Although spotted fever group rickettsiae (SFGR) have been reported as the most prevalent cause of rickettsiosis in rural areas of Sabah, Malaysia since the 1980s, this is the first detailed case report of suspected SFGR in the state. Current data on the prevalence, type, and thorough clinical reports on complications of SFGR and other rickettsioses in Sabah is lacking and required to raise the awareness of such diseases. There is a need to emphasize the screening of rickettsioses to medical personnel and to encourage the use of appropriate antibiotics as early treatment for nonspecific febrile illnesses in this region. Suspected rickettsioses need to be considered as one of the differential diagnoses for patients presenting with acute febrile illness for laboratory investigations, and early treatment instituted. Keywords: rickettsiae; Malaysia; diagnosis 1. Introduction Over the past 25 years, there has been an increase in the knowledge of rickettsial pathogens worldwide [1]. Nevertheless, there are still geographical areas where little information on rickettsioses is available regarding the clinical cases, the pathogens and the vectors [1–4]. Yet in Southeast Asia, and in travellers returning from this region, rickettsial diseases are among the leading causes of treatable acute febrile illnesses [5,6]. The clinical symptoms of rickettsioses may vary depending on the rickettsial species involved, and therefore diagnosis can be challenging even for experienced physicians [1]. Moreover, the specific laboratory methods for the diagnosis of infections are seldom available in remote areas, and even in large cities, in Southeast Asia [1,5,7]. In addition, the management of the disease could also be challenging given the (lack of) availability of the most commonly used antibiotics, the possible resistance to them, and the alternatives to such antibiotics [8,9]. Methods: To describe the changing epidemiology of malaria in Sabah, in particular the increasing incidence of P. knowlesi, a retrospective descriptive study was undertaken involving a review of Department of Health malaria notification data from 2012–2013, extending a previous review of these data from 1992–2011. In addition, malaria PCR and microscopy data from the State Public Health Laboratory were reviewed to estimate the accuracy of the microscopy-based notification data. Results: Notifications of P. malariae/P. knowlesi increased from 703 in 2011 to 815 in 2012 and 996 in 2013. Notifications of P. vivax and P. falciparum decreased from 605 and 628, respectively, in 2011, to 297 and 263 in 2013. In 2013, P. malariae/P. knowlesi accounted for 62% of all malaria notifications compared to 35% in 2011. Hence, the aim of this study is to investigate the genetic diversity of non‑repeat regions of csp in P. knowlesi from Malaysian Borneo and Peninsular Malaysia. Methods: A total of 46 csp genes were subjected to polymerase chain reaction amplifcation. The genes were obtained from P. knowlesi isolates collected from diferent divisions of Sabah, Malaysian Borneo, and Peninsular Malay‑ sia. The targeted gene fragments were cloned into a commercial vector and sequenced, and a phylogenetic tree was constructed while incorporating 168 csp sequences retrieved from the GenBank database. The genetic diversity and natural evolution of the csp sequences were analysed using MEGA6 and DnaSP ver. 5.10.01. A genealogical network of the csp haplotypes was generated using NETWORK ver. 4.6.1.3. Results: The phylogenetic analysis revealed indistinguishable clusters of P. knowlesi isolates across diferent geo‑ graphic regions, including Malaysian Borneo and Peninsular Malaysia. Nucleotide analysis showed that the csp non‑ repeat regions of zoonotic P. knowlesi isolates obtained in this study underwent purifying selection with population expansion, which was supported by extensive haplotype sharing observed between humans and macaques.

A total of 180 sites were surveyed in 2019 (2018: 212), 97 in Peninsular Malaysia and 83 in East Malaysia. The surveys are a continuation of a successful National Reef Check Survey Programme that has now run for thirteen years. 2. The surveys were carried out by trained volunteers as well as government officials from Marine Parks Malaysia, reflecting commitment from the Government in further improving management of Malaysia’s coral reefs. Surveys were carried out on several islands off Peninsular Malaysia’s East and West coast, covering both established Marine Protected Areas (MPAs) and non-protected areas, and in various parts of East Malaysia, both Sabah and Sarawak. 3. The results indicate that the Malaysian coral reefs surveyed have a relatively high level of living coral, at 40.63% (2018: 42.42%).

Methods: A community‑based cross‑sectional study was conducted from October 2019 to February 2020 involv‑ ing 468 respondents. Information on the socio‑demographic characteristics of the participants (six items), their KAP (44, 15 and 18 items on knowledge, attitude and practice, respectively) and treatment‑seeking behaviour (fve items) towards dengue was collected using a structured questionnaire. Data analysis was performed using SPSS and R software in the R Studio environment. The knowledge section was analysed by two‑parameter logistic item response theory (2‑PL IRT) using ltm package. The construct validity and reliability of items for sections on attitude, practice and treatment‑seeking behaviour were analysed using psy package. Results: For the knowledge section, only 70.5% (31/44) of items were within or close to the parameter acceptable range of 3 to 3 of difculty.

However, little is known about the incidence and distribution of this parasite in the Sandakan division, Malaysian Borneo. Therefore, the aim of the present epidemiological study was to investigate the incidence and distribution of P. knowlesi as well as other Plasmodium species in this division based on a most recent developed hexaplex PCR system (PlasmoNex™). Methods: A total of 189 whole blood samples were collected from Telupid Health Clinic, Sabah, Malaysia, from 2008 to 2011. All patients who participated in the study were microscopically malaria positive before recruitment. Complete demographic details and haematological profiles were obtained from 85 patients (13 females and 72 males). Identification of Plasmodium species was conducted using PlasmoNex™ targeting the 18S ssu rRNA gene. Results: A total of 178 samples were positive for Plasmodium species by using PlasmoNex™. Plasmodium falciparum was identified in 68 samples (38.2%) followed by 64 cases (36.0%) of Plasmodium vivax, 42 (23.6%) cases of P. knowlesi, two (1.1%) cases of Plasmodium malariae and two (1.1%) mixed-species infections (i e, P. vivax/ P. falciparum).
