This is also a great opportunity to review a patient’s comprehensive review of systems, social and family histories to identify relevant risk factors. Not only does this strengthen the doctor-patient relationship and provide a medical home for the patient. It can also potentially lessen patient worry about their overall health. • In your 60s and beyond – The patient’s risks for heart attack and stroke are elevated. To help prevent them, it’s importance to continue with comprehensive health maintenance screening as well as maintaining a healthy lifestyle. In addition to the above-mentioned screening in the younger decades, it’s also important to discuss any concerns about cognitive decline, risk of falls and polypharmacy (simultaneous use of multiple medications to treat a single condition). The focus not only centers on preventive care but also proactive medical management.
