Dear Parents,

Throughout the year we watch fun movies. We can only show G rated movies without permission from parents. The kids regularly watch PG movies at home, and many Disney movies and other kid-targeted movies are PG rated. Please look over the following list of movies the kids have told me they have at home that they could share with the class for special occasions. If there is one specifically you don’t want your child to see, but the others are okay, just circle that movie. Or, if there is a movie that isn’t listed that you don’t want your child to see, but is popular with kids, please write it in on the bottom of the page. I need a signed permission form for each child in our class. Please print this permission form off and send it in to me within the first week of starting school.
