management System training- CHC,UPHC, PMTI, SoN: One day training on supply chain management system for 21 CHC, 5 UPHC, 1PMTI and 4 School of Nursing was held on November 24, 2017 at the NHP Conference hall. 24 participants attended the training. Personnel of 15 CHCs, 4 UPHCs, 1PMTI and SoN Kohima were successfully trained and printed copy of asset register, stock register, indent form, monthly consumption report and training materials were given. The participants were directed to record the inventory of their health units and submit the same to the trainers within 2 months. 3. Supply Chain Management System training- PHCs- Dimapur, Mon and Peren: The training was held at Conference hall, Hotel Acacia, Dimapur on November 28, 2017. Total PHC target for training was 34. 30 personnel attended the training. Out of the 30 trainees, 22 were from PHCs, 1 from CHC Tobu, 2 from District Hospital Mon, 3 from SCs and 2 from DPMU Dimapur. 4. Supply Chain Management System training- PHCs- Phek, Kiphire and Kohima: The training was held at Conference hall, Directorate of Health & Family Welfare, Kohima on November 29, 2017. Total PHC target for training was 38. 19 personnel attended the training, out of which 16 were from PHCs and 1 from CHC Chozoba and 2 from SCs. 5. Supply Chain Management System training- PHCs-Mokokchung, Tuensang, Longleng and Zunheboto: The training was conducted at Longkumer Kilem Conference hall, Mokokchung on December 12, 2017. Out of the targeted 41 PHCs, only 15 PHCs personnel from Mokokchung and Zunheboto district attended the training. In addition, 2 Store keepers from CMO Mokokchung and 3 DPMU staffs attended the training. 6. Supply Chain Management System training- PHCs- Wokha, Zunheboto and Kohima: The training was held at Wokha on December 13, 2017. Out of the targeted 20 PHCs, 10 PHCs personnel, 1 personnel from CHC Sanis, CMO Wokha and SC Mekokla and 3 DPMU staffs attended the training. The Commissioner & Secretary, DHFW, CMO Wokha, DPM Wokha and Project Director, NHP attended the training. In all the training, the standard stock register, asset register, indent form, monthly reporting form and training materials were issued and the trainees were directed to submit the inventory data of their respective health units to the directorate. For all the six training sessions the trainers were Dr. Longai Phom, Deputy Director, DHFW and Er. Manenkala Longkumer, ICT Consultant, NHP. Training consisted of theory as well as practical sessions. 2018-19: For down training of Sub Centers and other health units (CHC,PHC) which could not be covered in the year 2017, it was decided that 2 champions from every district and 2 DPMU NHP staffs shall be trained as trainers to conduct down training in their respective districts. However, the district health administration of Kiphire and Tuensang request the directorate to conduct the training with the key trainers. The Training of District Champions and DPMU, NHP was held on July 4, 2018 at the Directorate Conference Hall, Kohima. During the training, 38 Personnel were trained and the training materials and registers for down training was issued.
