Product Ordering , FFP,

Ordering these products parallels ordering PRBC, with fewer selections on the Edit (CDS) screens. The Transfusion Order Set is shown below. PRBC ordering has been covered in the CovenantEHR email of April 24th, 2014. Ordering Platelets, FFP and cryoprecipitate is outlined below.

Remember that in order to have a blood product administered, you must order both the blood product and its administration.

In Order Entry, select Sets and Category, then open the Transfusion menu, as shown above.

From the Transfusion menu you can select Transfuse Blood Routine or Transfuse Blood Stat, as well as Transfuse Blood By Criteria and Hold Blood for Surgery. The Routine order set will be used for the examples here. Selecting the Routine order set brings up the following order set:

The upper portion of the set (above) contains the blood products and the Transfuse orders. Both an order for the desired blood product and the corresponding Transfuse order must be selected for each type of blood product.

This portion of the order sets contains orders for Pre- and Post-transfusion meds.

Platelets

Selecting platelets and the Red Edit Button brings up the following screen. NOTE that the following defaults are in place: Quantity 1, Scheduled Procedure, Irradiated and Split “No”. The quantity is one single donor unit.

The next step to complete the order is to select the Transfuse order to administer the platelets. Selecting the Transfuse order brings up the following screen:

Note that this screen is defaulted to 1 (single donor) unit of platelets and that the rate is that defined in Nursing Policy.

Fresh Frozen Plasma (FFP)

Selecting “Plasma (FFP)”S and the Red Edit Button brings up the following screen: NOTE that the only default that is in place is to transfuse at the Nursing Policy rate, the number of units to give must be selected. The only required field that is not prepopulated is the “Justification for Plasma”. Select the drop down menu here and chose the reason for the FFP.

The final step is to order the FFP transfusion. Doing this leads to another Red Edit Button. Selecting the Red Edit Button brings up the following screen: NOTE: The only required field here that is not prepopulated is the Number of Units to give.

Cryoprecipitate

Selecting the Cryoprecipitate order and the Red Edit Button brings up the following screen: NOTE: The only required field here is the “Justification for Cryo” drop down menu. Select this and chose the reason for the cryoprecipitate.

The final step to administer cryoprecipitate is to select the Transfuse order, select the Red Edit Button, and complete the following screen: NOTE: The only required field in this screen is to complete the number of units of cryoprecipitate to give to the patient.
