GENERAL DEFINITION OF TERMS Quantity Limits (QL) – Quantity limits have been placed on medications to be consistent with the maximum dosage that the Food and Drug Administration (FDA) has approved to be both safe and effective. Step Therapy (ST) – Step therapy is an electronic prior authorization (PA) process that takes place at the time the pharmacy submits the claim. Maximum Duration (MD) – Medications indicated will be available for a defined period of days per rolling year (365 days) before requiring a new or additional PA. Age Edit (AE) – Medications indicated are available for members above or below XX age without PA.     Disposal of Solid Chemicals in the Normal Trash Many solid chemicals can be safety discarded into the normal trash, provided they are in containers that are not broken or cracked and have tightly fitting caps. These chemicals are considered acceptable for ordinary disposal because they display none of the properties of hazardous waste, are of low acute toxicity, and have not been identified as having any chronic toxic effects as summarized in the National Institute of Occupational Safety and Health (NIOSH) “Registry of Toxic Effects of Chemical Substances”. Examples of chemicals acceptable for disposal as regular trash are listed below. To dispose of these chemicals, place the containers in a box lined with a plastic bag, tape the top of the box shut, write “Normal Trash” on the box and then place the box next to the lab trash container. Only solid forms of these chemicals can be disposed in this manner. Therefore careful attention is required in prescription and administration of calcium to avoid over- or under-dosing. Conversion factor for elemental Ca: 1 mg = 0.025 mmol = 0.05 mEq. Do not mix with any medication that contains phosphates, carbonates, sulfates or tartrates. Separate doses of the following by at least 2 hours: phosphate, iron, thyroxine and phenytoin. Action Calcium is essential for the functional integrity of the nervous, muscular, skeletal and cardiac systems and for clotting function. Drug Type Mineral. Trade Name CalSource Ca1000 effervescent tablets (Novartis). If required: Calcium Gluconate Injection (Phebra) (calcium 0.22 mmol/mL). Calcium Chloride Injection (Phebra) 10% (calcium 0.68 mmol/mL). Maximum Dose Oral – 5.5 mmol/kg Presentation Calcium carbonate, calcium lactate gluconate (CalSource Ca1000) effervescent tablets contain calcium carbonate 1.8 g, calcium lactate gluconate 2.3 g (equivalent to 1 g or 25 mmol of elemental calcium) and sodium 136.9 mg (5.95 mmol). If required: Calcium gluconate 10% 10 mL vial contains 0.22 mmol/mL of elemental calcium. Calcium chloride 10% 10 mL vial contains 0.68 mmol/mL of elemental calcium. Dosage/Interval Dose can vary. Estimate the calcium intake from all sources before prescribing oral calcium. Recommended total daily intake of elemental calcium from all sources: 120–200 mg/kg/day (3–5 mmol/kg/day).
