Other Health Services Other Health Care Services • Primary and Ambulatory Care • Private Practitioners • Certified Home Health Agencies • Adult Care Facilities Factor 6: The Existence of Other Health Care Services in the Affected Region, Including the Availability of Services for the Uninsured and Underinsured Primary and Ambulatory Care The attached table lists diagnostic and treatment (D & T) centers by region 1. D & T centers are clinics that offer a range of primary and specialty services. A large portion of D & T centers are not-for-profit and located in underserved areas. Many meet the criteria for Federally Qualified Health Centers (FQHC’s), and receive Federal funds in support of comprehensive primary care and 24/7 coverage to underserved, uninsured and Medicaid populations. D & T centers operated by hospitals are considered extension clinics of the hospital (list attached). Private Practitioners Physicians in private practice are a major health care resource. However, the private practice of medicine is not subject to DOH regulation, making the scope and volume of services delivered in a community difficult to measure. The best means of estimating the degree to which private practice physicians serve the underserved in a given area is through the analysis of Medicaid claims filed with DOH by physicians who participate in the Medicaid program. Certified Home Health Agencies Certified Home Health Agencies (CHHAs) provide part-time, intermittent health care and support services to individuals who need intermediate and skilled health care. CHHAs can also provide long-term nursing and home health aide services, can help patients determine the level of services they need, and can either provide or arrange for other services including physical, occupational, and speech therapy, medical supplies and equipment, and social worker and nutrition services. Adult Care Facilities Adult care facilities (ACFs) refers to a category of community-based facilities which provide temporary or long-term, non-medical residential care services to adults who are substantially unable to live independently. The types of adult care facilities are adult homes, enriched housing, and residences for adults. An assisted living program (ALP), which is available in some adult homes and enriched housing programs, combines residential and home care services. 1 Facilities are listed by authorized operating entity, some of which are certified to operate additional facilities in areas outside the entity’s home county and particular Commission region. This accounts for the occasional appearance of D&T centers and extension clinics whose location lies outside the parent facility’s county or borough.     Judson Health Center, 1920-1950 n 1951, Mayor Vincent R. Impellitteri of New York presented Dr. Eleanor A. Campbell with a testimonial scroll honoring her thirty years of service as the general director and co-founder of the Judson Health Center. In these thirty years, the center had logged in one million visits, both at the clinic and in people's homes, a phe- nomenal accomplishment considering the Center's humble begin- mngs. In 1920 Judson's minister, the Rev. Dr. A. Ray Petty, had invited Dr. Eleanor A. Campbell to start a clinic at Judson Memorial Church to serve the Italian immigrant population just south of the church. Dr. Campbell was a recent graduate of the Boston University School of Medicine. He had heard that she was planning to get in- volved in a public health program, and he wanted her in his district. Dr. Campbell later recalled that the Rev. Petty had said: "I just can- not stand the crooked legs of the children down here." 1 The Rev. Petty was keenly aware of the health conditions and the lack of health care services in his neighborhood. He had already been working with the New York Association for Improving the Condition of the Poor (AICP) to establish a Baby Health Station at 114 Thompson Street.2 The Department of Hygiene was sponsoring 1.
