AVIATION OCCURRENCE CATEGORIES DEFINITIONS AND USAGE NOTES December 2017 (4.7) Occurrence categories are used to classify occurrences (that is, accidents and incidents) at a high level to permit analysis of the data in support of safety initiatives. Categories, such as CFIT and “loss of control” have been developed specifically for this purpose. RECORD OF REVISIONS Date Version Section Revision Amended usage notes for LOC–I with the manned 12/2017 4.7 LOC–I and unmanned rotorcraft language Amended usage notes for FUEL with the exhausted 12/2017 4.7 FUEL battery language Amended usage notes for ARC with the rotor 12/2017 4.7 ARC strikes language 10/2013 4.6 Table of Terms Added new MED and NAV categories Added usage note to include deficiencies with 10/2013 4.6 Aerodrome (ADRM) snow, frost, or ice removal from aerodrome surfaces; reformatted usage notes Airprox/TCAS Alert/Loss of Separation/Near Midair Added cross-references to new NAV category, 10/2013 4.6 Collisions/Midair Collisions reformatted usage notes (MAC) Some CABIN events are now covered in the 10/2013 4.6 Cabin Safety Events (CABIN) new MED category. Updated and reformatted usage notes. Added usage note to include deficiencies related to 10/2013 4.6 Ground Handling (RAMP) removal of snow, frost, or ice from aircraft; reformatted usage notes 10/2013 4.6 Medical (MED) New category 10/2013 4.6 Navigation Errors (NAV) New category Added cross-references to new NAV category, 10/2013 4.6 Runway Incursion (RI) reformatted usage notes. 10/2013 4.6 Attachment A Updated to include new MED and NAV categories Added usage note to include failure of glider winch 7/2013 4.5 Aerodrome (ADRM) launch equipment. Added new sentence to “occurrence” definition to include Unmanned Aircraft Systems (UAS): “This includes accidents or incidents, as defined in Annex 4/2013 4.4 Introduction 13 to the Convention on International Civil Aviation, involving manned aircraft or unmanned aircraft that have a design and/or operational approval.” Removed references to wildlife other than birds, 4/2013 4.4 Bird (BIRD) which are covered under the Wildlife (WILD) category. Controlled Flight Into Terrain Added usage notes to exclude intentional grounding 4/2013 4.4 (CFIT) of UAS or UAS control failures. Removed reference to collision with animals. 4/2013 4.4 Ground Collision (GCOL) Updated cross references to Runway Incursion (RI) and Wildlife (WILD). Revised 12/2017 Page ii Date Version Section Revision Changed “collisions” to “occurrences” in the first usage note bullet to include all occurrences during 4/2013 4.4 Ground Handling (RAMP) passenger boarding and deplaning, not just collisions. Loss of Control–Ground Updated cross references to Runway Incursion (RI) 4/2013 4.4 (LOC−G) and Wildlife (WILD). Renamed Runway Incursion–Vehicle, Aircraft, or Runway Incursion–Vehicle, Person (RI–VAP) to Runway Incursion (RI). 4/2013 4.4 Aircraft, or Person (RI–VAP)/ Added exclusion to events coded as WILD or Runway Incursion (RI) BIRD. New category to replace Runway Incursion–Animal 4/2013 4.4 Wildlife (WILD) (RI–A) category and to augment Bird (BIRD) category. Formatted document and made minor editorial 10/2012 4.3 Document changes Added bullets regarding unmanned aircraft systems: “For unmanned aircraft, includes failure or malfunction of ground-based, transmission, or System/Component Failure or aircraft-based communication systems or 10/2011 4.2 Malfunction (Non-Powerplant) components or datalink systems or components.” (SCF–NP) and “Includes failures/malfunctions of ground-based launch or recovery systems equipment.” Added bullet for unmanned aircraft systems: “For unmanned aircraft events, includes hazardous outcomes involving deviation from intended flightpath associated with anticipated or 10/2011 4.2 Loss of Control–Inflight unanticipated loss of datalink. However, if loss of datalink is the direct result of a system/component failure or malfunction, code the occurrence as system/component failure or malfunction– non-powerplant (SCF–NP) only.” 5/2011 4.1.5 Document Reformatted document Added BIRD, CTOL, EXTL, GTOW, LOLI, and 5/2011 4.1.5 Attachment A UIMC categories to groupings and alphabetized groupings.
