Mid day meal consumption in Govt Food Under middle School Simariya was the likely cause of JH/KOE/2018/20/659 Jharkhand Koderma 26 00 19-05-18 19-05-18 outbreak. District RRT investigated the Poisoning Surveillance outbreak. House to house survey done. Health camp organized in the school and all cases treated symptomatically. Health education given. Cases of loose stools and vomiting reported from Village Sirmarg Manzmoh Block Qazigund. District RRT investigated the outbreak. 02 water samples (01 tap water sample and 01 reservoir water sample) and 02 Jammu And Acute Under stool samples sent for analysis to IDSP JK/KLM/2018/20/660 Kulgam 22 00 14-05-18 16-05-18 Kashmir Gastroenteritis Surveillance Provincial Lab RFPTC Barzulla Srinagar; both water samples were unsatisfactory. Culture of stool samples shown normal flora. All cases treated symptomatically. Health education given on personal hygiene and hand washing practices. Cases of fever with rash reported from Village Abhama, NTPHC Abhama Block Rajpora. District RRT investigated the outbreak. House to house survey done. 05 Blood Samples Jammu And Under collected and sent to PHL Barzulla for analysis; JK/PWM/2018/20/661 Pulwama Measles 09 00 14-05-18 16-05-18 Kashmir Surveillance 03 samples were positive for Measles IgM Elisa. All cases treated symptomatically. Isolation of cases advised. Health Education imparted on spread of disease and its preventive measures to the community. 5 | Page Cases of fever with rash reported from Govt. Middle School Zangli Kupwara, Block Kupwara. District RRT investigated the Jammu And Under outbreak. House to house survey done. Health JK/KWR/2018/20/662 Kupwara Chickenpox 17 00 21-05-18 21-05-18 education given to population about the disease Kashmir Control and its preventive measures. School authorities were advised to isolate cases and monitor hygiene of children. All cases treated symptomatically. Health education given. Cases of loose stools with abdominal pain reported from Village Chowki Thanda and Royila thanda, PHC/CHC Basavana Sangolgi, Taluk Aland. District RRT investigated the Acute outbreak. House to house survey done. Stool Under sample collected were negative for V. Cholera KN/GUL/2018/20/663 Karnataka Kalaburagi Diarrheal 39 00 16-05-18 17-05-18 Control culture. 14 Water samples were non potable. Diseases All cases treated symptomatically. Chlorine tablets distributed in community. Community educated on drinking boiled and cooled water. Super-chlorination of drinking water sources done. 6 | Page Cluster of 3 cases of Viral encephalitis reported from Block Perambra, PHC- Chengaroth. The unusual event started with death of a 26 yr male, clinically diagnosed as viral encephalitis at Medical College Hospital. Two weeks later 2 members of the same family reported with similar symptoms, rapidly progressed and died while on treatment, at a private tertiary care hospital. On subsequent days 6 more persons succumbed to similar symptoms, including a staff nurse of the taluk hospital, involved in treatment of the second case. All the affected cases were found to have close contact with a Nipah Viral Under confirmed case/death. District RRT team along KL/KOZ/2018/20/664 Kerela Kozikohode 09 07 04-05-18 05-05-18 with the central team of NCDC investigated the Encephalitis Surveillance outbreak. House to house active case search and contact tracing was done. Serum and CSF samples were sent to MCVR Manipal & NIV Pune. Nipah Virus Encephalitis PCR test was confirmed positive for initial three cases and therafter many more. Isolation points identified and infection control protocol strengthened. Adequate quantity of PPE and other logistics ensured in the district. Burial protocols issued. Home quarantine for the contacts and hospitalized observation for all symptomatic implemented. Medical camps conducted in the area for disease awareness and reducing panic. Cases of fever reported from Village Washi, SC Pargaon, PHC Shendi, Block Tandulwadi. District RRT investigated the outbreak. House Under to house survey done. Blood & serum sample MH/OSM/2018/20/665 Maharashtra Osmanabad Dengue 11 00 11-05-18 15-05-18 sent to Sentinel center, Beed for IgM Elisa; 04 Surveillance samples positive for Dengue. Source reduction and anti-larval surveys done. All cases treated symptomatically. Health education given on personal protection measures. 7 | Page Cases of vomiting with loose stools reported from Village Kherdi, Panwadi (Pisai PHC). District RRT investigated the outbreak. House Food Under to house survey done. Symptomatic Treatment MH/RTG/2018/20/666 Maharashtra Ratnagiri 20 00 18-05-18 19-05-18 Poisoning Surveillance of cases at SDH Dapoli. Health camp organized in the school and all cases treated symptomatically. Relevant health education given. Cases of fever with rash reported from Village Dhorera (SC Magarda) PHC Agra, CHC Vijaypur, Block Vijaypur. District RRT Madhya Suspected Under investigated the outbreak. House to house MP/SHY/2018/20/667 Sheopur-Kalan 09 00 15-05-18 16-05-18 Pradesh Measles Surveillance survey done. All cases treated symptomatically. Vaccination status of cases was not known. Vitamin A administration done and isolation advised to cases. Cases reported from Village Pakhao, CHC Sagolmang, Block Sawongbung. District RRT investigated the outbreak. House to housesurvey done. State RRT investigated the Under outbreak. 14 people were bitten by same dog. MN/MNP/2018/20/668 Manipur East Imphal Dog Bite 14 01 19-05-18 21-05-18 The dog has been killed and no samples could Control be collected. The rabies case eventually died on the same day. All the 14 cases are undergoing 5-dose regime of PEP. No other report of suspected rabies case in the area. Immunization of eligible dogs done by Veterinary Deptt. Cases of vomiting with loose stools reported from Village Taliudar (Kuliapada), SC Taliudar, PHC Kushang, Block Loisingha. District RRT investigated the outbreak. House to house survey done. Outbreak was possibly Food Under due to consumption of mela prasad. Food OR/BAG/2018/20/669 Odisha Bolangir 81 00 13-05-18 15-05-18 Poisoning Surveillance (PRASAD) samples taken by Food Safety Officer, Balangir. 02 Water Samples sent to DPHL, Balangir for investigation, MPN test; all were negative. Health camp organized in the school and all cases treated symptomatically.     DISTRICT PROFILE BACKGROUND OF THE DISTRICT Vaishali find its origin way back in history from the Mahabharata period. It is the place where democracy found its origin around 600BC. Numerous references to Vaishali are found in texts pertaining to both Jainism and Buddhism, which have preserved much information on Vaishali and the other Maha Janapadas. Based on the information found in these texts, Vaishali was established as a republic by the 6th century BC, prior to the birth of Gautama Buddha in 563, making it the world's first republic.Being the birthplace of the last Jain “Thirthankara” Lord Mahavir gives Vaishali a very special place in history. The founder of Buddhism, Gautam Buddha delivered his last sermon and announced his Parinirvana (attainment of enlightment) on this holy soil. It is also renowned as the land of Ambapali (Amrapali), the great Indian courtesan. It is held that the district derives its name from King Vishal. However, history records that around the time Pataliputra was the centre of political, cultural and economic activities in the Gangetic plains, Vaishali came into existence as centre of the Ganga, it was the seat of the Republic of Vajji. Vaishali is credited with being the World's First Republic to have a duly elected assembly of representatives and efficient administration. Vaishali is also renowned as the land of Ambapali (also spelled as Amrapali), the great Indian courtesan, who appears in many folktales, as well as in Buddhist literature. Quarter 2012 (April 2012) Background The report is prepared based on the findings of Quality Assurance Toolkit (QAT) filled in by the SHSB officials, SIHFW officials regarded as State QA Monitors; Medical College Professors, Regional Deputy Director, RPM, RM&E, RAM, who are regarded as Regional QA Monitors and the Medical Officers and Grade-A/ANMs being trained in NBCC regarded as District QA Monitors. This External Quality Monitoring of NBCC was conducted by SHSB supported by UNICEF STF team (State Task Force- QA FBNC). Major observations were noted related to infrastructure, equipment maintenance, manpower, supply of drugs and consumables, adherence to protocols and record keeping. Data related to management of newborn in NBCC were taken from the PHC Monthly NBCC Report submitted to DHS and SHSB. Objective of External Monitoring: To identify gaps/weaknesses in the quality of care and provide onsite corrective action. To assess the units for clinical care against common norms and standards To assess the knowledge & skill level of NBCC staffs To understand the Recording, Reporting & Feedback mechanisms in the units Categorization of status of NBCC- 3 categories with color coding 1. Equipment : All equipment are functional 2. Human Resources :All staff are trained & skilled (at least 4 ANM /Grade-A) Fully Functional 3. ATMA has been created in each of the districts to implement the new scheme. The institution of ATMA has facilitated bringing together researchers, extension workers, other stakeholders and farmer who are the principal stakeholder to carry out joint diagnostic action and prepare and carry out plan in terms of whether it is extension or the demonstration or trials for the research purposes. The preparation of District action plan in tune with local needs and situations. To perform this responsibility, ATMA has developed a demand driven, situation specific multi agency oriented Strategic Research & Extension Plan (SREP). This is the basic document that spells out the goals to be pursued, programme service to be accomplished, mode of implementation and effective utilization of all available resources. The SREP has been prepared in consultation with the farmers taking into consideration, their needs and aspirations through Participatory Rural Appraisal methodology (PRA). The programme proposed to be undertaken in the SREP is demand driven, agro-eco situation specific and multi disciplinary in nature. I extend my sincere thanks to the Project Director, ATMA and district heads of all departments in coordinating their efforts in bringing out this valuable document.
