Patient information leaflet Introduction This leaflet provides you with information that you may find useful during your time with our service.

We want to offer you as much help and support as you feel you need, to enable you to live the life you want. If you are not sure about anything in this leaflet please ask a member of staff.

What is the Community Treatment Team? The Community Treatment Team provides an assessment and treatment service for people who are experiencing difficulty with their mental health.

The team is made up of a number of health care professionals including Consultant Psychiatrists, Registered Nurses Mental Health, Occupational Therapists, Clinical Support Assistants, Peer Support Workers and Junior Doctors.

Students may also be involved in your assessment and treatment. They will speak to you and ask for your consent, but if you do not wish for them to be involved please let them know.

Teams will be linked to GP practices. Your care team will let you know which team you will receive your care from.

If this is inconvenient to you then appointments may also be arranged within your own home.

Who is it for? The Community Treatment Team provides a service for men and women over the age of 18 years who require a period of assessment and treatment.

Why do I need to attend? Involvement with us is of your choice, you can self refer or may be referred to the service by your GP, another health professional, social services or a family member. We always need your consent before we proceed with a referral from someone other than yourself. Often when people are experiencing difficulty with their mental health they will need to be seen by a specialist service to assess their current mental health needs and see how they can be helped.

What will happen at my first appointment? You will initially see a nurse and have a chance to talk about your current problems.

Following this the nurse will have a discussion with a doctor; whilst this is happening you will receive some basic physical health checks such as your blood pressure, pulse and temperature. We can also check your weight and BMI and take blood samples if they are indicated.

Both the nurse and the doctor will meet with you to discuss and agree options available to meet your current needs, either with our team or with another service.

2 Anything you say will be confidential within the team unless there is a legal duty to share information with other professionals, your family or carer. This will be explained before the assessment. If you have any concerns about confidentiality please talk to a member of staff.

After your assessment, a letter will be sent to your family doctor and the referrer if these are different. You will be asked if you would like to receive a copy of the letter.

You will be given a contact number of the Community Treatment Team so that you will be able to telephone if you cannot attend your next appointment.

Staff can provide you with health information leaflets which may include information about your diagnosis, treatment, self help and support agencies.

What will happen next? If you choose to see us again, you will meet with one of our team and together you will agree a treatment plan which is based on your individual needs. Treatment offered in our team is evidence based, which means it has been researched and proven to be effective.

How long are appointments? The first appointment can last up to two hours. You are welcome to bring a carer, friend or advocate with you.

Personal safety Trust staff carry devices that can monitor and record abusive behaviour. They can alert and receive support from emergency services and the recordings may be used in court proceedings. Staff will treat all patients and their families with dignity and respect and expect the same in return.

Interpreters If you would like an interpreter, this can be arranged prior to your appointment.

You will be able to contact your team through the new single point of access, the Initial Response Service (IRS), which will be able to help with any queries you have as well as putting you directly in touch with your team.
