Social Care & Housing Team Profiles Social Care & Housing Team Profiles Version 6 – March 2016 Foreword This Team Profile for Social Care and Housing Services is a brief guide to the roles and responsibilities of each of the teams in the service area, which I hope you will find helpful. The Social Care and Housing service is a complex business, governed within a national statutory framework, to meet local people’s needs. Many of our services are delivered by other organisations and this guide does not attempt to describe all of those providers. We are facing a period of significant change, with the Social Services and Well-Being Wales Act Regulation and Inspection Act and Housing Acts, giving us new responsibilities within a context of rising demand and reduced resources. Our focus is always to safeguard and support people in the most effective and efficient way and this means new models of service delivery and changes to current practice are required. Some of our teams are co-located or integrated with neighbouring public sector agencies and this is the future direction for us. This will ensure we use our resources wisely to meet future needs and it does not mean we can delegate our statutory responsibilities to other organisations. All of our collaboration plans will be based upon evidence of delivering benefits for Torfaen residents, no matter where a particular team or service is located. The team profiles have been published on Sharepoint and will be regularly updated. We have a well developed Members Enquiries system and all enquires should be addressed to Catherine Franklin on 01495 761600 or email [email protected] who will ensure a comprehensive response is provided by the right team member. Best wishes Sue Evans Chief Officer, Social Care & Housing Social Care and Housing Services Leadership Team Left to right: Keith Rutherford, Sue Evans, Gill Pratlett, Neil Howell Sue Evans Chief Officer Keith Rutherford Deputy Chief Officer/ Head of Children and Family Services Gill Pratlett Neil Howell Head of Adult Services Head of Housing & Business Support Senior Management Gill Pratlett Head of Adult Services Joanne Sarah Paxton Lisa Jo Webber Newman Group Beacham Manager Group Manager Group CRT Manager Manager Torfaen Community Resource Team Older Mental GWICES Persons Health Gwent Service Team Wide Integrated Rapid Community Response Equipment Nursing Personal Learning Service Care Team Disability Team Falls Management SII Disabled Service People Integration Day Reablement Service And Activities Improvement Team Hospital Team Adult Safeguarding Duty Pod Senior Management Keith Rutherford Deputy Chief Officer/ Head of Children and Family Services Sue Goddard Anne Sheehan Bernadette Group Group Anderton Manager Manager Group Manager Youth Family Children & Offending Placement Children Family Service & Family Services Services South 1 Collaboration North 1 with Monmouthshire South East Wales Children & Adoption Children Family Service & Family Looked After Services Services Children Team South 2 Collaboration North 2 with Independent Monmouthshire, Performance & Reviewing Newport, Development Officer Torfaen, Safeguarding Caerphilly and Unit Blaenau Gwent Disabled Children’s Team TYPSS Torfaen Young People Support Family Resource Service Team and Sixteen Plus Team Family Focus Team Senior Management Neil Howell Head of Housing and Business Support Direct line management Critical business relationship Elke Winton Eunice Jones Tracy Harris Kay Group Manager Group Manager Human Morris Resources Social Care & Lead Housing Finance Housing Officer Solutions Team Commissioning Unit (Resources Directorate) (Resources Directorate) Private Sector Housing Team Brokerage Homeseeker Business Support Financial Inclusion Performance Team Housing Renewals Housing Strategy & Enabling Performance & Policy Development Supporting People Assistive Technology Team Lead Assessor Adrienne Street The Assistive Technology Team (Lifeline and Telecare) provides a remote monitoring service to all vulnerable residents in Torfaen. The Telecare service objective is to empower service user’s independence, health, safety and wellbeing in the community. Referrals are passed to the Assistive Technology Team, to complete an individual needs based assessment. This is necessary because there is a wide range of equipment that can be used to support risk management and safety at home. The team work in partnership with other agencies including the falls clinic, carers support groups and have recently begun to support a group for carers of people with dementia. The majority of our service users are older persons but we provide specialist support to a diverse client group. Brokerage monitor and report provider activity and availability throughout the borough. One of the key service improvements has been the use of a central point capturing relevant information, for reports, commissioning of services, budget monitoring and contract review. This has allowed the authority to have accurate and up-to- date market information; the team has a key role in monitoring the block contract hours and informing commissioning strategies. Brokerage has provided considerable savings in care management time as they no longer have to spend time arranging care packages and respite, so can concentrate on other aspects of their role. Brokerage is a central point of contact for finance and care managers to provide information on a service users care package. Brokerage continues to book block respite and monitor the utilisation of these respite beds and also manage the short stay unit diary. Brokerage also ensures DRAIG is up to date with services arising out of carers assessments and ensure services users attending day activities or receiving supported living are entered onto DRAIG. As another recording season draws to a close for many species groups, it is in- teresting to look back at the summer’s finds throughout south east Wales. Whether it is a Welsh first (Geranium Bronze in Creigiau, pg 8), a phenomenon rarely seen in our temperate climate (Bioluminescent plankton in Wales, pg 9) or just taking the time to study something often overlooked (Slime Moulds, pg 18), the joys of discovery through wildlife recording is evident throughout this issue. It is also great to learn of the efforts being made to preserve and enhance biodi- versity by organisations such as Porthkerry Wildlife Group (Porthkerry Country Park Wildflower Meadow Creation, pg 4) and Buglife (Wales Threatened Bee Re- port, pg 22). Many thanks to all who contributed to the newsletter. I hope this newsletter will be an inspiration to all our recorders as the nights draw in and your backlog of summer recording beckons!
