Better understanding of the forces selecting resistance mutations could help develop strategies to optimize the use of antibiotics and slow the spread of resistance. Resistance to ciprofloxacin, a clinically important antibiotic, almost always involves target mutations in DNA gyrase and Topoisomerase IV. Because ciprofloxacin is a substrate of the AcrAB-TolC efflux pump, mutations causing pump up-regulation are also common. Studying the role of efflux pump-regulatory mutations in the development of ciprofloxacin resistance, we found a strong bias against gene-inactivating mutations in marR and acrR in clinical isolates.

Some reports describe the presence of cross-reactivity among different members of the group, although no predictive pattern has been established. No previous studies confi rm or rule out cross-reactivity between levofl oxacin and other quinolones. Therefore, a joint study was designed between 2 allergy departments to assess cross-reactivity between levofl oxacin and other quinolones. Material and Methods: We studied 12 patients who had experienced an immediate-type reaction (4 anaphylaxis and 8 urticaria/angioedema) after oral administration of quinolones. The culprit drugs were as follows: ciprofl oxacin (5), levofl oxacin (4), levofl oxacin plus moxifl oxacin (1), moxifl oxacin (1), and norfl oxacin (1). Allergy was confi rmed by skin tests and controlled oral challenge tests with different quinolones. The basophil activation test (BAT) was applied in 6 patients. Results: The skin tests were positive in 5 patients with levofl oxacin (2), moxifl oxacin (2), and ofl oxacin (2). BAT was negative in all patients (6/6). Most of the ciprofl oxacin-reactive patients (4/5) tolerated levofl oxacin. Similarly, 3 of 4 levofl oxacin-reactive patients tolerated ciprofl oxacin. Patients who reacted to moxifl oxacin and norfl oxacin tolerated ciprofl oxacin and levofl oxacin. Conclusions: Our results suggest that skin testing and BAT do not help to identify the culprit drug or predict cross-reactivity.

Interaction with metal ions has some important consequences for the solubility, pharmacokinetics and bioavailability of quinolones, and is also involved in the mechanism of action of these bactericidal agents. Many metal complexes with equal or enhanced antimicrobial activity compared to the parent quinolones were obtained. New strategies in the design of metal complexes of quinolones have led to compounds with anticancer activity.

A chromatographic method is described for assaying fourteen quinolones and fluoroquinolones (pipemidic acid, marbofloxacin, norfloxacin, ciprofloxacin, danofloxacin, lomefloxacin, enrofloxacin, sarafloxacin, difloxacin, oxolinic acid, nalidixic acid, flumequine, and pyromidic acid) in fish samples. The samples were extracted with m-phosphoric acid/acetonitrile mixture (75:25, v/v), purified, and preconcentrated on ENV + Isolute cartridges. The determina- tion was achieved by liquid chromatography using C18 analytical column. A mobile phase composed of mixtures of methanol-acetonitrile-10mM citrate buffer at pH 4.5, delivered under optimum gradient program, at a flow rate of 1.5 ml/min, allows accomplishing the chromatographic separation in 26 minutes. For the detection were used serial UV-visible diode-array at 280 nm and 254 nm and fluorescence detection at excitation wavelength/emission wavelength: 280/450, 280/495, and 280/405 nm. The detection and quantification limits were between 0.2–9.5 and 0.7–32 µg/kg, respectively. The procedure was applied to the analysis of spiked salmon samples at two different concentration levels (50 µg/k and 100 µg/kg).

Accepted 30 September, 2011 The aim of the present study was to investigate susceptibility of Klebsiella aerogenes towards pefloxacin. The MIC determined by broth dilution method and Hi-Comb method was 0.1 µg/ml. Morphological alterations on the cell surface of the K. aerogenes was shown by scanning electron microscopy (SEM) after the treatment with pefloxacin. It was observed that the site of pefloxacin action was intracellular and it caused surface alterations. The present investigation also showed the effect of Quinolone pefloxacin on DNA gyrase activity of K. aerogenes. DNA gyrase was purified by affinity chromatography and inhibition of pefloxacin on supercoiling activity of DNA gyrase was studied. Emphasis was also given on the inhibition effect of pefloxacin on dehydrogenase activity of K. aerogenes. We analyzed retail sales data of oral and injectable antibiotics (World Health Or- ganization (WHO) Anatomic Therapeutic Chemical (ATC) code J01) between 1997 and 2007 for Argentina, Brazil, Chile, Colombia, Mexico, Peru, Uruguay, and Venezuela. Antibiotics were aggregated and utilization was calculated for all antibiotics (J01); for macrolides, lin- cosamindes, and streptogramins (J01 F); and for quinolones (J01 M). The kilogram sales of each antibiotic were converted into defined daily dose per 1 000 inhabitants per day (DID) ac- cording to the WHO ATC classification system. We calculated the absolute change in DID and relative change expressed in percent of DID variation, using 1997 as a reference. Results. Total antibiotic utilization has increased in Peru, Venezuela, Uruguay, and Brazil, with the largest relative increases observed in Peru (5.58 DID, +70.6%) and Venezuela (4.81 DID, +43.0%). For Mexico (–2.43 DID; –15.5%) and Colombia (–4.10; –33.7%), utilization decreased. Argentina and Chile showed major reductions in antibiotic utilization during the middle of this period. In all countries, quinolone use increased, particularly sharply in Venezuela (1.86 DID, +282%). The increase in macrolide, lincosaminde, and streptogramin use was greatest in Peru (0.76 DID, +82.1%), followed by Brazil, Argentina, and Chile.
