In a new wave of what is known as ‘medical tourism’, it is surprising how many tourists, particularly elderly ones will make their way to Spain only to slip off the gangway as soon as they get to Bilbao, just to get hip replacement surgery. Many of the Islands off Spain are demanding extra funds to cover the cost of treating non-residents, as they receive the majority of tourists. Although the central government funds the Spanish healthcare service with €45 billion ($55 billion), tensions are rising with the regional administrations as they are responsible for the provision of services. As a result of the devolution of the healthcare services, regions took their share of the money but no common strategy or no common guidelines were followed on how the money was to be spent. Thus despite the big rise in healthcare spending there has been very little internal co-ordination. It has also opened up regional rifts between rich regions with little immigration and tourism and poorer regions with high numbers of immigration and tourists draining its healthcare funds. In Madrid alone, one million new arrivals in recent years have stretched the healthcare service to breaking point. The emergency services have to deal with a 30 per cent increase in casualties and a 40 per cent increase in medical consultations. Emergency rooms are overflowing and medical equipment availability is at an all time low. Madrid’s government have announced a need for more that €1.7 billion to cut waiting lists and attend to its immigrant community. Mr Zapatero has offered regional leaders €1.7 billion (this will cover one third of the €5 billion shortfall) but in exchange expects the regions to curb costs and raise local taxes to fund extra spending - something that they are not willing to do.

In many countries throughout the world, the regulation of travel insurance ranges from uneven to barely noticeable. In a bid to find a model guideline, Milan Korcok scrutinises insurance regulation around the world

Though it accounts for growing billions of dollars worth of coverage, travel insurance remains largely a lonely stepchild within the family of insurance producers, tagging along as an adjunct to accident and sickness, or property and casualty lines of business. But with international travellers increasingly aware that ‘going bare’ can be fiscally suicidal, travel insurers have the opportunity to carve out a distinct identity for themselves, so long as they can deal with growing legions of regulators and their sometimes vaporous and conflicting rules. In the wake of global financial industry failures, growing reliance on cross-border purchases (by the Internet or otherwise), international consolidation of companies and marketing efforts, and a growing consumerism that demands accountability of vendors, the responsibility for tighter regulation of financial services such as banks and insurers has become a top priority of governments worldwide.
