In order to check whether the facility is in accordance with the required standards, bring layout plans before starting construction and consult with those in charge of food hygiene at the health center. Also, ・Storage of food to be used for business ・Preparation of food ・Pre-opening events for involved parties Must be after receiving a business permit. You should secure enough time to prepare for the opening of your store.. Furthermore, ・In the case of using water from water tanks (not directly connected to water pipes), a water inspection is required ・ A food hygiene supervisor must be placed at each facility for sanitary management/operations *Regarding food hygiene supervisors Those with a Japanese cook/nutritionist license, or those who have completed Japanese training courses for acquiring licenses are eligible to become hygiene supervisors. In Tokyo, the Tokyo Food Hygiene Association (一般社団法人東京都食品衛生協会) holds training courses for acquiring licenses. Application forms are available at the general offices of the association headquarters and health centers within Tokyo. If you wish to attend, visit the website of the association or contact the association. Necessary documents ① Business permit application (営業許可申請書) ② Outline/layout of business facility (営業設備の大 要・配置図) (2 copies) (Use a special form for describing the details of the store facility. The layout plan can be separately prepared.) ③ Permit application fee (Cash only. Fees vary according to types of business) ④ Certificate of registered matters (only for corporations) ⑤ Water inspection result (only if using water from water tanks) ⑥ A certification of a food hygiene supervisor qualification (Completion certificate of food hygiene supervisor training course etc.) Consult with the staff at the center regarding the date of on-site inspection of the facility when submitting the documents. 3 Facility inspection The health center staff will confirm that the conditions of the facility are in accordance with the details in the application and meet facility standards. •The business operator and someone who can understand Japanese are expected to be present during an inspection if possible. •A permit will not be granted if the facility does not meet the required facility standards. Improve the indicated points and apply for re-inspection. * If the facility meets the required standards, a “notice regarding the expected delivery date of business permit (営業許可書交付予定日のお知らせ)” will be handed. 4 Delivery of business permit Bring the “notice regarding the expected delivery date of business permit (営業許可書 交付予定日のお知らせ)” after the scheduled delivery date of the business permit, and receive the business permit at the health center. Contact the food hygiene section of the health center for any inquiries. [Notification of Discontinuance of Business] In the following cases, a procedure for notification of discontinuance of business is required. When JMTIA was established in 1955, a list of members with their product lines was published. In the beginning it was called “Agency List” and in 1998, the name was changed to “Directory”. JMTIA has published “Agency List” or “Directory” every two years since 1955. In December, 1999 JMTIA opened its own website, which was renewed with new classification of machine accessories and tools in 2004. Furthermore, in 2012, JMTIA website recreated so that we can search a various kinds of information by utilizing the database of members’ information. The latest directory is edited based on the contents of JMTIA website.
