These are substantive posts and are available on a ten programmed activity basis (8 DCC: 2 SPA). A number of posts may be available and these will be to cover an expansion of anaesthesia provision. Applicants with a broad general interest would be welcomed. This is an exciting opportunity for enthusiastic and able individual join a friendly and dynamic department in an area with a fantastic outdoor lifestyle. The Anaesthetic Department has 27 Consultants (8 Intensivists), 2 Associate Specialists, 6 Speciality Doctors and 11 Specialty Trainees. The on-call for anaesthesia covers all general emergencies, trauma and obstetrics (~1:14). There is a separate on-call for Intensive Care Medicine. Applicants must have full GMC registration, a licence to practise and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to a CCT in Anaesthetics or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview. There is also a supernumerary F1 attached to the intensive care unit. All of these posts have royal college and deanery approval for training. Mrs Morag Ritchie is the department administrator and her office is located close to theatres and the intensive care unit. Duties and responsibilities You will be expected to work with local managers and professional colleagues in the efficient running of the service, and will share with consultant colleagues in the medical contribution to management. Subject to the provisions of the Terms and Conditions of Service you are expected to observe the NHS Highland Board’s agreed policies and procedures, drawn up in consultation with the profession, on clinical matters, and to follow the standing orders and financial instructions of NHS Highland. In particular, where you formally manage employees of NHS Highland, you will be expected to follow the local and national employment and personnel policies and procedures. You will be expected to make sure that there are adequate arrangements for hospital staff, involved in the care of your patients, to be able to contact you when necessary. You are required to comply with the NHS Highland Board’s Health & Safety Policies. You will be responsible for the training and supervision of the junior medical staff with whom you work, and you will be expected to devote time to this activity on a regular basis. In addition you will be expected to ensure that junior staff have access to advice and counseling. If appropriate, you will be named in the contracts of doctors in training grades, as the person responsible for overseeing their training, and as the initial source of advice to such doctors, on their careers. Job plans It is a requirement for all consultants to have a job plan. The proposed job plan is in the attached information pack. Agreed job plans will be prospective and will set out: • All professional commitments • Time and service commitments • Accountability and management arrangements • Objectives • Resources • Any agreed extra programmed activities All job plans will follow the model format contained at Appendix 4 of the New Consultant Contract. The job plan will be subject to review at least annually, or more often, if changes to staffing resources, or working practices, or the consultant’s circumstances require it. Anaesthesia The provisional programme is attached. The on-call is dedicated to anaesthesia, and subject to small changes, is ~1:14 with prospective cover. There is a separate intensive care medicine on-call rota. There are 2 anaesthetists resident in the hospital overnight - this typically comprises of an SAS/Specialty doctor and a trainee (CT2 or ST3). They are expected to work collaboratively, but one of these covers the ICU and the other covers theatres & obstetrics/epidurals etc. The main theatre suite has 10 operating theatres and also an obstetric theatre within the labour suite. They are currently undergoing a major renovation/update. Six of the theatres have been completed and work is planned to continue for another 6 months. The successful candidate will be required to have a broad range of skills and work with the following surgical specialties: • General Surgery (including upper GI) • Vascular • Orthopaedics and Trauma • Gynaecology • Obstetrics • Ear, Nose and Throat • Urology • Maxillo-facial/Dental • Ophthalmology Obstetrics The maternity unit has approximately 2000 deliveries a year. There are daily scheduled caesarean section lists and we provide a 24 hour epidural service. In the last 12 months there were 680 caesarian sections performed at Raigmore, 336 of these were elective. Labour ward/theatres is staffed with a consultant anaesthetist mon-fri 0800-1800 hrs. There are currently 6 consultant anaesthetists with regular sessions and an interest in obstetrics. Acute pain The Acute Pain Service is delivered by 2 Consultant Anaesthetist and the daily management of pain problems in the in-patient population is dealt with by two senior nurses. The acute pain service is not only involved in the daily management of patients but also provides in-house education to all the staff in the hospital. Paediatrics We have a lead Paediatric

Introduction from CEO The engagement strategy which is helping to shape the ‘Culture Fit for the Future’ has been moving ahead. Fiona Hogg will be giving a detailed update on our progress with our Culture Programme later in the agenda. I am pleased with the progress which is being made, it’s a long-term piece of work which needs careful research and planning to make sure we understand the problems we need to address. There are some key milestones being achieved and presented at this Board meeting. A governance structure is in place and our revised plans are ready to be agreed and rolled out. Many of the actions planned have been shaped by our ongoing engagement with the Board and with our colleagues across North Highland and Argyll & Bute and I’m delighted that our impending review in Argyll & Bute is going to provide further valuable insights and information. In terms of the cost improvement programme, we continue to make good progress, with the current level of identified opportunities valued at £29M which when adjusted for the likelihood of delivery reduces to £22M. Of particular note is that 80% of the forecast savings are recurrent so the savings gain will benefit future financial years. Workstreams are ensuring the remaining plans in this financial year are approved, that ideas are progressed to plans and that changes already implemented go on to deliver the expected savings.

JOB PURPOSE To provide an efficient and effective Physiotherapy Service, to patients referred to the Physiotherapy Service in the Raigmore /South and Mid divisions. To provide a high standard of Physiotherapy, consistent with both local and national standards. This post could be static or part of the Physiotherapist rotations which can be throughout any hospital in Raigmore /South and Mid divisions. This post may include on-call and weekend work depending upon the particular department. 1 3. DIMENSIONS Physiotherapy in NHS Highland is currently divided into Raigmore, South & Mid and North & West Divisions. The acute service unit is based at Raigmore Hospital Inverness. All Physiotherapy posts are part of Multidisciplinary teams in the Hospital or the Integrated Community teams. Rotational posts can be in Raigmore Hospital, Invergordon County Community Hospital, Ross Memorial Hospital, Dingwall, Royal Northern Infirmary (RNI) Inverness, Nairn Town and County Hospital, or any other community site within the South and Mid and Raigmore divisions. The rotations may include all physiotherapy specialties, including paediatrics and mental health. 4. Summary On 29th November 2016 NHS Highland Board approved the recommendations of ‘‘Caithness Maternity and Neonatal Services – A Public Health Review’ including the move to set up midwife-led Community Maternity Unit (CMU) in Caithness and the designation changed on Monday 4th December 2016. An interim report on progress was presented to the board in May 2017. This paper provides an update on the arrangements as of the 1st November 2017, including a summary of clinical activity. 1.1 Number of Births. Transfers and Retrievals Over the eleven month period from 1st December 2016 to 31st October 2017 there have been total of 218 births to women resident in Caithness. Of these, 199 births were in Raigmore Hospital, and 18 (9%) normal births in the CMU, with one planned delivery elsewhere Transfers and Retrievals are lower than before the change in designation and this is an important indicator that the new arrangements are working as planned.
