GUIDE TO USING THE CHI HEALTH LAB DRAW CENTERS AND THE DIAGNOSTIC CENTERS FOR SPECIMEN COLLECTION.

CHI Health Laboratory collection sites are located throughout the metro area and are staffed with professional phlebotomists to provide the best specimen collection experience possible.

There are two Primary Draw Centers (CUMC-Bergan Mercy and Lakeside) and two Secondary collection sites at the Hospital Diagnostic Centers. (Mercy and Immanuel).

Patients must bring the following information with them when visiting a collection site:

 A laboratory requisition provided to the patient by the clinic requesting the laboratory tests  A current insurance card (Medicare, Private insurance, HMO/PPO...)  A photo ID (for example, a driver's license or state issued identification card). Minors must be accompanied by their parent or legal guardian

Patients that present to one of the Primary Draw Centers will be registered as a reference lab patient.

Patients that present to any of the Diagnostic Centers with one of the reference laboratory requisitions listed below, will be registered as a reference lab patient. This will ensure the patient gets the appropriate pricing and also that the laboratory results will be a part of the patient's clinic chart.

The Diagnostic Centers will register all patients presenting without a reference laboratory requisition as hospital outpatients.

The following are acceptable requisitions to present to either the Draw Centers or the Diagnostic Centers.  EPIC requisition  EPIC Future Order requisition  Clinic requisitions that are interfaced with the lab computer system (Cerner)  Green or Pink requisitions

Other specimens (stools, urines...) can also be dropped off at any of these locations. Please have the appropriate paperwork as described above.

Appointments are not required. Walk-ins are welcome. The locations are busiest from opening until 10:00 am. If fasting is not required for your test, you may wish to come during off-peak hours.

PRIMARY PATIENT SERVICE CENTERS Send patient with Laboratory Requisition, copy of demographics and insurance cards. Patients without lab requisitions (Scripts, Billing forms) will have to fill out demographic and insurance forms and be patient billed. Please include diagnosis codes or testing may be held until one is given. Medicare patients should have a signed Advance Beneficiary Notice (ABN) when appropriate (refer to our billing page for more information).

SECONDARY PATIENT SERVICE CENTERS You may visit one of the Hospital Diagnostic Centers to have your lab work drawn, or to drop off a specimen. If you do not have an appropriate CHI Health Laboratory requisition, you will be registered as a Hospital Outpatient and billed accordingly.

Diagnostic Center at Hours Located 2nd floor in Hospital. From CHI Health Immanuel Medical Monday - Friday medical building take hall way toward Center 6:30 a.m. – 6:00 p.m. hospital.
