It also lowers the convulsive threshold and causes alterations in EEG and sleep patterns. Orally administered amitriptyline is readily absorbed and rapidly metabolized. Steady-state plasma concentrations vary widely and this variation may be genetically determined. Amitriptyline is primarily excreted in the urine, mostly in the form of metabolites, with some excretion also occurring in the feces. INDICATIONS AND CLINICAL USE ELAVIL® (amitriptyline hydrochloride) is indicated in the drug management of depressive illness. ELAVIL® may be used in depressive illness of psychotic or endogenous nature and in selected patients with neurotic depression. Endogenous depression is more likely to be alleviated than are other depressive states. ELAVIL® ®, because of its sedative action, is also of value in alleviating the anxiety component of depression. As with other tricyclic antidepressants, ELAVIL® may precipitate hypomanic episodes in patients with bipolar depression. These drugs are not indicated in mild depressive states and depressive reactions.

    Tricyclic Antidepressant Overdose R. Kevin Smith, DO, and Karen O'Mara, DO Chicago, Illinois Overdose from tricyclic antidepressants (TCAs) is increasing. TCAs are well absorbed orally, highly protein bound, and high­ ly lipid soluble. Clinical features of poisoning with TCAs occur within 12 hours of ingestion, usually after a dose of 20 mg/kg or more. Clinical symptomatology involves various anticholiner­ gic, central nervous system, and cardiovascular effects. Car­ diovascular toxicity accounts for the majority of the fatalities and may include a hyperdynamic response, various arrhyth­ mias and heart blocks, or severe hypotension. Prolongation of the QRS interval of 10 msec or more implies severe toxicity. Many factors limit the usefulness of drug levels in the over­ dosed patient. Treatment revolves around good supportive care and general poisoning management. The physician should no longer use physostigmine precipitously. Sodium bicarbon­ ate is effective in treating many of the cardiovascular compli­ cations. Other cardiac drugs are used but with varying effi­ cacy. Patients with significant signs or symptoms of toxicity require monitored hospitalization until clinically free of mani­ festations for 24 to 48 hours. Untreated depression is associated with substantial morbidity and increases the risk of suicide and death. An estimated 15% of mood disorders end in suicide.1 Depression is a biologic illness with an unknown cause. Depression occurring secondary to medications is similar in presentation to endogenous depression and carries similar risks of morbidity and mortality. While the overall prevalence is unknown, drug-induced depression poses a significant challenge for practitioners, as it may undermine the effectiveness of much-needed treatment. Follow patients for signs and symptoms of respiratory depression and sedation (see WARNINGS and PRECAUTIONS). • The use of benzodiazepines, including chlordiazepoxide and amitriptyline hydrochloride tablets, exposes users to risks of abuse, misuse, and addiction, which can lead to overdose or death. Abuse and misuse of benzodiazepines commonly involve concomitant use of other medications, alcohol, and/or illicit substances, which is associated with an increased frequency of serious adverse outcomes. Before prescribing chlordiazepoxide and amitriptyline hydrochloride tablets and throughout treatment, assess each patient’s risk for abuse, misuse, and addiction (see WARNINGS). • The continued use of benzodiazepines, including chlordiazepoxide and amitriptyline hydrochloride tablets, may lead to clinically significant physical dependence. The risks of dependence and withdrawal increase with longer treatment duration and higher daily dose. Abrupt discontinuation or rapid dosage reduction of chlordiazepoxide and amitriptyline hydrochloride tablets after continued use may precipitate acute withdrawal reactions, which can be life-threatening. To reduce the risk of withdrawal reactions, use a gradual taper to discontinue chlordiazepoxide and amitriptyline hydrochloride tablets or reduce the dosage (see DOSAGE AND ADMINISTRATION and WARNINGS). • Antidepressants increased the risk of suicidal thoughts and behaviors in pediatric and young adult patients in short-term studies.

    SINEQUAN® (doxepin HCl) CAPSULES ORAL CONCENTRATE Suicidality and Antidepressant Drugs Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Anyone considering the use of Sinequan or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Sinequan is not approved for use in pediatric patients. Inert ingredients for the capsule formulations are: hard gelatin capsules (which may contain Blue 1, Red 3, Red 40, Yellow 10, and other inert ingredients); magnesium stearate; sodium lauryl sulfate; starch.
