The Langerhans cells are the KEYWORDS immunocompetent cells of the epidermis, and they represent the first defense barrier of the Histiocytoma, benign immune system towards the environment. The objective was to immunohistologically compare fibrous; the densities of S100-positive Langerhans cells in the healthy peritumoral epidermis against Skin neoplasms; those in the epidermis overlying dermatofibroma (20 cases), using antibodies against the S100 S100 Proteins molecule (the immunophenotypic hallmark of Langerhans cells). The control group (normal, healthy skin) included ten healthy age and sex-matched individuals who underwent skin biopsies for benign skin lesions. A significantly high density of Langerhans cells was observed both in the epidermis of the healthy skin (6.00 ± 0.29) and the peritumoral epidermis (6.44 ± 0.41) vs. those in the epidermis overlying the tumor (1.44 ± 0.33, p < 0.05). The quantitative deficit of Langerhans cells in the epidermis overlying dermatofibroma may be a possible factor in its development. © 2020 Sociedade Brasileira de Dermatologia. EPIDEMIOLOGY z Sarcomas are rare tumors compared to other BONE AND SOFT malignancies: 8,700 new sarcomas in 2001, with TISSUE TUMORS 4,400 deaths. z The incidence of sarcomas is around 3-4/100,000. z Slight male predominance (with some subtypes more common in women). z Majority of soft tissue tumors affect older adults, but important sub-groups occur predominantly or exclusively in children. z Incidence of benign soft tissue tumors not known, but Fabrizio Remotti MD probably outnumber malignant tumors 100:1. A survey of the English literature reveals 13 cases acceptable as mixed hepatoblastoma; these have been described and published under a variety of names. Difficulties in nomenclature and the histology of these cases are discussed. Diagnosis depends on the identification of both malignant mesenchymal and malignant epithelial elements. The former include myxoid connective tissue resembling primitive mesenchyme and areas resembling adult fibrosarcoma. Mature fibrous tissue with calcification and bone for- mation may be seen. Epithelial areas show tissue resembling fetal liver, poorly differentiated epithelial cells, and/or areas of adenocarcinoma. The current view on histogenesis is also given. There was no history of preceding trauma. The review of systems was normal. Initially the lesion was thought to be a pyogenic granuloma and treated with two courses of pulse dye laser. After no response to treatment, a shave biopsy was performed. Because the histopathology was interpreted as an atypical melanocytic proliferation with Spitzoid features, a conservative, but complete excision with margins was performed. The pathology of this excision was interpreted as malignant melanoma measuring 4.0 mm in thickness. A sentinel lymph node biopsy was subsequently performed and demonstrated focal spindle cells within the subcapsular sinus of a left preauricular lymph node. -

The importance of this lesion is that it must be distinguished from more aggressive soft tissue chondrosarcoma so as to spare the patient from unnecessary radical therapy. Here, we present a case of ESC of hand in a 32 yr old female with a variable histological appearance exhibiting chondroblastoma- like areas leading to a mistaken diagnosis of Extraskeletal myxoid chondrosarcomas (ESMCS). Hence, the case had to be carefully evaluated to exclude ESMCS and to make the diagnosis of ESC. The treatment was limited to simple excision of the tumor and extensive surgery and radiotherapy was avoided.
