The opinions expressed herein are those of the authors and do not necessarily reflect the views of USAID. 1 Summary The EngenderHealth ACQUIRE Project has been supporting the Tanzanian Ministry of Health (MOH) since early 2005 to decentralise the management of Post Abortion Care (PAC) services to Primary Health Care facilities (health centres and dispensaries), with the intention of bringing services closer to women who are unable to access them at district hospitals. The decentralised service has been pilot tested in one district, Geita District in Mwanza Province, northern Tanzania, to provide the Ministry with experience and operational information before it expands decentralization to the rest of the country. The pilot project was introduced by ACQUIRE in eleven health facilities (seven health centres and four dispensaries) in May 2005. In November 2005, the Population Council FRONTIERS Program began documenting the process of decentralising PAC and prospectively assessing its feasibility, effectiveness and cost. FRONTIERS collected data in selected pilot sites at two points in time; November 2005, the findings from which were provided to the MOH and ACQUIRE to address issues arising from introduction of the intervention; and in September 2006, to assess the feasibility, cost and effectiveness of the intervention. The specific objectives of this evaluation were to: Document the process of introducing and implementing the package in health centres; Assess changes in provider knowledge, attitudes and practices in the management of miscarriage and abortion complications that can be attributed to the training; Describe experiences of postabortion clients and their perceptions of the new service; Document changes in community attitudes and knowledge about abortion complications following introduction of the services, and the acceptability of local facilities offering PAC services; Examine the kinds of referral networks put in place by the intervention, and document their effectiveness and, Establish the additional resources that would be required by the MOH to support introduction of PAC services. Data were collected from a sample of one hospital and five health centres during the first round of data collection; no dispensaries were then offering PAC. During the second round, two dispensaries were included and a sixth health centre. A combination of data collection methods was used: in-depth interviews with service providers and with PAC clients; facility audits; and focus-group discussions with members of the communities served. Service records were reviewed to determine PAC and other RH service utilization trends. Financial records were reviewed to establish government expenditures on medical supplies, program support costs and staff emoluments. Overall, the results indicate that PAC services are feasible and easy to set up in the lower health facilities, and are considered to have offered a satisfactory level of quality of care for clients. The intervention is seen to have broadened service providers‟ range of clinical skills and led to an increase in the number of women accessing family planning services after the evacuation. Facilities are well prepared to provide the PAC service, and few clients are being referred for advanced clinical management for incomplete abortions within 12 weeks of conception. i However, linking MVA clients with other RH services besides FP remains weak in most of the sites; few facilities have managed to integrate STI/HIV screening and services into the PAC service. The costs of introducing and sustaining the service were calculated and projections provided to inform the MOH and its development partners of the anticipated costs of extending PAC to all district hospitals and health centres in Tanzania. In conclusion, this project has demonstrated that decentralizing PAC services to health centres and dispensaries is feasible and effective, and the approach could be scaled up at a reasonable cost to other lower level facilities in Tanzania. ii Acknowledgements The authors would like to acknowledge the support received from different individuals and institutions during designing and implementing the study as well finalizing this report. These operations dramatically change the local watershed topography and Stream flow expose downslope agricultural fields and forest to tailings runoff. Given that most mine tailings expose DEM large quantities of surface area to oxidation and transport by water, any heavy metals associated with the Tanzania Geita Gold Mine deposit are mobilized to move along with the runoff. In Tanzania, the Geita Gold Mine (GGM) area is such a site and the Government of Tanzania has yet to develop a water monitoring network to protect villages adjacent to the mines. As a result, mining company data are the only data available to monitor water supply and quality. Typically in mining and oil sand extraction, geospatial data are used to report and monitor land reclamation at the mining site, and while these efforts are useful, they do not consider hydrologic changes and risks. In this paper we evaluate the use of Digital Elevation Model (DEM) data from the Space shuttle Radar Topography Mission (SRTM) and the Advanced Spaceborne Thermal Emission and Reflection Radiometer (ASTER) in an effort to identify the changes in local topography and surface hydrology around the GGM and assess the implications these changes have for the potential increased mobility of tailings and their effects upon farmers, village water supplies, and community forests using a hydrologic flow model.

    ARD MANAGEMENT AT GEITA GOLD MINE1 Ulrich Sibilski,2 and Rebecca Stephen3 Abstract: The Geita Gold Mine (GGM) is located approximately 4km west of Geita in the Mwanza Region of northern Tanzania. The mine is situated at the headwaters of the Mtakuja River that drains into Lake Victoria approximately 20km north west of plant site. GGM is owned and managed by AngloGold Ashanti Limited which is one of the major gold producer companies in the world. GGM alone produces up to 610,000 oz of gold per year moving towards 880,000 oz in 2007. GGM has sound environmental practices and Acid Rock Drainage (ARD) is one of the main issues of concern. This paper describes the management of ARD at GGM which is mostly done through proper identification and handling of potentially acid forming waste material from all operating five pits. The procedure for ARD management involves: producing a waste model for the life of mine for each pit; validating of waste models using in-pit geological mapping; selective handling and placement of waste in designated areas of the waste rock dump and the tailings dam embankments; validating placement of waste within the waste rock dump and tailings dam embankment; monitoring the placement using piezometers within the waste rock dump, tailings embankment and downstream of these facilities; regular technical reviews and implementing the mentioned procedures for the life of mine.     "THE GOLD AND THE SPIRITS ARE CLOSE� Popular Religion and Social Transformation in Sukumaland, North-west Tanzania.1 Frans Wijsen Since the introduction of the free market economy and the multi party system in Tanzania economic and political changes occur day after day. These changes are reflected and transcended in changes in the cultural and the religious field, as the growing tension between Christians and Muslims (Hagamu, 1991) and the growing gap between official and popular religion (Magesa, 1994) show. In this paper it is assumed that the generative capacity of symbolic thought is one of the primary factors in cultural change. Popular religion is an appropria- te theme to discover this change. A semiotic analysis can be used to discover the transformations in society as a whole or aspects of it by way of an analysis of the transformations in the cultural symbolism. Popular religion is understood here not only as the religion of the ordinary faithful in contrast with that of the religious leaders within a certain tradition, but also as the religion of the masses in contrast with that of the elites as different groups of people within the Sukuma social structure. The aim of this paper is to explore if popular religion can be understood as a source of transformation (rather than as a hindrance as is often thought), and to indicate some consequences for the planning of a missionary policy and strategy, starting from popular religion. First we will investigate some new rituals addressing evil spirits in the gold mines in Geita District, Northwest Tanzania.
