It is our hope that we will be able to complete the NAME His- tory ebook for the occasion of the Golden Anniversary Meeting in 2016. Al- though we revised and updated articles, we will still require more “filling in” effort to cover more ME offices, memoirs, special topics and dedication to specific his- toric pioneer and members. We sent out to the membership specifically seeking the following information. Committee Plan for History of Medical Examiners Offices Subsection I – a. Statewide Medical Examiner’s Office – History including the establishment of the State wide Medical Examiner’s system, and personalities prior to the changes, before the ME system, and community support to conversion into the Statewide ME office, First Chief Medical Examiner and subsequent chiefs. Office, type of cases, relationship to the Counties. Subsection I – b History of the City or County ME Office. Subsection I - c Authors who are interested in writing the history and develop- ment of medical examiner system in your state. We are aware of some states may have ME offices and Coroner’s offices in the state. Subsection I-d medical examiner in the U.S. Territories. Section II Historical review of development of each states Section III – Special topics series Starting 2013, we began includes special topics, Women in the History of the NAME, now we seek for the Standards, Inspection and Accreditation, Federal Pro- grams and others. ii In addition, we are seeking articles following: 1. History of Inspection and Accreditation Program, 2. Dr. Lukash’s unpublished article on the Standards presented in the NAME Meeting in 1970 (Tom will be adding this article) 3. Development of DNA Laboratory in the Armed Forces ME Office, 4. The American Board of Pathology establishing subspecialty in Forensic Pathology and Certification Program in Forensic Pathology 5. NAME Research and Development program 6. Who were the first pathologists in each state, prior to the establish ME. 7. History of the NAME Officers, Presidents, Vice Presidents, Secretary – Treasurer by the year and photographs 8. History of Office of the Armed Forces Medical Examiner 9. The NAME foundation 10. NAME Annual Meetings and Interim Meetings 11. Milton Helpern Laureate History of beginning, and Named lectureship, George Gantner lecturer, names of honorees and year presented, William G. Eckert named awards 12 History of membership in affiliated membership, investigative, consultants, and administrative categories. Section VI – Memoir series, We will continue to seek for your memoir articles from the senior members of the NAME. The editorial team is interested in more of your memoirs. Section IV – Dedication series We are interested your articles on the NAME pioneers of immediate teachers or founders, such as Drs. Helpern, Spelman, Lukash, and early leaders. We are also interested in having dedication article of pioneer pathologists who supported im- provement of the death investigative system. iii We are interested in knowing more about chief medical examiners in the early days, such as Al Majoska of Hawaii, Don Condon of Arizona, etc. Many pio- neers prior to the NAME is born, such as Ruddy Muelling, I noted that we are seeking articles on more comprehensive topics that requires re- search of he data, specially in special topics, I would like to express my apprecia- tion to all 2014 contributors and specially to Jerry Spencer for spent many hours and visiting sources places and archives to complete the history of the military medical examiner’s system. The committee is interested in receiving articles on history of major medical examiner’s offices, overview of each state death investiga- tion system, and further special topics. On the occasion of publishing the forth edition of the History eBook, I would like to express my sincere appreciation to the past presidents who have proof read and edited the submitted articles. Those past presidents are Randy Hanzlick, Jeffry Jentzen, Marcella Fierro as regular proof readers, but for this year, we asked Andy Baker to help us. Another appreciation is extended to our project coordinator, Denise McNally and graphic designer Raul Vergara. Raul Vergara has been assist- ing graphic layout for NAME Foundation letter as well annual history eBook production. He and I have regularly meet and discussed the way and means to improve the eBook. Raul Vergara recommended me to use new Apple eBook for- mat for 2014 edition. It works well in iPad and iPhone, off course, we will have pdf for computer to read. I would like each one of the NAME organization, please contribute your write up about your office, register in the NAME eBook, and you will be able to update annually. The production team is intended to refer back to each author in Sep- tember 2015 for review of previously submitted article and revise or add more in- formation for 2016 final edition. iv We will discuss during the Past Presidents lunch in September 2014, so we are mindful of only two more years left to be complete. As I look back to the First Edition of the NAME Past Presidents Committee Annual History Project done in 2011, we began with guided autobiographies. Seventeen past presidents submitted their memoirs following the guided questions prepared by Randy Hanzlick. The Publication Team consists of Randy Hanzlick, Jeffrey Jentzen and myself. Production team consists of coordinator, Denise McNally and graphic de- signer Raul Vergara of Los Angeles. In 2012, we added two more sections, i.e. His- tory of Medical Examiner’s Office and dedication. The total pages were over 300. In 2013, we have further improved by adding more sections. We now call Chap- ters. Chapter I is newly added audio and video series. I obtained an audiotape from my friend and colleague, Amnon Carmi of Haifa, Israel, who asked me if I could use this audiotape that Milton Helpern gave me. William G. Eckert re- corded these audiotapes in 1970. One recording was Milton Halpern’s comments on why he decided to establish an organization exclusive for the medical examiner officers and system. The tape may have been deteriorated; some areas were not clearly audible. Randy Hanzlick spent many hours listening to the video, and com- pared with initially transcribed audio by Denise McNally. He suggested if some- one had not seen nor heard Dr. Helpern, we should use subtitles, and after 4 or 5 revisions of subtitles, further suggesting introducing Mr. Richard Child, who was the Director of the Municipal League and instrumental for the idea of creating a new organization, NAME. Chapter II is also new. This series deals with specific topics, such as women in the history of the NAME, Helpern Laureate Award and Gantner lecturer, etc. Chapter III is the history of the Medical Examiner’s Office, but for 2013, we divided into the State-Wide Medical Examiner system from the County Medical Examiners Officers. Chapter IV is also new. I solicited contribu- tions on the overview of history for each state. We had an article by Joe Prahlow and John Pless regarding State of Indiana, and now adding the State of Arizona, and of Pennsylvania. Chapter V is memoir series and Chapter VI is expanded into the dedication of deceased colleagues by co-workers or students dedicating the accomplishment. I decided to use collected obituaries filed from the NAME vi Date Committee. As we increase edition, we should be able to fill in, and complete our History. Review and proof reading is an important job. For 2013, we asked Past President Marcella Fierro to assist the team in proof reading. Raul Vergara, graphic de- signer who is in charge of layout and selection of software for eBook, as new soft- ware is becoming improved, and popularity of iPad and other devices, we are now able to include video recording into the eBook. As all editions, Jeff Jentzen totally reviewed prior to release of eBook 2013. In the Past Presidents Luncheon and committee meeting, we will work on eBook 2014. We have only three more years to complete the NAME History Book, hopefully published in the occasion of the 50th Anniversary Meeting. Thomas T. Noguchi, MD September 13, 2014 vii Preface to the 2012 NAME History eBook Second Edition The Past Presidents Committee has been continuing its effort of compiling the NAME history for the occasion of the 2016 NAME Meeting’s 50th Golden Anni- versary Meeting. The Committee began collecting historical materials and now so- licits the histories of individual NAME Members in the format of a guided autobi- ography, i.e. memoir. Seventeen past presidents have already contributed their memoirs, which were publish in a eBook in 2011. We continued the same guided autobiography format for compiling historical ma- terial, and now have additional memoirs to add also. This year, the book will be combined with the 2011 material, and some previous chapters have been updated.
