Dear Board of Appeals,

We write to you today in response to an Appeal filed on San Francisco Department of Public

Works permit No. 18WR-0369, a personal wireless service facility permit to locate a small cell mobile PCS antenna facility onto a utility pole within the public right-of-way adjacent to 2900

25th Street. An application for the proposed facility was filed by ExteNet Systems LLC

(“ExteNet”) and its contractor, Richard Hirsch, for use by T-Mobile.

The grounds or basis for this Appeal are not clear. Appellant requested an extension to file a

brief by June 13 instead of June 9 and ExteNet did not object to the request. An appellant brief

was not received.

Below, we have addressed commonly raised concerns involving electromagnetic frequency

(EMF) propagation, aesthetics, distance measurements, the required public notification process, alternative locations considered, and potential obstruction of view and/or blockage of

light considerations.

Public Health / EMF concerns

As required by San Francisco Public Works Code Article 25 and DPW Order No. 184504, as

superceded by Order No. 201970, ExteNet has demonstrated that the permitted facility will comply with all Federal Communications Commission EMF emission standards. The statements of Hammett & Edison, Inc., licensed consulting engineers, confirm that the facility will operate well within FCC exposure guidelines. More specifically, the maximum EMF energy output at the top floor elevation of any nearby building is predicted to be 2.8% of the FCC public exposure limit, according to the Hammett & Edison analysis. The maximum exposure level at the ground would not exceed 1.3% of the FCC public exposure limit per the same analysis. The

Department of Public Health has reviewed the EMF energy report and confirms that the proposed facility “would be in compliance with FCC standards and would not produce radio frequency energy exceeding the FCC public exposure limits.”

Planning, Zoning and Aesthetics

The facility was reviewed by the San Francisco Planning Department, as required, due to the

pole’s location within an RH-2 zoning district. The Planning Department determined that the

facility would comply with all Zoning and Planning Protected Location Compatibility Standards.

ExteNet has collaborated extensively with Planning to ensure that this small cell facility is well-

designed and placed to ensure that it blends in with existing utility infrastructure and is visually

unobtrusive to the greatest extent possible. Planning determined that a facility at the proposed

location would be consistent with “the public health, safety, convenience and general welfare” and “would not unreasonably affect, intrude upon, or diminish any identified City resource.”

Planning has specifically determined “that the proposed Personal Wireless Service Facility

would not significantly detract from the character of the adjacent

residential/commercial/mixed-use Districts, Scenic Vistas; or potential and/or known historic

Buildings/Districts.”

Distance measurements

The Appelant’s home is located across York Street at 2880 25th Street at the northeast corner of

the intersection of York and 25th Streets. That home fronts upon 25th Street and has its westerly

side wall abutting York Street. The measured linear distance between the subject pole and the

closest wall of the appelant’s home is 45 feet, and the distance from the permitted equipment

and closest wall of that home is approximately 42.5 feet.

Public notification requirements

DPW Determination notification packages for the facility were both mailed and posted in strict

compliance with all Notification requirements and timeframes as stipulated in Article 25 of the

Public Works Code and Public Works Order No. 201970. The Notice posting and mailing

personnel separately submitted signed/dated affidavit packages affirming completion of the

noticing process in full compliance with Article 25 and DPW Order No. 201970.

Consideration of alternative locations

ExteNet conducts exhaustive research relative to each possible location within each facility search area and considers potential impacts to adjacent residences when evaluating pole usage. In this case, the selected pole was the least obtrusive option available, while still achieving ExteNet’s network coverage and capacity objectives. Shifting this facility to another pole would likely have resulted in reducing coverage and would also likely have resulted in the need to place two facilities in order to achieve the same level of coverage. Please also see

attached exhibit indicating surrounding poles that were evaluated by eventually rejected for

use due to a variety of criteria.

Obstruction of View/Blockage of Light

In this case, the radio equipment enclosure would be approximately 45 feet away from the nearest wall of the Appellant’s home, and the side-mounted antenna arm would be approximately 42.5 feet from that wall. These distances are substantially greater than the typical minimum distances referenced in DPW Article 25 and Order No. 201970 and no obstruction of view or blockage of light can reasonably be assumed to result from the permitted facility.

The permitted site for DPW PWSF 18wr-0369 complies with all requirements for obtaining a

Personal Wireless Service Facility Site Permit. Based on the facility design, the issued permit and the methodology for evaluation as described above, there is no evidence nor basis for reversing the Department of Public Works issuance of a telecommunications facility permit for this location. We respectfully request that the Board uphold SFDPW’s issuance of PWSF Permit

No. 18WR-0369. Thank you kindly for your consideration.

Public Works submits this response to Appeal No. 20-042 filed by appellant

Richard Segovia concerning Personal Wireless Service Facility Site Permit No. 18WR-

0369 (“Permit”) issued to ExteNet Systems (California) LLC under Article 25 of the

Public Works Code. The Permit authorized ExteNet Systems to install a Personal

Wireless Service Facility (“Wireless Facility”) on a utility pole in the vicinity of 2900 25th

Street. Public Works is submitting this brief even though appellant failed to submit a brief in support of his appeal. As discussed below, the Board of Appeals should deny the appeal because Public Works properly issued the Permit.

On December 28, 2018, ExteNet Systems submitted an application for Permit

No. 18WR-0369 (“Application”) to install a proposed Wireless Facility on a utility pole

Page 1 owned by Pacific Gas & Electric Company. As required by Article 25 of the Public

Works Code, Public Works reviewed the application and determined it was complete.

Public Works then referred the Application to the Planning Department and the

Department of Public Health (“DPH”).

The Planning Department found that the proposed Wireless Facility met the Tier

B (Zoning Protected Location) Compatibility Standard and, therefore, recommended that Public Works approve the Application. See Planning Department letter dated April

28, 2020 (Exhibit A). DPH reviewed the radio frequency and noise reports submitted by

ExteNet Systems and found that the proposed Wireless Facility would meet the Public

Health Compliance Standard. DPH recommended that Public Works approve the

Application. See Department of Public Health memorandum dated January 28, 2019

(Exhibit B.).

As a result, on June 4, 2020 Public Works issued a Final Determination

Approving the Application (Exhibit C). Thereafter, as required by Article 25, ExteNet

Systems mailed and posted Notice of the Final Determination. (See Mailing Affidavit and Certified Notification List (Exhibit D); and Certified Location of Posted Notices

Affidavit (Exhibit E).) On June 18, 2020, Public Works issued the Permit (Exhibit F).

As set forth above, Public Works issued the Permit after finding that the

Application complied with all of the requirements for a Personal Wireless Service Site

Permit contained in Article 25. For this reasons, Public Works requests that the Board of Appeals deny the appeal.

Department of Public Works Code Article 25 and Order No. 183,440 require review by the Planning Department to determine that the Application for a Tier A or Tier B Personal Wireless Service Facility Site Permit satisfies the applicable Tier A or Tier B Compatibility Standard. An Application for a Tier B Personal Wireless Service Facility Site Permit shall satisfy the Tier B Compatibility Standard for a Zoning and/or a Planning Protected Location, and shall not obstruct the view from or the light into any adjacent residential window. A proposed Personal Wireless Service Facility shall be consistent with the public health, safety, convenience and general welfare and will not unreasonably affect, intrude upon or diminish any identified City resource.

DETERMINATION The Planning Department determines that the proposed Personal Wireless Service Facility WOULD NOT significantly detract from the character of the adjacent residential/commercial/mixed-use Districts, Scenic Vistas; or potential and or known historic Buildings; Districts. The Planning Department recommends APPROVAL WITH CONDITIONS in conformance with architectural plans dated July 25, 2018 and photo simulations dated August 8, 2018, and associated with DPW Wireless Application No. 18WR-0369.

Findings:

The proposed Tier B Personal Wireless Service Facility is to be located in the public right-of-way in a Zoning Protected Location, adjacent to 2900 25th Street, and on a street with Average Street Views.

The proposed Personal Wireless Service Facility is, on balance, consistent with Article 25 of the Public Works Code and the Objectives and Policies of the General Plan, as follows:

Article 25 Compliance:

I. The proposed Personal Wireless Service Facility would not significantly detract from any of the defining characteristics of the Residential or Neighborhood Commercial zoning

district. This site is located within the RH-2 (Residential-House, Two Family) Zoning District.

The proposed T-Mobile personal wireless service facility would be situated within the RH-2 (Residential-House, Two Family) district. The RH-2 District is intended to recognize, protect, conserve and enhance areas characterized by dwellings in the form of houses, usually with one, two or three units with separate entrances, and limited scale in terms of building width and height. Such areas tend to have similarity of building styles and predominantly contain large units suitable for family occupancy, considerable open space, and limited nonresidential uses. These Districts are devoted to one-family and two-family houses, with the latter commonly consisting of two large flats, one occupied by the owner and the other available for rental. Structures are finely scaled and usually do not exceed 25 feet in width or 40 feet in height. Building styles are often more varied than in single-family areas, but certain streets and tracts are quite uniform. Considerable ground-level open space is available, and it frequently is private for each unit. The Districts may have easy access to shopping facilities and transit lines. Planning has determined that the proposed T-Mobile Mobility personal wireless service facility is designed in a streamlined manner, as proposed it will be located on an existing light pole which would not significantly detract from any of the defining characteristics of the RH-2 ( Residential-House, Two Family) district.

General Plan Compliance:

I. URBAN DESIGN ELEMENT The Urban Design Element concerns the physical character and order of the city, and the relationship between people and their environment. The Urban Design Element is concerned both with development and with preservation. It is a concerted effort to recognize the positive attributes of the city, to enhance and conserve those attributes and to improve the living environment where it is less than satisfactory. The Plan is a definition of quality, a definition based upon human needs.

OBJECTIVE 1 Emphasis of the characteristic pattern which gives to the City and its neighborhoods an image, a sense of purpose, and a means of orientation.

Policy 1.1 Recognize and protect major views in the city, with particular attention to those of open space and water.

OBJECTIVE 2 Conservation of resources which provide a sense of nature, continuity with the past, and freedom from overcrowding.

Policy 2.9 Review proposals for the giving up of street areas in terms of all the public values that streets afford.

OBJECTIVE 4 Improvement of the neighborhood environment to increase personal safety, comfort, pride and opportunity.

Policy 4.14 Remove and obscure distracting and cluttering elements.

The project has been designed to reduce, to the best extent possible, the blocking or other impairment of pleasing street views, and preserves an important characteristic of the unique setting and quality of the city.

The project has been designed to maintain, to the best extent possible, views from streets which can provide a means for orientation, and preserves the ability for an observer to perceive the City and its districts clearly.

II. TRANSPORTATION ELEMENT The Transportation Element concerns pedestrian movement in the city as to ensure the city is safe, convenient, and pleasant as pedestrian travel is an important component of the transportation system. The close-knit fabric of San Francisco, in junction with the dramatic hills and sweeping vistas, makes walking an ideal mode for exploring and moving about the city. The sidewalk is a shared space and provides a strong sense of the overall image of the city.

OBJECTIVE 23 Improve the city’s pedestrian circulation system to provide for efficient, pleasant, and safe movement.

Policy 23.5 Establish and enforce a set of sidewalk zones that provides guidance for the location of all pedestrian and streetscape elements, maintains sufficient unobstructed width for passage of people, strollers and wheelchairs, consolidates raised elements in distinct areas to activate the pedestrian environment, and allows sufficient access to buildings, vehicles, and streetscape amenities.

The project has been designed to maintain at least four (4) feet unobstructed width for pedestrian passage as outlined in the Pedestrian Network Streets and Design Guidelines of the Transportation Element.

OBJECTIVE 24 Improve the ambience of the pedestrian environment.

Policy 24.4 Preserve pedestrian-oriented building frontages.

The project has been designed as a minimally-visible facility to be attached to an existing light/utility pole extant in the public sidewalk. The facility components are designed as an extension to the pole and equipment boxes, with requirements to be painted or shrouded to match the pole further reducing their visibility and any conflicts with the building frontages within the RH-2 Zoning District.

Conditions:

1. Plant and maintain an appropriate street tree. 2. Antenna, and all equipment (external conduit, radio relay units, blinders used to shroud bracket bolts [if needed], and mounting mechanisms); except signage, if used for screening, shall all be painted to match the pole and repainted as needed. 3. Remove raised equipment signage (including filling in manufacturer logo indentations on radio relay units/cabinets) and equipment decals that may be visible from sidewalk and dwellings, unless required by government regulation. 4. Utilize smallest RF warning signage allowed (4 x 6 inches); and place the warning sticker facing out toward street, at a location as close to antenna as is feasible. Sticker shall face away from street, when not facing a nearby window within 15 feet. Background color of sticker shall match the pole-mounting surface; and logo and text shall be white. 5. Stack equipment enclosures (not including antenna) as close as allowed by applicable regulation and manufacturer equipment standards. 6. Seams and bolts/screws at antenna and shroud assembly area shall be fabricated and installed in a manner so as to reduce their visibility (e.g. flush mounting screws) from sidewalk level. 7. Not utilize any visible flashing indicator lights or similar. 8. Not obstruct the view from, or the light into any adjacent residential window. 9. New below ground enclosure excavations (vault), if utilized, shall not damage or remove granite curbs. No significant gaps shall be created between vault enclosure lid and primary sidewalk material due to installation. Any other existing historic architectural elements within the public right-of-way shall be retained and protected during installation. No carrier logo or carrier name may be placed on the vault lid. 10. Non-essential radio relay unit elements (handle and legs) shall be removed. 11. The installer shall arrange to have Planning Department staff review the initial installation, in order to ensure compliance with the aforementioned conditions (notwithstanding inspections by pole owner and Department of Public Works). 12. As requested, I have reviewed the documentation that you and Extenet have provided to me regarding the proposed installation of a CommScope Model VVSSP-65S-R1B, on a utility pole or similar structures located at the above listed location in the City and County of San Francisco.

This review includes August 29, 2018 radio frequency energy report prepared by Hammett and Edison Inc. for this site. The report states that one CommScope Model VVSSP-65S-R1B 2-foot tall directional cylindrical antenna will be mounted on an SFMTA pole near the location listed above. The antenna will be at 22.5 feet above ground level in the 170°T direction which is along York St. Due to the mounting location, the antenna would not be accessible to the general public.

The maximum effective radiated power from this antenna is estimated to be 188.4 watts.

The maximum calculated exposure level at the ground level will not exceed 0.013 mW/cm2, which is 1.3% of the FCC public exposure standard. The three dimensional perimeter of the radio frequency (RF) levels equal to the public exposure limit is calculated to extend a maximum of 9 feet from the face of the antenna and does not reach any publicly accessible areas. The maximum calculated exposure level at any nearby building is 2.8% of the FCC public exposure limit for the adjacent building, 12 feet away.

Based on the information provided in the Hammett and Edison report, I would agree that this CommScope antenna, light pole installation would be in compliance with the FCC standards and would not produce radio frequency energy exceeding the FCC public exposure limits.

In addition, a noise evaluation was done on the combination of equipment assumed to be installed at this location which was prepared by Hammett & Edison and was dated December 24, 2018. This evaluation found that the maximum noise level from the Ericsson Model 2203 cabinet is 38.5 dBA at a reference distance of 6.56 feet meaning that the applicable noise limit would be met for the configuration described above when placed at least 6.5 feet away from any building façade. As such, the installation of the equipment would be in compliance with the noise standards as outlined in the DPW Code, Article 25.

Approval Conditions:

 Ensure that any equipment associated with the pole installation of this antenna does not produce a noise in excess of 45 dBA as measured at three (3) feet from the nearest residential building façade.

 Ensure that there are no publicly occupied areas within nine (9) feet from the face of the antenna.

 Once the antenna is installed, Extenet must take RF power density measurements with the antenna operating at full power to verify the level reported in the Hammett and Edison report and to ensure that the FCC public exposure level is not exceeded in any publicly accessible area. This measurement must be taken again at the time of the permit renewal.

 Extenet should be aware that the general public may have concerns about the antenna and potential RF source near their dwellings. Extenet should have in place a procedure for taking RF power density levels in nearby dwellings when requested by the members of the general public.

 In accordance with the San Francisco Public Works Code, Art. 25, Sec. 1527 (a)(2)(C) AT&T is responsible for paying a fee of $210.00 to the San Francisco Department of Public Health for this review.

Public Works has made a determination that the Application satisfied the applicable requirements of the Public Works Code. The final approval includes the following condition(s):

San Francisco Public Works Conditions: 1. This recommendation is based on no variation from the depicted drawings and/or photo simulation; if a variation is different a re-submittal is required. Should the installation vary from said conditions, it should be resubmitted to Department(s) for further review and comment 2. New Poles: no new poles shall be erected or placed in underground districts. 3. Down Guys: Follow all excavation codes to obtain the necessary permits for placement of down guys. Down guy shall avoid crossing conflicting areas but not limited to driveways, curb ramps. 4. Comply with ADA code requirements for Federal, State, local laws. Make sure path of minimum required clear width for accessible path of travel is four feet. 5. At the conclusion of the work, provide a set of as built photos of the installation to the Bureau Street Use & Mapping Permit Office. 6. Maintain a valid certification of insurance annually and forward a copy to the Bureau Street Use & Mapping Permit Office.

San Francisco Planning Department Conditions: 1. Plant and maintain an appropriate street tree. 2. Antenna, and all equipment (external conduit, radio relay units, blinders used to shroud bracket bolts [if needed], and mounting mechanisms); except signage, if used for screening, shall all be painted to match the pole and repainted as needed. 3. Remove raised equipment signage (including filling in manufacturer logo indentations on radio relay units/cabinets) and equipment decals that may be visible from sidewalk and dwellings, unless required by government regulation. 4. Utilize smallest RF warning signage allowed (4 x 6 inches); and place the warning sticker facing out toward street, at a location as close to antenna as is feasible. Sticker shall face away from street, when not facing a nearby window within 15 feet. Background color of sticker shall match the pole-mounting surface; and logo and text shall be white. 5. Stack equipment enclosures (not including antenna) as close as allowed by applicable regulation and manufacturer equipment standards. 6. Seams and bolts/screws at antenna and shroud assembly area shall be fabricated and installed in a manner so as to reduce their visibility (e.g. flush mounting screws) from sidewalk level. 7. Not utilize any visible flashing indicator lights or similar. 8. Not obstruct the view from, or the light into any adjacent residential window. 9. New below ground enclosure excavations (vault), if utilized, shall not damage or remove granite curbs. No significant gaps shall be created between vault enclosure lid and primary sidewalk material due to installation. Any other existing historic architectural elements within the public right- of-way shall be retained and protected during installation. No carrier logo or carrier name may be placed on the vault lid. 10. Non-essential radio relay unit elements (handle and legs) shall be removed. 11. The installer shall arrange to have Planning Department staff review the initial installation, in order to ensure compliance with the aforementioned conditions (notwithstanding inspections by pole owner and Department of Public Works). 12. San Francisco Public Works has finally approved the above-referenced Application for a Personal Wireless Service Facility Site Permit in the vicinity of 2900 25TH ST submitted by ExteNet Systems, Inc.

The antenna and other equipments to be installed at this location include: One (1) 4G antenna and equipment enclosures (radios, meter, and disconnect switch). A photo-simulation of the approved Personal Wireless Service Facility is attached hereto.

Public Works has made a determination that the Application satisfied the applicable requirements of the Public Works Code. The final approval includes the following condition(s):

San Francisco Public Works Conditions: 1. This recommendation is based on no variation from the depicted drawings and/or photo simulation; if a variation is different a re-submittal is required. Should the installation vary from said conditions, it should be resubmitted to Department(s) for further review and comment 2. New Poles: no new poles shall be erected or placed in underground districts. 3. Down Guys: Follow all excavation codes to obtain the necessary permits for placement of down guys. Down guy shall avoid crossing conflicting areas but not limited to driveways, curb ramps. 4. Comply with ADA code requirements for Federal, State, local laws. Make sure path of minimum required clear width for accessible path of travel is four feet. 5. At the conclusion of the work, provide a set of as built photos of the installation to the Bureau Street Use & Mapping Permit Office. 6. Maintain a valid certification of insurance annually and forward a copy to the Bureau Street Use & Mapping Permit Office.

San Francisco Department of Public Health Conditions: • Ensure that any equipment associated with the pole installation of this antenna does not produce a noise in excess of 45 dBA as measured at three (3) feet from the nearest residential building façade.

• Ensure that there are no publicly occupied areas within nine (9) feet from the face of the antenna.

• Once the antenna is installed, Extenet must take RF power density measurements with the antenna operating at full power to verify the level reported in the Hammett and Edison report and to ensure that the FCC public exposure level is not exceeded in any publicly accessible area. This measurement must be taken again at the time of the permit renewal.

• Extenet should be aware that the general public may have concerns about the antenna and potential RF source near their dwellings. Extenet should have in place a procedure for taking RF power density levels in nearby dwellings when requested by the members of the general public.

• In accordance with the San Francisco Public Works Code, Art. 25, Sec. 1527 (a)(2)(C) AT&T is responsible for paying a fee of $210.00 to the San Francisco Department of Public Health for this review.

Please note that this approval and any conditions apply only to the equipment and installation as described. If any changes in the equipment or any increase in the effective radiated power described above are made, a new review by the Department of Public Health must be conducted.

San Francisco Planning Department Conditions: 1. Plant and maintain an appropriate street tree. 2. Antenna, and all equipment (external conduit, radio relay units, blinders used to shroud bracket bolts [if needed], and mounting mechanisms); except signage, if used for screening, shall all be painted to match the pole and repainted as needed. 3. Remove raised equipment signage (including filling in manufacturer logo indentations on radio relay units/cabinets) and equipment decals that may be visible from sidewalk and dwellings, unless required by government regulation. 4. Utilize smallest RF warning signage allowed (4 x 6 inches); and place the warning sticker facing out toward street, at a location as close to antenna as is feasible. Sticker shall face away from street, when not facing a nearby window within 15 feet. Background color of sticker shall match the pole-mounting surface; and logo and text shall be white. 5. Stack equipment enclosures (not including antenna) as close as allowed by applicable regulation and manufacturer equipment standards. 6. Seams and bolts/screws at antenna and shroud assembly area shall be fabricated and installed in a manner so as to reduce their visibility (e.g. flush mounting screws) from sidewalk level. 7. Not utilize any visible flashing indicator lights or similar. 8. Not obstruct the view from, or the light into any adjacent residential window. 9. New below ground enclosure excavations (vault), if utilized, shall not damage or remove granite curbs. No significant gaps shall be created between vault enclosure lid and primary sidewalk material due to installation. Any other existing historic architectural elements within the public right- of-way shall be retained and protected during installation. No carrier logo or carrier name may be placed on the vault lid. 10. Non-essential radio relay unit elements (handle and legs) shall be removed. 11. The installer shall arrange to have Planning Department staff review the initial installation, in order to ensure compliance with the aforementioned conditions (notwithstanding inspections by pole owner and Department of Public Works). 12. Ensure Wi-Fi Access Points and associated wiring, utilized by the City’s Department of Technology, are not damaged during installation (if present). 13. Should the installation vary from said conditions, the application shall be resubmitted to the Planning Department for further review and comment.

Within fifteen (15) calendar days of the issuance of this notice, any person may appeal the issuance of this permit to the Board of Appeals. Appeals must be filed in person by either the appellant or the appellant’s agent. Generally, the Board of Appeals requires that an appointment be made to file an appeal. NOTICE OF FINAL DETERMINATION TO APPROVE A PERSONAL WIRELESS SERVICE FACILITY SITE PERMIT

San Francisco Public Works has finally approved the above-referenced Application for a Personal Wireless Service Facility Site Permit in the vicinity of 2900 25TH ST submitted by ExteNet Systems, Inc.

The antenna and other equipments to be installed at this location include: One (1) 4G antenna and equipment enclosures (radios, meter, and disconnect switch). A photo-simulation of the approved Personal Wireless Service Facility is attached hereto.

Public Works has made a determination that the Application satisfied the applicable requirements of the Public Works Code. The final approval includes the following condition(s):

San Francisco Public Works Conditions: 1. This recommendation is based on no variation from the depicted drawings and/or photo simulation; if a variation is different a re-submittal is required. Should the installation vary from said conditions, it should be resubmitted to Department(s) for further review and comment 2. New Poles: no new poles shall be erected or placed in underground districts. 3. Down Guys: Follow all excavation codes to obtain the necessary permits for placement of down guys. Down guy shall avoid crossing conflicting areas but not limited to driveways, curb ramps. 4. Comply with ADA code requirements for Federal, State, local laws. Make sure path of minimum required clear width for accessible path of travel is four feet. 5. At the conclusion of the work, provide a set of as built photos of the installation to the Bureau Street Use & Mapping Permit Office. 6. Maintain a valid certification of insurance annually and forward a copy to the Bureau Street Use & Mapping Permit Office.

San Francisco Department of Public Health Conditions: Ensure that any equipment associated with the pole installation of this antenna does not produce a noise in excess of 45 dBA as measured at three (3) feet from the nearest residential building façade.

Ensure that there are no publicly occupied areas within nine (9) feet from the face of the antenna.

Once the antenna is installed, Extenet must take RF power density measurements with the antenna operating at full power to verify the level reported in the Hammett and Edison report and to ensure that the FCC public exposure level is not exceeded in any publicly accessible area. This measurement must be taken again at the time of the permit renewal.

Extenet should be aware that the general public may have concerns about the antenna and potential RF source near their dwellings. Extenet should have in place a procedure for taking RF power density levels in nearby dwellings when requested by the members of the general public.

In accordance with the San Francisco Public Works Code, Art. 25, Sec. 1527 (a)(2)(C) AT&T is responsible for paying a fee of $210.00 to the San Francisco Department of Public Health for this review.

Please note that this approval and any conditions apply only to the equipment and installation as described. If any changes in the equipment or any increase in the effective radiated power described above are made, a new review by the Department of Public Health must be conducted.

San Francisco Planning Department Conditions: 1. Plant and maintain an appropriate street tree. 2. Antenna, and all equipment (external conduit, radio relay units, blinders used to shroud bracket bolts [if needed], and mounting mechanisms); except signage, if used for screening, shall all be painted to match the pole and repainted as needed. 3. Remove raised equipment signage (including filling in manufacturer logo indentations on radio relay units/cabinets) and equipment decals that may be visible from sidewalk and dwellings, unless required by government regulation. 4. Utilize smallest RF warning signage allowed (4 x 6 inches); and place the warning sticker facing out toward street, at a location as close to antenna as is feasible. Sticker shall face away from street, when not facing a nearby window within 15 feet. Background color of sticker shall match the pole-mounting surface; and logo and text shall be white. 5. Stack equipment enclosures (not including antenna) as close as allowed by applicable regulation and manufacturer equipment standards. 6. Seams and bolts/screws at antenna and shroud assembly area shall be fabricated and installed in a manner so as to reduce their visibility (e.g. flush mounting screws) from sidewalk level. 7. Not utilize any visible flashing indicator lights or similar. 8. Not obstruct the view from, or the light into any adjacent residential window. 9. New below ground enclosure excavations (vault), if utilized, shall not damage or remove granite curbs. No significant gaps shall be created between vault enclosure lid and primary sidewalk material due to installation. Any other existing historic architectural elements within the public right- of-way shall be retained and protected during installation. No carrier logo or carrier name may be placed on the vault lid. 10. Non-essential radio relay unit elements (handle and legs) shall be removed. 11. The installer shall arrange to have Planning Department staff review the initial installation, in order to ensure compliance with the aforementioned conditions (notwithstanding inspections by pole owner and Department of Public Works). 12. Mga Kundisyon ng San Francisco Department of Public Health: Tiyakin na ang anumang kagamitang kaugnay ng pagkakabit ng poste ng antenang ito ay hindi gagawa ng ingay na lagpas sa 45 dBA ayon sa nasukat sa tatlong (3) talampakan mula sa pinakamalapit na harapan ng residensiyal na gusali.

Kapag nakalagay na ang antena, dapat magsagawa ang Extenet ng mga pagsusukat sa RF power density habang ang antena ay gumagana sa full power upang mapatotohanan ang lebel na naulat sa ulat ng Hammett and Edison at para matiyak na ang FCC na lebel sa pagkalantad ng publiko ay hindi lalagpas sa anumang lugar na mapupuntahan ng publiko. Dapat muling gawin ang pagsukat na ito sa panahon ng pag-renew ng pahintulot.

Dapat alam ng Extenet na maaaring may alalahanin ang pangkalahatang publiko tungkol sa antena at potensiyal na pagmumulan ng RF na malapit sa mga tirahan nila. Dapat magtalaga ang Extenet ng mekanismo sa pagkuha ng mga lebel ng RF power density sa malalapit na tirahan kapag hiniling ng mga miyembro ng pangkalahatang publiko.

Ayon sa San Francisco Public Works Code, Art. 25, Sec. 1527 (a)(2)(C) responsable ang AT&T sa pagbabayad ng bayaring $210.00 sa San Francisco Department of Public Health para sa pagrepasong ito.

Pakitandaan na ang pag-aprubang ito at anumang mga kundisyon ay para lamang sa kagamitan at instalasyon ayon sa isinalarawan. Ang mga dugtungan at mga bolt/screw sa antenna at assembly ng shroud ay ipasasadya at ikakabit sa paraan na makakabawas sa kanilang visibility (hal. mga flush mounting screw) mula sa lebel ng bangketa. 7. Huwag gumamit ng anumang makikitang kumukutitap na indikasyong ilaw o katulad nito. 8. Huwag harangan ang paningin mula sa o ilaw papunta sa anumang malapit na bintana ng tirahan. 9. Ang bagong nasa ilalim ng lupang hukay ng enclosure (vault), kapag ginamit, ay hindi sisira o aalis ng mga granite curb. Walang makabuluhang puwang ang gagawin sa pagitan ng vault enclosure lid at pangunahing materyal ng bangketa dahil sa pag-install.
