ABOUT THE COURSE TRAUMA, CRITICAL CARE & ACUTE CARE SURGERY 2019 April 15 - 17, 2019 This annual Las Vegas Trauma, Critical Care & Acute Care Surgery Confer- ence has become the best attended such course in the world, being oversub- scribed the last 13 years. Of the more than 1300 attendees, over 80% are . Our faculty are physicians in active practice who continue “in the trenches” day after day, night after night, dealing with the most complex surgical challenges. Each year, new “rising stars” join seasoned expert faculty. As you visit the Scientific Program section of our site, you will be impressed that such a stellar faculty has been assembled and how so much practical surgical information has been incorporated into two and one-half days. Past attendees have aptly described the conference content as an, “Incredible journey in surgical knowledge, with “tour guides” whose expertise and personalities make every excursion a uniquely exceptional experience,” “Amazingly wonderful, fast paced experience - like drinking water from a fire hose; it blew me away!” No other course like this in the world; can’t wait to return next year.” As we enter our 52 nd record-breaking year, it is appropriate that the continued focus be TRAUMA, CRITICAL CARE, & ACUTE CARE SURGERY. For every community, there continues to be a need for surgical and medical teams to evaluate and treat the sickest of the sick and the severely injured. With many graduates going into practices based primarily in minimally invasive surgical , it is appropriate that this course take a leadership role in provid- ing a continuing education venue for the complex operative and critical care surgical challenges, with continuing to be the epitome of such challenges. GOALS This program offers comprehensive continuing education in the treatment of critically ill and injured patients, stressing current basic and cutting edge guidelines and technology for evaluation, diagnosis, and management. The course is designed to enhance the knowledge and skills of those caring for ill and injured patients in rural, urban, and suburban hospitals.

9 OBJECTIVES 1) Describe innovative, appropriate techniques and technology for optimal care of the injured or seriously ill patient in urban and rural environments, 2) Apply concepts from urban and rural trauma and acute care surgery cases to the practice setting, 3) Describe practical exposure techniques and guidelines for management and early control of injuries and acute surgical conditions, 4) Discuss appropriate surgical response and timing of intervention for diverse traumatic injures and critical illnesses, 5) Identify the dilemmas, ethics, and solutions relative to managing critically ill and injured patients, 6) Discuss care issues particular to the surgical intensive care unit, including rapid response team, ECMO, appropri- ate antibiotic use, fungal infections, DVT prophylaxis, rib fixation, vent , and non-evacuated clotted hemothorax, 7) Address issues relative to trauma and acute care surgery evolution, including hybrid rooms, , abdominal , small bowel obstruction, orthopedic infections, and minimal TBI, 8) Recognize and evaluate management of complicated conditions, including nonocclusive mesenteric ischemia, pelvic fracture bleeding, the geriatric trauma patient, as well as patients with comorbidities, mediastinal traverse, caustic burns of upper GI tract, severe necrotizing faciitis and the pregnant trauma patient, 9) Discuss how to respond to unique situations including pediatric trauma patients in adult trauma centers, OR and ICU downstream effects of field/EC administered medications, value of practice guidelines, and recognizing futility, 10) Discuss ongoing advances and the controversies surrounding them, including appropriate management of the unstable patient in the EC, appropriate of REBOA, opioids for trauma patients, strategies, nonoperative management, and TXA, 11) Address evolving nonclinical issues, including maintaining autonomy in times over regulation the risks of risk assessment, and, unblinding trauma center outcomes, 12) Discuss innovations, including extended use of ultrasound, social media in acute care surgery, simulation training, and how advances in lead to advances in trauma patient care. EDUCATIONAL METHODS Didactic lectures, panel discussions, audiovisual aids, course syllabus, and MOC exam TARGET AUDIENCE The course is designed primarily for trauma surgeons working in Level I-IV trauma centers. In addition, general surgeons, emergency physicians, anesthesiologists, trauma coordinators, surgical residents, thoracic surgeons, trauma nurses, PAs and NPs, administrators, and EMS coordi- nators will benefit from this course.

MEDICAL DISASTER RESPONSE 2019 Sunday, April 14, 2019

MEDICAL DISASTER RESPONSE 2019 has been developed by the group that brings you Trauma, Critical Care & Acute Care Surgery. This course made its debut in 2008 and was an immediate sell-out. It is specifically for clinical personnel who, in the “heat of battle,” are trying to save patients’ lives and limbs, often with limited space and workers, while trying to quickly determine how to manage more patients than the facility can process. Whether it is an overwhelmingly busy Friday night or a full-scale mass casualty, the clinical personnel are on the front line.

Course faculty are selected for their first-hand experience in medical disaster response, and the program is designed to present maximum content in formats most conducive to learning and audience involvement.

Physicians, nurses, administrators, and others in the hospital and clinical arenas will, sooner or later, be faced with the care of multiple patients following a natural, terrorism, industrial, or other catastrophic event that might be considered overwhelming mass casualty or “disaster.” Many journals, courses, and event “model” disaster plans address infrastruc- ture, supplies, causes, public health, and logistics. However, the medical/clinical aspects, the most important determinant of survival, are often overlooked.

This course focuses on these aspects and targets the clinical personnel who are the first to receive the victims of a catastrophic event. Whether it is a shooting on a military base or a devastating earthquake, the roles of the individuals responsible for immediate medical response will be the focus. Every physician, nurse and administrator working in a hospital clinical setting has the potential to be called upon, without notice, to fulfill these responsibilities.

OBJECTIVES

1) Develop strategies to meet challenges of triage, limited resources, the active shooter, and making critical decisions under heavy media/public scrutiny, 2) Discuss blood use, crisis standards, and teamwork in mass casualty response, 3) Modify individual situation responses based on data from past responses, 4) Gain a knowledge of resources available from government agencies in mass casualty response; 5) Discuss specific clinical response in MCI, including crush injuries, multiple fractures, rural response, and scene amputa- tions; 6) Discuss military lessons that have been/can be effectively applied to civilian disaster response.
