MP ’s - Volunteer Award Nomination Form

Recognize a deserving volunteer in your community by nominating them for an MP John Brassard Volunteer Award! A congratulatory ceremony and luncheon will be held for recipients and their guests on April 26, 2017 during volunteer week.

All nomination forms must be submitted by 5:00pm on April 14th, 2017. We thank you for taking the time to complete the nomination form. The nominator will receive written confirmation of the outcome of the nomination.

Nomination Categories

Youth Recognition Award The Youth Recognition Award is presented to an individual less than 18 years of age who has gone above and beyond in making a positive difference in the lives of others through volunteering. This could be through school programs, extracurricular activities, youth programs, etc.

Eligibility Requirements  Nominees must reside in Barrie-Innisfil.  Nominees must be under 18 years of age.  The volunteer work must be aside from the 40 hours required to graduate high school.  Nominees cannot have received payment for their volunteer work.

Individual Recognition Award The Individual Recognition Award is given to an adult 18 years or older who has demonstrated commitment, dedication and service above self to improving their community by volunteering for non for profit organizations, service clubs, community programs etc.

Eligibility Requirements  Nominees must reside in Barrie-Innisfil.  Nominees must be 18 years of age or older.  Nominees cannot have received payment for their volunteer work or performed the services as part of their regular business or professional duties.

Senior Recognition Award The Senior Recognition Award is given to an adult 60 years or older who has positivity impacted the wellbeing of others in their community by the way of volunteering. This individual performs selfless responsibilities and duties by the way of non for profit organizations, service clubs, church groups etc.

Eligibility Requirements  Nominees must reside in Barrie-Innisfil.  Nominees must be 60 years of age or older.  Nominees cannot have received payment for their volunteer work or performed the services as part of their regular business or professional duties.

Business or Service Club Recognition Award The Business or Service Club Recognition Award is presented to a business that has contributed to the positive development and enhancement of the community. Their involvement could be with local charities, events, community programs etc. by the way of volunteering or activity participating.

Eligibility Requirements  Nominees must operate in Barrie-Innisfil.  Nominees must have a valid business license (if applicable) and be in good community standing.  Nominees cannot have received payment for their volunteer work or performed the services as part of their regular business or professional duties.  Nominations must not be exclusive of financial sponsorship.
