Rick McDonough, MD President, Florida Society of Clinical Oncology Florida Medical Clinic

BIOGRAPHY: Dr. Rick McDonough is from Clearwater and has lived most of his life in Florida. He is a proud graduate of the University of Florida. He enjoys traveling, reading and spending time with his wife and three children. Dr. McDonough is a long standing active member of the Florida Society of Clinical Oncology and has proudly served on the board of directors for the past five years. Dr. McDonough practices at Florida Medical Clinic, a large multi specialty group in the greater Tampa area. Dr. McDonough is active in medical mission work, traveling every year with a group to Guatemala, providing primary care to a rural area in the mountains northwest of Guatemala City since 2003.

BIOGRAPHY: Luis E. Raez, MD, FACP, FCCP; is the Medical Director of Memorial Cancer Institute (MCI), he is also the Oncology Research Director of Memorial Healthcare System (MHS) and Director of the Thoracic Oncology Program, as well as the Clinical Professor of Medicine at Florida International University (FIU) and Visiting Professor of Medicine at Cayetano Heredia University in Peru. Currently Dr. Raez designs phase I-III clinical trials with new chemotherapeutic agents and combinations. Dr. Raez does translational research in the areas of cancer vaccines and has been funded by NCI and the pharma industry. Dr. Raez has given oral presentations and lectures in national and international meetings in the US, Europe, Latinamerica and Asia. He is American Board Certified in: Internal Medicine, Medical Oncology and Geriatric Medicine. He is board elegible in Hematology. Dr. Raez is a member of AACR, ESMO, ASCO, IASLC, ALLIANCE, NCCTG, ACCP, ACP, ACSG, FLASCO among other institutions.

Monica M. Bertagnolli, MD 2018 – 2019 ASCO President Chief, Division of Surgical Oncology and Associate Surgeon Brigham and Women’s Hospital, Dana-Farber Cancer Institute Richard E. Wilson Professor of Surgery in the Field of Surgical Oncology, Harvard Medical School

BIOGRAPHY: Monica M. Bertagnolli, MD, a long-time member and volunteer of ASCO, took office as President during the ASCO Annual Meeting in Chicago in June 2018. “Serving as ASCO President is a tremendous personal honor for anyone in the field of oncology,” said Dr. Bertagnolli. “Much more importantly, it is an opportunity for me to make a meaningful difference by providing a strong voice in the health-care community for those whom I consider to be my particular constituency: clinical and translational researchers, community and academic oncologists participating in clinical research, and surgical oncologists.” Dr. Bertagnolli has been an active ASCO member since 1995, Dr. Bertagnolli is Chief of the Division of Surgical Oncology at Dana-Farber/Brigham and Women’s Cancer Center, Professor of Surgery at Harvard Medical School, and an Associate Surgeon at Brigham and Women’s Hospital and Dana-Farber Cancer Institute. Dr. Bertagnolli has served on the ASCO Board of Directors; the Cancer Prevention Committee; and the Strategic Planning Committee, among others. Dr. Bertagnolli received her board certification in surgery in 1993. She went to the University of Utah School of Medicine and completed her residency in Brigham and Women’s Hospital in surgery. Dr. Bertagnolli has clinical interests in familial adenomatous polyposis, gastrointestinal cancers, gastrointestinal stromal tumors, and sarcoma.

Best of ASCO- FL Hospital September 14-16, 2018 in Orlando, FL Leslie Maxwell, MBA- Florida Hospital BIOGRAPHY: Leslie Maxwell has over 30 years of experience in the health care field, with 18 of those being in oncology. She served as the 2001-2017 Administrator of Florida Hospital Cancer Institute Central Region and then joined Florida Hospital Waterman as the Cancer Institute Director in September 2017. Leslie is responsible for program operations and development, strategic planning, marketing, physician relations, financials.

2018 ASTRO Annual Meeting October 21-24, 2018 in San Antonio, TX David Asher, MD- University of Miami BIOGRAPHY: David Asher is a PGY-4 and current, co-chief resident in the Department of Radiation Oncology at the University of Miami and Jackson Health System. Raised in Elon, North Carolina, he graduated from Duke University with a B.A. in Religion. He then completed his medical degree at Virginia Commonwealth University where he was inducted into the Alpha Omega Alpha (A.O.A.) Honor Society. At the conclusion of transitional year internship at Riverside Regional Medical Center in Newport News, Virginia, he was voted the top intern by his upper level residents. In June 2016, he began his training in Radiation Oncology in Miami, Florida. He has a passion for serving his community and considers it a privilege to be an oncologist. He has interests in a variety of areas including MR-guided radiotherapy, brachytherapy, health disparities, and quality improvement. He currently resides in Miami Beach with his wife, Adelina, and his two kids, Henry (age 3), and Norah (age 1). Pharmacy Student Travel Award Yen Tran, MBA- Nova Southeastern University BIOGRAPHY: Yen Tran is PharmD candidate 2019 from Nova Southeastern University - College of Pharmacy. He received his Bachelor of Pharmacy degree from the University of Medicine and Pharmacy of Ho Chi Minh city in Vietnam and Master of Business Administration from Solvay School of Business- Belgium. He has 10-year working experience in pharmaceutical industry and commercial management for multinational corporations such as Merck, AstraZeneca, Abbott and Les Laboratories Servier, across therapeutic areas including cardiovascular, diabetes, oncology and respiratory. He was awarded Best Product Marketing Manager by Merck Vietnam in 2015. He is a member of Rho Chi Honor Society, Phi Lamda Sigma - Pharmacy Leadership Society, Dean’s Ambassador and Kappa Psi Pharmaceutical Fraternity, Inc. Upon graduation from program, Yen Tran aims to pursue a PGY-1 pharmacy practice residency program and PGY-2 in oncology specialty. Alyssa Donadio, PharmD- Baptist Hospital of Miami BIOGRAPHY: Alyssa Donadio, PharmD, is currently a PGY-2 Oncology Pharmacy Resident at Baptist Hospital of Miami, where she also completed her PGY-1 Pharmacy Practice Residency. Before moving to Florida, she obtained her Bachelor’s Degree in Biology from Cornell University in 2013 and her of Pharmacy from Albany College of Pharmacy and Health Sciences in 2017. In addition to FLASCO, Alyssa is also involved in the American Society of Health System (ASHP) as well as the Florida Society of Health System Pharmacists (FSHP). Her areas of interest in oncology include hematologic malignancies and investigational drug services.

ASH Annual Meeting December 1-4, 2018 in San Diego, CA

Bilal Farooqi, MD- University of Florida BIOGRAPHY: Bilal Farooqi, MD is a third year fellow in hematology-oncology at the University of Florida in Gainesville. He completed his residency in internal medicine from Mercer University in Macon, Georgia and was excited to move back to Florida to complete his fellowship here. Dr. Farooqui is looking forward to graduating from fellowship in June 2019. He plans to reside in his hometown of Tampa, FL and looks forward to being more involved in the community as a hematologist-oncologist following the completion fellowship.

Monica Chatwal, MD- Moffitt Cancer Center BIOGRAPHY: Monica Chatwal is a native Floridian, and grew up in Stuart. She received a bachelors degree in biochemistry and molecular biology from the University of Miami. She attended medical school at Florida State University College of Medicine. She then completed her internal medicine residency at Emory University School of Medicine in Atlanta, GA. Currently, she is a third year hematology oncology fellow at Moffitt Cancer Center. She has a particular interest in studying physician resiliency and burnout and incorporating arts and humanities in medicine.

Sharad Khurana, MD- Mayo Clinic BIOGRAPHY: Sharad Khurana, MD, is a current PGY5 in Hematology/Oncology fellowship program at Mayo Clinic in Jacksonville, FL. Alma mater of Grant Medical College in Mumbai, India and graduated in 2013. Dr. Khurana was fortunate to do a masters in clinical research from Rush University while undergoing internal medicine training at John H Stroger Hospital of Cook County in Chicago, Illinois. He has special interest in research in malignant hematology and clinical trials. He hopes to start an academic career after completing fellowship.

Candice Baldeo, MD- Mayo Clinic BIOGRAPHY: Candice Baldeo, MD is the Chief Hematology Oncology Fellow at Mayo Clinic Jacksonville and will graduate in June 2019. She was born and raised in Trinidad and Tobago. After completing medical school in Trinidad, she moved to Jacksonville, Florida for internal medicine residency at the University of Florida, Jacksonville. Dr. Baldeo has a strong interest in Gastrointestinal Oncology and her research has been focused on pharmacogenomics in colorectal cancer.

Schmidt College of Medicine, Florida Atlantic University BIOGRAPHY: Dr. Edgardo Santos practices at the Center for Hematology-Oncology (CHO)/Lynn Cancer Institute where he is the Medical Director of Cancer Research. Dr. Santos is a Clinical Associate Professor at Florida Atlantic University, and the Chairman of the FLASCO Program Planning Committee and Board of Directors Member. He earned his M.D. in 1994 at the University of Panama, School of Medicine, Republic of Panama. Dr. Santos completed his internship and residency training in Internal Medicine at Jackson Memorial Medical Center, University of Miami School of Medicine. He completed his fellowship in Hematology/Oncology at the Sylvester Comprehensive Cancer Center. Dr. Santos has authored or co-authored several manuscripts in peer-reviewed journals and also serves as reviewer for several scientific publications. Dr. Santos is a member of the Editorial Board of Expert Review series, Recent Patents on Biomarkers, and Clinical Practice. Dr. Santos was a former faculty member of University of Miami Miller School of Medicine, Miami, Florida where he held an academic rank as an Associate Professor of Medicine (2008-2012) and Tulane University Health Sciences Center, New Orleans, Louisiana (2004- 2008). Dr. Santos has occupied several administrative, educational, and research leadership positions including Tulane University Principal Investigator at Southwest Oncology Group (SWOG), Associate Scientific Director of Tulane Cancer Center’s Office of Clinical Research, Associate Director of the Fellowship Programs, Chief of the Hematology/Oncology Section at the Southeast Louisiana Veterans Healthcare System, Chair of the Committee Research Advisory Board, Co- Leader of Clinical Research for the Louisiana Cancer Research Consortium, Co-Leader of the Head and Neck Cancer Program at Sylvester Cancer Center, and many others. He is an active member of the International Association for the Study of Lung Cancer where he is the Chair of Publication Committee, member of the Educational Committee for the World Congress on Lung Cancer in 2018 (Toronto, Canada), and the Scientific Chair of 2018 LALCA Congress (Cordoba, Argentina); the American Association for Cancer Research, ASCO, the European Society of Medical Oncology, ASH, and the American College of Physicians-American Society of Internal Medicine. Dr. Santos is also a member of the Addario Lung Cancer Medical lnstitute which is focused in developing personalized medicine in this field. Eric Harris, DO~ FLASCO Program Co-Vice Chairman and Board of Directors Member Florida Cancer Specialists and Research Institute, Daytona Beach, Florida BIOGRAPHY: Eric Harris, DO, is a hematologist/oncologist at Florida Cancer Specialists in Daytona Beach, Florida. Dr Harris has a keen interest in clinical research. He is an active principal investigator on multiple clinical trials with his collaboration with the Sarah Cannon Research Institute. He has been published in Case Reports in Oncology and has presented abstracts and published for several annual scientific meetings, including the Florida chapter of the American College of Physicians and the American Society of Hematology. He is a member ASCO, and serves on the executive committee of FLASCO. Dr Harris received diverse preparation in his early medical training, having been an immunology research technician as well as a supervising medical technologist in a flow cytometry laboratory. He was twice awarded the Gold Key Customer Service Award by UF Health Shands Hospital (2009 and 2014), as well as the Teaching Excellence Award by University of Florida in 2010. Dr Harris earned his DO from Nova Southeastern University College of Osteopathic Medicine in Fort Lauderdale, Florida, and completed a residency in internal medicine and a fellowship in hematology/oncology at University of Florida in Gainesville, Florida where he served as Chief Fellow. He enjoys teaching and is currently part of the clinical faculty at Florida State University and has given multiple community-based and continuing medical educational lectures. Joseph D. Rosenblatt, MD~ FLASCO Program Co-Vice Chairman and Board of Directors Member Chief, Hematology Division, DOM- Miller School of Medicine, University of Miami BIOGRAPHY: Joseph D. Rosenblatt, M.D., is a Professor of Medicine, Microbiology and Immunology, and is Chief of the Hematology Division at the Sylvester Comprehensive Cancer Center (SCCC) at the University of Miami (UM). Dr Rosenblatt joined University of Miami in 2001, bringing extensive administrative and research experience. He holds a graduate cross appointment in the Department of Microbiology and Immunology at UM. Dr. Rosenblatt is also a member of the Sylvester Tumor Biology Program. In the past 20 years, Dr. Rosenblatt has published over 150 journal articles and chapters in the areas of human retroviruses, immune therapies for cancer, and human gene therapy. From 1986 to 1995, Dr. Rosenblatt had been a member of the faculty in Hematology Oncology (Associate Professor) and was elected an Associate Member of the Molecular Biology Institute at the UCLA School of Medicine. Dr. Rosenblatt then served at the University of Rochester from 1995-2001 as the James P. Wilmot Chair in Hematology-Oncology and Chief of the Hematology-Oncology Unit, and had a secondary appointment within the Department of Microbiology and Immunology. He has had continuous NIH and other extramural funding for retroviral, gene therapy and immunotherapy related research since 1986 and has previously been a standing member of the NCI Experimental Therapeutics II (ET-2), the Clinical Immunology and Immunotherapy II (C-II) study sections and more recently as a member of the Clinical Oncology Study section at the NCI. His research includes the synthesis and testing of novel antibody—fusion proteins which target the HER2 and EGFR cancer antigens which can inhibit angiogenesis or augment the immune response to breast and other cancers. He has also developed novel gene therapy approaches for use in lymphoma and chronic lymphocytic leukemia, and is currently exploring the role of B-cells in modulating the immune response to cancer in animal and human cancers. Dr. Rosenblatt completed a Fulbright Scholarship in 1990 at the Rabin Medical Center and is also a founding member of the Bar Ilan Faculty of Medicine in Safed, Israel. He is a nationally recognized expert in the treatment of lymphoma and other hematologic malignancies and maintains an active clinical practice.

He is also a Senior Member of the Division of Population Sciences and Professor at the University of South Florida. Dr McLeod is chair of the NHGRI eMERGE network external scientific panel and a recent member of the FDA committee on Clinical Pharmacology and the NIH Human Genome Advisory Council. Since 2002, Dr McLeod has been vice chair for Pharmacogenomics for the ALLIANCE NCI clinical trials group, overseeing the largest oncology pharmacogenomics portfolio in the world. Dr McLeod is also a 1000 talent scholar of China and a Professor at Central South University in Changsha, China. Howard has published over 550 peer reviewed papers on pharmacogenomics, applied therapeutics, or clinical pharmacology and continues to work to advance individualized medicine.

She was previously Professor of Medicine and Chief of the Division of Hematology-Oncology at the Texas Tech University Health Sciences Center, El Paso (TTUHSC-EP) and prior to that Associate Professor and Co-Leader of the Breast Cancer Multidisciplinary team at the Wayne State University-Barbara Ann Karmanos Cancer Institute. Dr. Nahleh subspecializes in breast cancer and has special interests in cancer survivorship and disparities. Her research interests were funded by the National Cancer Institute (NCI), the Cancer Prevention Institute of Texas and other scientific agencies. Dr. Nahleh is a certified principal investigator (CPI) by the Academy of Clinical Research Professionals. She served as a principal investigator in the SWOG oncology cooperative group and has authored national protocols in breast cancer neoadjuvant therapy and male breast cancer. She served on the NCI Neoadjuvant and Metastatic Breast Committees. Dr. Nahleh has received numerous awards, including the Barbara Ann Karmanos Hero of Breast Cancer Award for Scientific Distinction, the TTUHSC Dean’s Excellence in Research Award as well as the Distinguished Chancellor’s Council Award for Research. She has authored and co-authored numerous articles, book chapters and books in the field of cancer and breast cancer research, and has presented at national and international cancer meetings. She serves on the editorial board of numerous medical journals. She has a double American board certification in hematology and oncology.

BIOGRAPHY: Mohammad Jahanzeb is Professor of Clinical Medicine, Hematology Oncology at University of Miami’s Miller School of Medicine. Dr. Jahanzeb currently serves as the Medical Director of UM Sylvester Deerfield Campus and an Associate Director of Community Outreach for the Sylvester Comprehensive Cancer Center. An international lecturer on issues related to breast and lung cancers, Dr. Jahanzeb has given more than 900 lectures as an invited speaker and has authored or contributed to more than 200 original reports, book chapters, reviews, and abstracts. Dr. Jahanzeb is board certified in medical oncology and limits his practice, as well as clinical research, to cancers of the lung and breast. He received his medical degree with high honors from King Edward Medical College at the University of Punjab in Lahore, Pakistan. Dr. Jahanzeb is a fellow of the American College of Physicians, and a Fellow of American Society of Clinical Oncology, and belongs to several professional societies including the American Society of Clinical Oncology, where he serves as the Chair for Quality Oncology Practice Initiative Steering Task Force and is the founding Chair for their task force for International Quality Efforts (iQuality). He is currently a member of the FLASCO Board of Directors. He has also previously served on the Board of Directors and the Expert Panels for Lung Cancer and Breast Cancer of the National Comprehensive Cancer Network between 2003 and 2013.

She joined Florida Hospital Medical Group in May of 2018, with an appointment as Assistant Professor of Radiation Oncology at the University of Central Florida College of Medicine. Previous to this she served as faculty at Moffitt Cancer Center and assistant professor at the University of South Florida Morsani School of Medicine. Her past honors have included an award for best-performing student at her alma mater, Rose-Hulman and selection for the John H. Edwards Fellowship at the Indiana University School of Medicine. She was also peer nominated as a Tampa Top Doctor in 2017. Valente is a board- cerfied Breast Surgeon and the Director of the Breast Surgery Fellowship Program at the Cleveland Clinic and Assistant Professor of Surgery at the Cleveland Clinic Learner College of Medicine and Case Western Reserve University. She joined the Clinic in 2011 after completing her Surgical Breast Oncology fellowship at the University of Southern California in Los Angeles. Dr. Valente has a strong interest in advanced surgical techniques including minimally invasive breast surgery, hidden scars, oncoplastic surgery and nipple sparing mastectomy. She has a passion for treating young women with breast cancer, as well as those with genetic andhigh-risk disease. She is active in research for development of innovative surgical procedures to prevent lymphedema. She is also a leader in Intraoperative Radiation Therapy (IORT) serving as 1st author of the TARGIT-R trial, as well as Chair of the Research committee for the TARGIT Collaborative Group for IORT. Burger, M.D., Ph.D. is a Professor of Medicine at the MD Anderson Cancer Center in Houston, USA. Dr. Burger received his MD/PhD degree at Albert-Ludwigs-University in Freiburg, Germany, with subsequent clinical training and board certification in Internal Medicine and Hematology/Oncology at Freiburg University Hospital, before moving to MD Anderson in 2005. In 2006, Dr. Burger received the “Venia legendi” () for Internal Medicine at Albert-Ludwigs-University in Freiburg. Moreover, from 1996 to 1999, Dr. Burger received postdoctoral research training in Hematology and Immunology at the University of California, San Diego. As a laboratory and clinical investigator, Dr. Burger focuses on developing new therapeutic approaches for targeting the microenvironment in CLL, other B cell malignancies, and acute leukemia. Laboratory research by Dr. Burger’s group characterized B cell chemokine receptors, such as CXCR4 and CXCR5, adhesion molecules, and B cell receptor (BCR)-associated kinases, such as SYK, BTK, and PI3 kinases as therapeutic targets. Dr. Burger serves as a principal investigator on clinical trials which explore several of these targets in CLL. For his innovative research, Dr. Burger has received several awards, among them, the Arthur Pappenheim Award by the DGHO, a Kimmel Scholar Award, ASCO Young Investigator and Career Development awards, the Leukemia and Lymphoma Society Scholar award, and the MD Anderson Faculty Scholar Award. As a Resident at Jackson his devotion to teaching and continued learning was evident and resulted in him being awarded Resident of the Year as well as inducted into the Alpha Omega Alpha Honors Medical Society. Louise Morrell, MD~ Medical Medical Director, Lynn Cancer Institute, Boca Raton Regional Hospital BIOGRAPHY: Dr. Louise Morrell is the Medical Director of the Lynn Cancer Institute in Boca Raton Florida. Her clinical practice is now dedicated to inherited risk assessment and testing for genetic predisposition cancer syndromes through the Morgan Pressel Center for Cancer Genetics. Genetic testing is currently integrated into the multimodality clinics at the Lynn Cancer Institute. The center is a member of the Clinical Cancer Genomics Community of Practice through the City of Hope and participants in the international genetics registry.

Jingsong Zhang, MD, PhD~ Medical Associated member, Genitourinary Oncology, Moffitt Cancer Center BIOGRAPHY: Dr. Jingsong Zhang is a genitourinary oncologist at Moffitt Cancer Center. His primary interest is in developing new treatments and biomarkers for prostate cancer and bladder cancer. As a member of the Moffitt experimental therapeutics program, Dr. Zhang has been the Principle Investigator of more than 30 clinical trials. Dr. Zhang has also been actively involved in early phase clinical development of novel therapeutic agents. Among them Durvalumab received accelerated approval by FDA for treating metastatic bladder cancer in May 2017 and Enfortumab was granted FDA breakthrough designation for bladder cancer in April 2018. Dr. Zhang received his medical degree from Peking Union Medical College in China, followed by a Ph.D. in pathology from the University of Southern California. He served as a post- doctoral research fellow at Children’s Hospital Los Angeles, before pursuing a residency in internal medicine at New York Downtown Hospital and fellowship in hematology/oncology at University of Michigan Comprehensive Cancer Center. Jasreman Dhillon, MD~ Pathology Associate Member, Anatomic (GU) Pathology, Moffitt Cancer Center BIOGRAPHY: Dr. Dhillon is an associate member in the Department of Pathology at Moffitt Cancer Center (MCC) who specializes in GU pathology. Dr. Dhillon has been practicing GU pathology for the last 7 years and is the section head of GU pathology at MCC. She received her fellowship training in GU pathology and Cytopathology at MD Anderson Cancer Center. Dr. Dhillon is actively involved in basic and clinical research and has co-authored more than 30 publications and few book chapters as well.

Tim Williams, MD~ Radiation Medical Director, South Florida Proton Therapy Institute BIOGRAPHY: Tim R. Williams, MD, FACR FASTRO is the Medical Director of South Florida Proton Therapy Institute. He has practiced as a board certified radiation oncologist since 1989 in south Florida. He has full medical staff privileges at Boca Raton Regional Hospital and Delray Medical Center and West Boca Medical Center. Dr. Williams completed his undergraduate education at the Georgic Institute of Technology, cum laude, in 1978. His medical education was at the Medical College of Georgia (1983), and his residency was at Shands Hospital of the University of Florida (1987). Before coming to Boca Raton he was Assistant Professor of Radiation Oncology at the Bowman Gray School of Medicine of Wake Forest University. In addition to full-time clinical practice, Dr. Williams is past President (2009) and past Chairman of the Board of Directors (2010) of the American Society for Radiation Oncology. He has also served on the Board of Trustees of the American Registry of Radiologic Technology (2000-2008) and was Chairman in 2006. He has also served as President of the Florida Radiological Society (1999) and received the Gold Medal of the Society in 2013. BIOGRAPHY: Jorge J. García obtained his Doctor of Pharmacy degree from Nova Southeastern University (NSU) College of Pharmacy in 2010. Later that year, he also received a Master of Business Administration (MBA) from the NSU Huizenga School of Business. Dr. García then transitioned to the University of Pittsburgh Medical Center (UPMC) to pursue an ASHP- accredited and combined PGY-1/PGY-2 Health System Pharmacy Administration Residency. As part of this residency program, he also completed a Master of Science (MS) in Pharmacy Administration from the University of Pittsburgh. More recently, Dr. García also completed a Master of Healthcare Administration (MHA) from Florida Atlantic University. Upon completion of residency training, he joined Broward Health Medical Center (BHMC) in Fort Lauderdale, FL, where he served as a Pharmacy Operations Coordinator and Medication Safety Committee co-chair for three years. Dr. García joined the Memorial Healthcare System in 2014 as a Director of Pharmacy for the oncology service line. In this role, he served in multiple committees such as the Oncology and System Pharmacy and Therapeutics Committee, the organization’s Ethics Committee, and led efforts to ensure pharmacy compliance with accreditation entities such as the Commission on Cancer (CoC), the Quality Oncology Practice Initiative (QOPI), and the Oncology Care Model (OCM). Dr. García also founded and chaired the Pharmacy Revenue Integrity Committee and pioneered system efforts to incorporate committee findings in value-based medication management decision-making. In this role, Dr. García significantly impacted financial goals and was the proponent of a fee-for- service to full capitation evolution strategy to leverage value for patients and the organization. Premier Inc. and the Advisory Board are particularly interested in this work and has extended opportunities to Dr. García to speak at national meetings and discuss with the Premier Inc. ACO national cohort. Under his leadership, the Memorial Cancer Institute launched - led Oral Oncolytic Management Clinics that worked alongside the Memorial Specialty Pharmacy to deliver high quality specialty pharmaceutical care. Dr. García also successfully led several patient outcomes research projects to further patient care while also providing an opportunity to leverage access to limited-distribution pharmaceuticals. These efforts were integral to successful negotiations to access Pfizer's Limited Distribution Portfolio. Dr. García has served on the ASHP Residency Advancement and the Manager Development Advisory Groups. Dr. García also currently holds several adjunct faculty appointments at different colleges of pharmacy throughout the State. Dr. García has recently served as a speaker at the national level in the areas of specialty pharmacy, biosimilars, alternative payment models, and oncology treatment adherence. Dr. García has worked very closely with FLASCO over the last few years creating an avenue for pharmacy memberships, a pharmacy education track, and currently serves as a FLASCO board of directors member. Dr. García is the recipient of the FLASCO 2018 Above and Beyond Award. Most recently, Dr. García has achieved advanced status as a fellow of the American College of Healthcare Executives (ACHE). Today, Dr. García serves as the Assistant Vice President of Oncology Pharmacy Services at Baptist Health South Florida.

Bailey was promoted to Director of Pharmacy in 2010, two years after joining FCS as Pharmacy Manager. From 2005 until 2007, Mr. Bailey was General Manager of Walgreens/OptionCare of Southwest Florida. Also, he has been a Staff Pharmacist at Naples Community Hospital, owner of a pharmacy in Pine Island, FL and President/CEO-Owner of OptionCare of Southwest Florida, a national franchise of home infusion pharmacy services. Ray earned his bachelor's and graduate degree from the University of Georgia. SESSION I- ORAL ONCOLYTICS ~Strategies for Optimization of Adherence to Oral Oncolytic Therapies~ OBJECTIVES: 1. Mabel Arslanyan, BS, PharmD, BCOP BIOGRAPHY: Mabel Arslanyan, Pharm.D., BCOP, obtained her Doctor of Pharmacy degree from Nova Southeastern University (NSU), College of Pharmacy and proceeded to complete her Pharmacy Practice Residency training at the University of Illinois at Chicago Medical Center. She further pursued specialized training and completed a PGY2 Hematology Oncology residency at Memorial Sloan Kettering (MSK) Cancer Center in New York City. After her residency training she joined the MSK pharmacy team as a Clinical specialist focusing on the areas of leukemia and lymphoma. She then relocated to South Florida and took on the role of Clinical Pharmacy Supervisor at Miami Cancer Institute. She currently holds the position of Hematology- Oncology Medical Science Liaison. Jessica Unzaga, PharmD, BCPS, BCOP Pharmacy Clinical Coordinator Miami Cancer Institute, Baptist Health South Florida BIOGRAPHY: Jessica Unzaga is a Pharmacy Clinical Coordinator at the Miami Cancer Institute at Baptist Health South Florida. He provides care and support for oncology patients and their families. Dr. Rejtman is board certified in family practice, geriatric medicine, and hospice and palliative medicine. He has subspecialiazed in palliative care since 2001, and has developed expertise in non-interventional pain and symptom management. He also understands the critical psychosocial and spiritual challenges facing oncology patients and their families and is fluent in English, Spanish, and Hebrew. Prior to joining Memorial, Dr. Rejtman served as the lead physician for VITAS Palliative Care Solutions and later became the founding medical director of Catholic Hospice. Dr. Rejtman has served on peer review, ethics and quality improvement boards to help optimize patient care and patient experiences at various hospitals. He was also the lead physician at an institutional review board (IRB) for 6 years, assisting in the research of new medications and medical devices. In addition to providing patient care, Dr. Rejtman enjoys teaching the next generation of physicins. He has been involved in medical education since 2009, teaching family medicine, geriatric medicine and most recently serving as a program fellowship dirctor in hospice and palliative medicine.

She graduated from the Wingate University School of Pharmacy in 2009 with a Doctor of Pharmacy Degree. Following graduation Dr. Khalil became Board Certified in Pharmacotherapy and has worked as a clinical staff pharmacist in internal medicine at Baptist Health as well as well an ambulatory care pharmacist at Jackson Memorial. Dr. Khalil has been at the UHealth system since 2015. She transitioned into the reimbursement analyst role in 2016.

BIOGRAPHY: Shannon began her nursing career in 1992. She has been both an intensive care nurse and CVICU nurse but found her calling as an oncology nurse in 2002. She states, “Oncology patients are the reason I have dedicated my last 14 years to cancer care”. She has managed an oncology unit and is currently the oncology program coordinator as well as certified by both the National Consortium of Breast Centers and Oncology Nurses Society as a Breast Health Navigator. She has been in this role for over 4 years and wants to expand into working in the community as an educator, advocate, and resource for cancer screening and cancer prevention in under served communities. Natalee Roper, RN, BSN, CHPN Nurse Coordinator Moffitt Cancer Center BIOGRAPHY: Natalee Roper is a clinical nurse at Moffitt Cancer Center and is the nurse coordinator for the Cardio-Oncology Program. She works closely with the director for the Cardio- Oncology Program at Moffitt to coordinate patient care during their cancer journey. Natalee earned her Bachelor of Science in Nursing at the University of South Florida and has national certification in Hospice and Palliative Care. Prior to starting her career at Moffitt, she spent her early years working on the progressive care, and cardiac post anesthesia care units, where her passion for cardiac care was nurtured. She worked with patients, so they could recover after open heart surgery, lung surgery, and other cardiac procedures. She began her career at Moffitt in 2010, where she worked on the telemetry in-patient unit for 7 years, before she transitioned to the out-patient setting. Cathy Elstner, RN, BSN AYA Nurse Navigator, Moffitt Cancer Center BIOGRAPHY: Cathy has over 25 years of pediatric and adult oncology experience. For the last 8 years her focus has been adolescent and young adult (AYA) oncology; primarily in the Sarcoma department as well as helping to develop the first AYA program in the state of Florida. Last year she transitioned into the unique position of AYA nurse navigator and is supporting young patients in several departments at Moffitt Cancer Center. Katie Goodman, RN, BSN, CCRP~ Moderator Director of Clinical Research, Florida Cancer Specialists BIOGRAPHY: Katie Goodman is Director of Clinical Research at Florida Cancer Specialists where she oversees the research operations of their early phase research program in Sarasota, FL and late phase programs in 36 community-based locations throughout Florida. Under her leadership, Florida Cancer Specialists currently has a menu of over 100 clinical trials and her team enrolls over 600 patients annually to treatment trials.

Christin Hart, RN, BSN, OCN Urology Cancer Care Coordinator, Florida Hospital Cancer Institute BIOGRAPHY: Christin Hart is an Oncology Nurse Navigator at Florida Hospital Cancer Institute with a focus in Genitourinary Cancers. She started her career in Nursing and love for Oncology in 2003. Christin graduated with a BSN from University of Central Florida and shortly thereafter became an Oncology Certified Nurse. Christin’s 15 years of Oncology experience working in both the inpatient and outpatient Oncology settings has prepared her for her current role as a Nurse Navigator. Christin is most passionate about providing patients with the knowledge and caring expertise needed to navigate their disease throughout the cancer care continuum.

Jose Ortiz-Pagan Survivor BIOGRAPHY: Mr. Ortiz-Pagan was born in Puerto Rico. He served his country and is a veteran of the Vietnam war. Mr. Ortiz-Pagan also served on the New Jersey police force and retired from the profession after years of service. Mr. Ortiz-Pagan was diagnosed with Renal Cell Carcinoma and was given the option of participating in a clinical trial. Mr. Ortiz-Pagan made the decision to enroll and is still currently a part of a clinical trial for the treatment of his kidney cancer.

Beth Davison, MSN, APRN, CNM Clinical Trial Specialist, Clinical Trial Support Center The Leukemia & Lymphoma Society

BIOGRAPHY: In her role as a Clinical Trial Specialist, Beth Davison helps in the Leukemia & Lymphoma Society’s efforts to educate and assist individuals that have been affected by blood cancers regarding the treatment option of clinical trials. Patients who may benefit from expanded clinical trial support services are referred to her by her colleagues in the Information Resource Center (IRC), the first point of contact for many patients affected by a blood cancer. Through education, advocacy, coaching and decision- support services, she and her peers help patients weigh their unique concerns around receiving care through standard treatment or within a clinical trial. LLS is the world’s largest voluntary health agency dedicated to blood cancer. The mission of the Leukemia & Lymphoma Society is to cure leukemia, lymphoma and myeloma, and improve the quality of life of patients and their families. Prior to LLS, Beth held a position as a Certified Nurse Midwife at Reading Health Care System in Reading, PA. In her early career, she worked in many areas of nursing including: Correctional, Public Health, and Case Management. Beth graduated from the University of St. Francis with a Bachelors in Science Nursing, and obtained her Master’s degree in Nursing with a specialty in Midwifery from Frontier Nursing University. She resides in St. Augustine, FL with her husband and three children Tristan, Nyna, and Rylan. AFTERNOON TRACK- SESSION I ~SUPPORTIVE CARE THROUGHOUT THE CANCER JOURNEY~ Ana Rosa Espinosa, DNP, MBA, RN, OCN~ Moderator Administrative Director Oncology Clinical Services, Memorial Cancer Institute BIOGRAPHY: Dr. Ana Rosa Espinosa is the Administrative Director of the Oncology Clinical Services at Memorial Cancer Institute. In her role she oversees the practice of all nursing and APRN’s. She is an exceptional Nursing Director with extensive education in nursing as well as business development skills . She has been in the oncology field for most of her long nursing career, and is Oncology certified. She has a Doctor of Nursing Practice from the University of Miami, a Masters in Business from Nova Southeastern University and a Bachelors in Nursing from Barry University . She is a speaker, has published several nursing articles and has an interest in nursing research. She is promoter of higher education and supporter of nursing research and evidence-based practice. Prior to Joining Memorial Cancer Institute, she was the Nursing Director for the Outpatient Clinics at Sylvester Comprehensive Cancer Center. She is a member of the Florida Nursing Association and the Oncology Nursing Society. She is an active member in her community and is involved in many cancer events. Adrienne Vazquez Guerra, MSN, ACNP-BC, AOCNP~ Moderator Survivorship Operations Leader, University of Miami, Sylvester Comprehensive Cancer Center BIOGRAPHY: Ms. Adrienne Vazquez is a board certified acute care nurse practitioner. She studied at Barry University in Miami Shores for both her undergraduate and graduate degrees and is a Sigma Theta Tau member since her 2004 induction. Ms. Vazquez worked in the inpatient unit and bone marrow transplant unit at Jackson Memorial Hospital, in thoracic/head and neck oncology and as the infusion nurse practitioner at the University of Miami Sylvester Comprehensive Cancer Center. She was awarded the 2013 Nurse Practitioner of the Year at Sylvester Cancer Center. In early 2015 she was offered a position at Memorial Sloan Kettering Cancer Center in New York City within the department of thoracic oncology managing patients on clinical trials. She returned back to South Florida and accepted a position as the Operations Leader of the University of Miami Survivorship and Precision Medicine programs. She is currently the director of the clinical Survivorship program at Sylvester Cancer Center and has had numerous abstracts and papers written on the topic. Ms. Vazquez is an Advanced Oncology Certified Nurse Practitioner® (AOCNP) and is the current chair of the item writing and test development national committees for the AOCNP board exam. She has attended numerous medical missions to South America and Project Medishare in Haiti. In August 2014 she volunteered to be the nurse at Camp Kesem (a camp for children whose parents have cancer). ~Palliative Care~ OBJECTIVES: 1. Describe current trends in Oncology care today 2. Articulate key issues for patients, families, and healthcare systems related to cancer diagnosis, treatment, goals of care, etc. Mayra Lima, APRN, MSN, FNP-C Breast Cancer Treatment and Prevention Program, Sylvester Comprehensive Cancer Center, UM BIOGRAPHY: Mayra Lima has been a Registered Nurse since 1985 (30 years). She has worked in many different roles such as ER, Ophthalmology, Surgery, Disease Case Management, and Director of Nursing. She accomplished this all the while continuing her education and completing her Bachelor's degree at FIU, followed by the Masters in Nursing Science and Family Nurse Practitioner with a Certificate in Gerontology by December of 2000. As an nurse practicioner, Ms. Lima, has dedicated her work to oncology and solid tumors in GI and Research at University of Miami/Sylvester for 6 years. Ms. Lima decided to dedicate herself to helping those with breast cancer once she became a survivor of breast cancer herself. She has worked with breast cancer patients for the past 9+years. Recently she relocated from the Memorial Breast Cancer Center to the Sylvester Breast Cancer Center in Plantation. In her personaI life she loves to travel, arts & crafts, dancing, music and all within the context of her family. Ms. Lima’s survival of breast cancer propelled her to do half marathons. Since 2007 she has completed 16. Ms. Lima also has a love of knowledge, she is a recent alumna of the Cancer Genetics Risk Assessment Program. ~Florida’s New Law on Controlled Substance Prescribing~ OBJECTIVES: 1. Garcia, APRN, MSN, FNP Oncology Supportive Care Services (Palliative Care/Pain Management) Memorial Cancer Institute BIOGRAPHY: Ana Garcia is an Advanced Registered Nurse Practitioner, specializing in Oncology Supportive Care Services (Palliative Care/Pain Management). Ana received her Bachelor’s of Science in Nursing and Master’s of Science in Nursing at Florida International University in Miami, Florida. She began her career as a Medical Doctor in Colombia when she graduated from Libre University in Cali, Colombia (1995). In 2000, she completed her Master’s in Occupational Health at Libre University. She worked for a primary care physician and Occupational Health Physician in Colombia for serveral years. She migrated to United States and attended the program at Florida International University for foreign trained doctors to become nurse practitioners. She received her Bachelor of Science in Nursing (BSN) in 2012 graduating Summa Cum Laude and received her Master’s of Science in Nursing (MSN) as a Family Advanced Nurse Practitioner in 2013. She received her National Certification from American Assocation of Nurse Practitioners (AANP). After graduation, she worked in Bradenton, Manatee County as a Family Nurse Practitioner in a primary care setting for 2 years. In August of 2015, she joined the Memorial Cancer Institute (MCI), Oncology Supportive Care Services (Palliative Care/Pain Management) to provide quality care to Memorial Cancer Institute (MCI) patients, as part of a multidisciplinary team. Ana has been in the healthcare field for over 20 years. ~Survivorship~ OBJECTIVES: 1. Define supportive care. 2. Discuss care needs of individuals during and after cancer treatment. 3. Describe supportive care interventions that are patient specific. 4. Identify care needs of individuals facing advanced and/or progressing cancer across all domains of health. 5. Review interventions to optimize functional abilities at the end of life, consistent with patient preferences. Jessica McMillan, DNP, FNP, APRN-BC Breast Cancer Treatment and Prevention Program Mayo Clinic Florida BIOGRAPHY: Dr. McMillan has been a member of the Mayo Clinic staff since 1998, in multiple roles. Currently, she serves as an Advance Practice Provider in the Department of Hematology/Oncology since 2010. She has also worked as a Clinical Nurse Educator and Charge Nurse/Team Leader in the surgical Intensive Care Unit. Dr. McMillan obtained her family Nurse Practitioner Certification from the University of North Florida in 2009, and went on to obtain her Doctorate of Nursing in Family Practice with a Sub-specialty in Palliative Medicine in 2015. She serves as a preceptor for physician assistant and nurse practitioner students at Mayo Clinic Florida School of Medicine. She also works as a Adjuvant Instructor for the University of North Florida where she inspires and mentors future nurses. She is very active with the Florida Society of Clinical Oncology and the Northeast Florida Advance Nurse Practitioner Association.Within the Division of Hematology/Oncology, she specialiezes in Gastrointestinal Tumors but serves as a resource in all areas of solid tumors for her feelow coleagues. Her passion is patien care and education. She feels very fortunate to work with cancer patients where she able to utilize her skills and education to forge relationships for patients who face uncertain futures. Cheryl Habaue, MSN, RN, OCN, CCRP~ Moderator Nursing/Navigator Track Chairman Florida Society of Clinical Oncology Clinical Research Nurse BIOGRAPHY: Cheryl Habaue received her BSN from Mount Mercy College in Cedar Rapids, Iowa and MSN from Chamberlain College of Nursing. She has been in research for the past 6 years working on studies related to all cancers including but not limited to Lung, Breast, GI, GYN, GBM, Leukemia, Multiple Myeloma, Lymphoma, Head and Neck, as well as Sarcomas and Melanomas. Cheryl currently works on phase I-IV clinical trials, focusing on trial management, process improvement and project management.

Andrea Uitti, DNP, RN Associate Director of Research Programs and Services Florida Center for Nursing Florida Action Coalition BIOGRAPHY: Dr. Andrea Uitti is the Associate Director of Programs and Grants, Florida Center for Nursing & the Florida Action Coalition. In this role, she works to help nurses transform healthcare in Florida. Drawn to the Florida Center of Nursing’s innovative and strategic vision to address all nursing workforce issues in Florida, she is helping to promote purposeful interventions in our states primary areas of need. Andrea successfully led grants that provide leadership training for nurses, research, educational, and collaboration opportunities for the nursing community in Florida. Accomplished with the unwavering support and funding from the Florida Blue Foundation, leadership of the Florida Action Coalition, and our national partners AARP & RWJF. Her current goals include engaging nursing groups and community members to play a vital role in educating the public and policymakers. Working to build a critical mass of awareness, and channeling public concern about our state healthcare crisis into effective action. Previously, Andrea worked in cultural and medical anthropology, both in Mexico and with the Choctaw Indians, with a focus on diabetes. The Mott Foundation Safe Energy Communication training, media skills training and outreach strategies helped her begin her career working with volunteers and coalitions. Andrea served on environmental committees and task forces, providing recommendations to the public and policymakers. She built effective general media and targeted communication campaigns to achieving environmental protection policy goals in Birmingham, Alabama. Dr. Uitti received a DNP at Samford University, with a focus on policy and administration. As an RN she practiced as an ICU nurse, while completing a master’s degree in nursing education. BIOGRAPHY: Maggie Wiegandt has been a healthcare administrator for over 15 years. In her current role as Administrative Director for Outreach and Liaison Development, she leads business development and philanthropic activities for the Memorial Cancer Institute Adult Specialties and Joe DiMaggio Children’s Hospital. In her past role as Vice President of Oncology Services, she was responsible for providing programmatic, operational and strategic leadership to further the growth, improved outcomes and integration of cancer services within the Memorial Healthcare System. Memorial is located in Broward County and it the 3rd largest public healthcare system in the nation.

Maggie’s involvement and collaboration with community Oncology partners has been critical to improving the cancer care provided at Memorial Cancer Institute. Her community commitments have included Gilda’s Club of South Florida Executive Committee Board Member, American Cancer Society Board Member, Making Strides Against Cancer Committee Chair, Pink Angels of the Memorial Foundation Executive Committee Member, and Past Chair of the Board for Miramar/Pembroke Pines Chamber of Commerce. Awards include 1,000 Plus Woman of the Year, Hispanic Woman of Distinction and South Florida Business Journal Up and Comers.

Maggie earned her Bachelor of Science Florida State University and a combined Master of Healthcare Administration and Master in Business Administration from the University of Houston. She lives in Palm Beach County with her family.

Abby Castillo Moffitt MHCT at Memorial

BIOGRAPHY: Abby Castillo, is the Program Director for the Moffitt Malignant Hematology & Cellular Therapy Program at Memorial Healthcare System. She is responsible for the operational management and support of the new Moffitt Cancer Center partnership at Memorial. She collaboratively oversees interdepartmental operations and facilitates optimizations for team members, patients and families while ensuring excellence in patient care for the Heme-Malignancies and Bone Marrow Transplant program. Abby has established her professional career over the past 19 years working for major universities and healthcare systems in Missouri, Illinois and Florida. Before joining the Memorial team, she was responsible for capital projects for system wide strategic and facility initiatives at Jackson Health System in Miami. Previously at Memorial she led the Enterprise Portfolio Program Management Office for Strategic Planning. Abby received her Bachelor’s degree from Southern Illinois University Edwardsville with a major in Construction Management, followed by a MBA from Florida International University with a focus in Healthcare. Abby sits on the Broward Board for Leukemia Lymphoma Society, is a committee chair for Tour de Broward, an active volunteer in the Memorial Healthcare System H.E.R.O.s program, and her and her husband have a non-for-profit that provides school supplies for children in Nicaragua.

Gail Erentreich Cancer Care Centers of Brevard

BIOGRAPHY: Gail has been in the healthcare industry since 1982 and Medical Oncology since 1986. She went to Fisher College in Boston and then moved to the west coast and lived in Los Angeles from 1982 to 1986 where she managed a general medicine and cardiology practice. Dr. Edward Knight, Gail’s father, was a practicing Medical Oncologist in Rockledge, Florida. He asked her to come and run his practice in 1986 and even though she wasn’t sure she wanted to work for her father, she accepted and has been in the same office for almost office for 32 years as the Director of Clinical Services for Cancer Care Centers of Brevard. Gail has preceded all of the 7 Medical Oncologists currently practicing at Cancer Care Centers of Brevard. Medical Oncology has been very challenging and yet, she can’t think of doing anything else. Gail worked with the University of Central Florida and Oncology Managers of Florida to develop a certificate program specific to the Management of Medical Oncology which was repeated at UCF 4 times. Gail has been on the board of Oncology Managers of Florida since 1992 and after holding the position of President in 1997and 2007, she continued as Vendor Coordinator. Gail’s claim to fame was when she was 6 years old and lived in Stockbridge, Massachusetts she went on a house call to Norman Rockwell, the famous illustrator’s house with her father.

Josh Mattson Mayo Clinic Florida

BIOGRAPHY: Joshua Mattson is an Operations Administrator for Cancer Programs at Mayo Clinic in Florida. In his current role, he oversees operations for Hematology Oncology, Radiation Oncology, BMT, Center for Breast Health and serves as Administrator for Mayo Clinic enterprise Breast Cancer Service line and enterprise Radiation Oncology. Mr. Mattson has worked in healthcare for 19 years and has been a part of the Mayo Clinic team since 2007. Prior to joining Cancer Programs, he held positions within the Revenue Cycle and the Department of Transplantation. Most recently, Mr. Mattson was the Clinical Operations Manager for Cancer Programs at Mayo Clinic in Florida where he was responsible for day to day clinic operations. Mr. Mattson holds a Bachelor of Arts degree in Business Administration from the University of North Florida and the Master of Business Administration from Florida International University. He is a member of the American College of Healthcare Executives and Northeast Florida Medical Group Management Association. In addition, he is a member of the North Florida Leukemia and Lymphoma Society Light the Night Executive Committee. Mayo Clinic Florida Cancer Center, an NCI designated Comprehensive Cancer Center is located in Jacksonville, Florida. In 2017, the State of Florida recognized for a second time as a State of Florida Cancer Center of Excellence. The Jacoby Center for Breast Health at Mayo Clinic Florida is accredited by the NAPBC and was recognized as a Best Practice for Interdisciplinary Patient Management.

BIOGRAPHY: As Chief Revenue Cycle Officer for Florida Cancer Specialists and American Oncology Network (AON), LLC, Sarah Cevallos is responsible for patient financial services, billing, timely collection, and precise payment posting, while delivering a positive patient experience. Sarah is also responsible for fostering payer partnerships and leads the value-based care initiatives to enhance quality payment programs to support the delivery of high quality cancer care to patients for the network of providers. Also, Sarah serves as Clinical Practice Committee Member for the Florida Association of Clinical Oncology (FLASCO) and works closely with the Community Oncology Alliance (COA) on reimbursement and practice transformation strategies towards value- based care. She has been recognized by the Florida Society of Clinical Oncology (FLASCO) as the inaugural recipient of the Distinguished Service Award for her continued support of facilitating resolution of reimbursement issues for oncologists in Florida. Sarah holds a Bachelor of Science degree in Business Management from Nova Southeastern University and an MBA degree from Western Governors University with a concentration in Healthcare Management. Sarah has been quoted and interviewed by the American Journal of Managed Care (AJMC) on various topics ranging from the Oncology Care Model (OCM) to collecting patient data to drive decisions.

David Segal Aetna

BIOGRAPHY: As Network Vice Present of Network Management with Aetna, David Segal is responsible for the Tampa / Southwest Florida local market managing the entire provider relations and healthcare contracting team. He has more than 22 years of healthcare experience with Aetna and over 16 years collaborating directly with Florida healthcare providers. Through the years David has received numerous awards from the National Winner’s Circle (for most improved hospital deal) to national recognition in helping with the development of Aexcel in 2004, one of Aetna’s first performance networks. He currently is a voting member for the Southeastern United States for Aetna’s network steering committee which determines national network strategy. David has always been collaborative with providers to come up with ways to keep healthcare more affordable. David Graduated from Jacksonville University with a Bachelor in Business Administration majoring in business administration and business management and enjoys cruising the Caribbean with his family.

BIOGRAPHY: Dr. Hertler, as the CMO of New Century Health, is responsible for the advancement of the company’s clinical quality and value-based strategy, utilization management policies and clinical thought leadership initiatives. A practicing board certified oncologist for 30 years, he is a nationally recognized leader in oncology clinical practice. Dr. Hertler has volunteered on a number of American Society of Clinical Oncology (ASCO) committees, including the Clinical Practice, Quality of Care and Payment Reform Committees, as well as the Quality Oncology Practice Initiative Certification Program Oversight Council.

BIOGRAPHY: Dr. Hussein has participated in numerous trials and he’s one of the Principal investigators for lung cancer trial within Florida Cancer Specialists & Research Institute. He encourages qualified patients to consider participating in clinical trials not only because of the future development of new therapies, but because patients who participate often receive a promising new therapy even before it is approved for general use. Dr. Hussein is an active participant and long standing member of the Florida Society of Clinical Oncology. He is the FLASCO Treasurer and an invaluable contributor on the Board of Directors.

BIOGRAPHY: Amanda Brahim, Pharm.D., is the Clinical Pharmacy Manager for Hematology/Oncology at Memorial Healthcare System. She holds a position on the American Society for Blood and Marrow Transplant Pharmacy SIG membership committee and American College of Clinical Pharmacists (ACCP) Hematology-Oncology PRN task force. Dr. Brahim received her Doctor of Pharmacy degree from Palm Beach Atlantic University in West Palm Beach, Florida in 2011 and completed a pharmacy practice residency at Cleveland Clinic Florida. Dr. Brahim is Board Certified in Oncology, Pharmacotherapy and Ambulatory Care. She holds professional memberships with ASBMT, ACCP, ASHP and HOPA.

BIOGRAPHY: Shelli Johnson has more than 20 years of healthcare experience in both the provider and insurance arenas. The focus of her career for the last 15 years has been in community based oncology, providing administrative, financial counseling, and AR management for cancer and hematology patients in Brevard and Indian River Counties. Her unique experience has allowed her to pursue her goals of providing not only excellent care to her patients, but equal and affordable access to care for all patients in need regardless of their situation. Shelli attended Ohio State University and Keiser University. She currently lives in Palm Bay, Florida and enjoys spending time with her husband, and two sons.

Candice Banks Cancer Graduate

BIOGRAPHY: My name is Candice Banks. I am a forty-two-year-old wife and mother of two sons. Before breast cancer, I was a very active individual. We would go on family camping trips to the Keys, Kayak along the Indian River, fish with my family, and participate in a multitude of outdoor activities. Having Breast Cancer has impacted that a bit leaving me drained of energy and have random pains everywhere. I am a warrior. A fight for my life. When asked to write a biography about myself it was difficult for me. The reason for this is that it is hard for me to remember my life without cancer, because my wife and I have been completely consumed with it in every aspect. Living and breathing breast cancer. Now, friends and even strangers call me an inspiration; reaching out to me for guidance, advice, and support for their individual breast situations.

I have been blessed with a huge support group throughout this journey and I am making new breast cancer fighter/survivor friends. Today, I get privilege to tell my story to whoever wishes to hear it. I’ve been asked to speak at the FLASCO Fall Session, and am becoming a voice for something I believe in and have worked hard for; what an honor. It hasn’t even been a year that I have been fighting breast cancer, but I’ve already grown from the experience and I will continue to grow and beat it.

FLASCO was formed in 1977 as a statewide 501(c)(3) nonprofit organization committed to facilitating and promoting multidisciplinary efforts to improve patient care in Florida. FLASCO continues to make strides in promoting excellence in care of patients afflicted with cancer and hematologic disorders and leads efforts to respond to policies and legislation that threaten to compromise patient access to quality cancer care.

FLASCO is gaining momentum as more professionals use us as the catalyst to develop partnerships, share knowledge, and stand together to demonstrate strength and determination to move forward in the battle against cancer.

FLASCO consists of over 3,200 members that include physicians and their supporting staff, allied healthcare, and industry representatives that represent the oncology professionals of Florida.

FLASCO will continue to be recognized as a nationally respected state professional society as we stand together as “the Voice of Oncology in Florida”.
