Operating under the 1863 Congressional charter of the National Academy of Sciences, the National Academies are private, nonprofit institutions that work outside of government to provide objective advice on matters of science, technology, and health. Studies from the National Academies are often congressionally mandated or commissioned by government agencies, and their recommendations can have lasting impact on domestic and global policy. Who We Are An independent, evidence-based scientific advisor. The NAM is committed to applying scientific rigor and objective evidence to achieve balanced, authoritative solutions to complex challenges in health. To carry out our work, we harness the talents and expertise of accomplished, thoughtful volunteers and undertake meticulous processes to avoid and balance bias. Our foundational goal is to be the nation’s most reliable source for credible scientific and policy advice on matters concerning human health. A national academy with global scope. Although the National Academies were originally created to advise the U.S. government and advance the well-being of the U.S. population, our mandate is now much broader. Lynn Caporale, Strategic Scientific Advisor and Author The following slides were presented at the recent meeting of the American Association for the Advancement of Science. This project and paper is an outgrowth of that session, and will combine qualitative data on Nobel Prize Winners family histories along with analyses of the pattern of Nobel Winners. The first set of slides show some of the patterns so far found, and will be augmented for the formal paper. The second set of slides shows some examples of the Nobel families. The authors a developing a systematic data base of Nobel Winners (mainly US), their careers and their family histories. This turned out to be much more challenging than expected, since many winners do not emphasize their family origins in their own biographies or autobiographies or other commentary. Dr. Caporale has reached out to some laureates or their families to elicit that information. We plan to systematically compare the laureates to the population in the US at large, including immigrants and non‐immigrants at various periods.     A DNA database in the NHS: Your freedom up for sale? May 2013 In April 2013, the Caldicott Committee, including Government Chief Scientist Sir Mark Walport, proposed new rules for data-sharing which would allow the Government to build a DNA database of the whole population of England in the NHS by stealth.1 The plan is to make NHS medical records and people’s genetic information available to commercial companies and to use public-private partnerships to build a system where all private information about every citizen is also accessible to the police, social workers, security services and Government. The Wellcome Trust, which was involved in the Human Genome Project and was led by Walport for ten years, has produced a plan which involves including a variant file, containing the whole genome of every person minus the reference genome, as an attachment to every medical record in the NHS in England.2 This data would be made available to ‘researchers’ (including commercial companies) for data-mining in the cloud and personalised risk assessments would be returned to individuals. The aim is to transform the NHS in line with proposals developed more than a decade ago by former GlaxoSmithKline Chairman Sir Richard Sykes. This is expected to massively expand the market for medicines, medical tests and other products, such as supplements and cholesterol-lowering margarines, by allowing products to be marketed to individuals based on personal risk assessments, created using statistical analysis of genetic data, medical records and other health information. The proposal to build a DNA database in the NHS was endorsed by the Human Genomics Strategy Group in 20123,4 and the Government (led by Prime Minister David Cameron) has quietly adopted this recommendation without telling members of the public.

The future is now. So, how do we balance moving forward with user-friendly digitization, telemedicine and virtual healthcare delivery while simultaneously recognizing the unintended consequences of the innovative criminal mind? The first step is to recognize there’s a problem. Counterfeit Medicines: A Moveable Feast Once upon a time, at the beginning of the new millennium, counterfeit medicines in the United States were largely “lifestyle” products such as erectile dysfunction drugs – Viagra being the poster child of the problem.i Other categories of fake pills included treatments for depression.ii The common denominator was patient shame and embarrassment. Ordering from seemingly benign (i.e., “from Canada”) websites seemed like a safe and anonymous way to address their conditions without having to visit either a physician, mental health professional or pharmacist. A second category of counterfeit prey were people seeking higher risk drugs (opioids, steroids, etc.) to facilitate a more dangerous lifestyle. The rationale for this second group was easier access to more dangerous (often controlled) substances.iii To respond to this emerging threat, the FDA formed a Counterfeit Drug Task Force in July 2003.iv As a former FDA Associate Commissioner, I was proud to serve as a member of that task force.

    Sentinel Initiative Public Workshop Participant Biographies Mark McClellan, MD, PhD is director of the Engelberg Center for Health Care Reform and Leonard D. Schaeffer Chair in Health Policy Studies at the Brookings Institution. At the Center, his work focuses on promoting high-quality, innovative and affordable health care. A doctor and economist by training, he also has a highly distinguished record in public service and in academic research. Dr. McClellan is a former administrator of the Centers for Medicare & Medicaid Services (CMS) and former commissioner of the Food and Drug Administration (FDA), where he developed and implemented major reforms in health policy. These include the Medicare prescription drug benefit, the FDA’s Critical Path Initiative, and public-private initiatives to develop better information on the quality and cost of care. Dr. McClellan chairs the FDA’s Reagan-Udall Foundation, is co-chair of the Quality Alliance Steering Committee, sits on the National Quality Forum’s Board of Directors, is a member of the Institute of Medicine, and is a research associate at the National Bureau of Economic Research. He previously served as a member of the President’s Council of Economic Advisers and senior director for health care policy at the White House, and was an associate professor of economics and medicine at Stanford University. Margaret Hamburg, MD is Commissioner of Food and Drugs, confirmed on May 18, 2009, by a unanimous Senate vote. The second woman to be nominated for the position, Dr. Hamburg is exceptionally qualified to serve with her training and experience as a medical doctor, scientist, and public health executive.

McEwen, PhD Lab Neuroendocrinology, Rockefeller University, New York, NY The hippocampal formation, which expresses high levels of adrenal steroid receptors, is a plastic brain structure that is important for certain types of learning and memory. It is also vulnerable to insults such as stroke, seizures and head trauma. The hippocampus is also sensitive and vulnerable to the effects of stress and stress hormones and it is responsive to the actions of sex hormones as well, both during development and adult life. Stress and sex hormones regulate 3 types of structural plasticity in the adult hippocampus: synaptogenesis, reorganization of dendrites, and neurogenesis in the dendrite gyrus. Developmentally-programmed sex differences are also seen in the hippocampus. Suppression of dentate gyrus neurogenesis and atrophy of dendrites of hippocampal pyramidal neurons are produced by chronic psychosocial stress, involving the actions of adrenal steroids acting in concert with excitatory amino acid neurotransmitters. As far as we can tell, these changes are reversible as long as stress is terminated after a number of weeks. However, there are also reports that much longer durations of psychosocial stress leads to permanent loss of hippocampal pyramidal neurons. In the human hippocampus, MRI studies along with neuropsychological testing have revealed memory impairment and atrophy of the whole human hippocampus in some individuals as they age. This is reminiscent of individual differences in aging in rodents, which appear to reflect life-long patterns of stress hormone reactivity that are developmentally programmed, although a developmental influence upon human individual differences is only a matter of speculation.
