The problems are obvious: many thousands of articles are published every year, yet the clinician has very little time available for reading, digesting, and assimilating new developments. How can the busy clinician keep up? The Editorial Board of Current Medical Literature – Lysosomal Storage Diseases seeks, on a bi-annual basis, to distil the best of the world literature on all clinical aspects of this subspeciality – with a special emphasis on mucopolysaccharidosis type 1 for this volume – into a manageable, easy-to-read journal. The Editors do more, however, than simply summarize the vital papers of the day – they also place them into context, helping the busy clinician relate the findings of research studies to their day-to-day practice. The Editors are all practicing clinicians themselves, and they know only too well how hard it can be to juggle the many and varied demands of modern clinical practice.     On the Inheritance of Hand and Foot Anomalies in Six Families OLA JOHNSTON AND RALPH WALDO DAVIS Department of Biology, North Texas State College, Denton, Texas INTRODUCTION Malformations of the hands and feet are common and of many kinds. Ac- cording to Gates (1946) there probably are more abnormalities of the hands and feet than of any other part of the body, with the exception of the eye. It is true that some hand and foot anomalies are the result of accident and disease but it is equally true that many are the result of variation in heredity. The extent to which the latter is true and the mode of inheritance of those variations which have some genetic basis are questions which are not com- pletely answered. Hence when an opportunity presented itself to study a number of different hand and foot anomalies which appeared to have a he- reditary basis, it seemed worthwhile to investigate them and to present the findings. The malformations which are included are syndactyly and split hand and foot, polydactyly, and brachydactyly. Each will be considered more or less independently and in the order indicated. A BRIEF REVIEW OF LITERATURE 1. Syndactyly and Split Hand and Foot Syndactyly is the condition in which two or more fingers or toes are adherent or are more or less completely grown together. Split hand and foot (also called lobster claw) is a deformity in which the central digits of the hands and/or feet are lacking. It may represent an extreme variant of syndactyly. According to Lewis (1909) a description of split hand and foot is difficult because of the great variation in the deformity even within the same family.

    (Jurnal Plastik Rekonstruksi, 2021; Vol 8, No 1, 1-6) CRANIOFACIAL CASE SERIES APERT SYNDROME: CASE SERIES AND REVIEW OF THE LITERATURE Silvina, Rizka Khairiza, & Muhammad Rizky Setyarto*) Division of Plastic Reconstructive and Aesthetic Surgery, Dr. Kariadi Central-General Hospital, Semarang, Indonesia ABSTRACT Summary: Apert syndrome is a type 1 acrocephalosyndactyly, a rare syndrome characterized by the presence of multiple craniosynostoses, dysmorphic facial manifestations, and syndactyly of hand and feet. It affects 1:100.00 of birth and the second most common of syndromic craniosynostosis. Molecular genetic tests that identify the heterozygous pathogenic variant in FGFR2 genes - identical with Apert syndrome cost too high to be applicable in developing countries. Therefore, the diagnosis of Apert syndrome should be suspected from the clinical findings. Three cases from the Community of Indonesian Apert Warrior Group were collected. These series were based on medical and surgical records. We obtained the patient characteristic from the phenotypic manifestations only. We present cases of 6-years-old male, 2-years-old female, and 3-years-old female, respectively, with similar anatomical findings, such as skull shape abnormality, midface hypoplasia, intraoral disfigurement, and hands and feet deformities that resemble Apert Syndrome. Our series presents similar Apert syndrome characteristics, such as typical craniofacial dysmorphic with symmetrical syndactyly of both upper and lower extremities. These clinical findings are essential to establish an initial diagnostic of Apert Syndrome. Rheumatology This is the branch of medicine dealing with arthritis and related disorders of the musculoskeletal system including the multi system autoimmune diseases. Rheumatologists are medical specialists in musculoskeletal disease. C. Value of the History and Physical The history and physical are critical to arriving at an accurate diagnosis of musculoskeletal disorders. Although the same might be said for all systems, it is particularly true in this area. In the diagnosis of musculoskeletal disease, 70% of the weight might be placed on the history, 20% on the physical and 10% on laboratory data. II. TERMS-YOU SHOULD BE FAMILIAR WITH A. It leads not only to secondary distortion of skull shape but to various complications including neurologic, ophthalmic and respiratory dysfunction. Craniosynostosis is very heterogeneous in terms of its causes, presentation, and management. Both environmental factors and genetic factors are associated with development of craniosynostosis. Nonsyndrom- ic craniosynostosis accounts for more than 70% of all cases. Syndromic craniosynostosis with a certain genetic cause is more likely to involve multi- ple sutures or bilateral coronal sutures. FGFR2, FGFR3, FGFR1, TWIST1 and EFNB1 genes are major causative genes of genetic syndromes associat- ed with craniosynostosis. Although most of syndromic craniosynostosis show autosomal dominant inheritance, approximately half of patients are de novo cases. Apert syndrome, Pfeiffer syndrome, Crouzon syndrome, and Antley-Bixler syndrome are related to mutations in FGFR family (especially in FGFR2), and mutations in FGFRs can be overlapped between different syndromes. Saethre-Chotzen syndrome, Muenke syndrome, and cranio- frontonasal syndrome are representative disorders showing isolated coronal suture involvement. Compared to the other types of craniosynostosis, single gene mutations can be more frequently detected, in one-third of coronal synostosis patients. Molecular diagnosis can be helpful to provide adequate genetic counseling and guidance for patients with syndromic craniosynostosis.
