We look forward to seeing you there. “Foreign DonorInfluence in Health System Restructuring in Central Asia” Dr. EricaJohnson, Global Studies Abstract Under what conditions do authoritarianregimes comply with foreign aid conditionality? The role ofinternational organizations in health sector reform and health servicedelivery grew fourfold over the last 20 years, but these donors have noagreed upon set of health systems goals and often promote competingreform programs.Indeed, even within one donor organization (e.g., WHO,World Bank, etc.) multiple members have varied perspectives on whathealth systems should accomplish. When the Soviet Union collapsed in1991, the governments of the post-Soviet Central Asian states inheritedidentical and seriously flawed health care systems supported with lowlevels of state health care spending. After independence, theseauthoritarian governments adopted significantly different strategiesfor health care spending and content of reform policies, but they haveall welcomed the participation of international donors in thesedecisions. My research explores the tensions that arise betweendomestic actors and international donors to examine why and whenpost-Soviet Central Asian authoritarian regimes adopted the healthsystem reforms that international donors promoted. I find that, even instruggling economies, the goals of international donors potentiallyclash with national-level authority, as domestic politicians mustrespond to the population’s demands for the maintenance of healthsystems as legacy of the Soviet social contract and as a way todemonstrate regime legitimacy. Despite their heavy involvement inhealth system reforms throughout Central Asia, international donorshave been slow to recognize that inefficiencies in the health caresystem are an aspect of political legitimacy and regime survival. Thepaper demonstrates the important role that coordination among state,society, and international donors plays in restructuring and distributing health information and services in resource constrainedenvironments.
