Program Highlights

Primary Care

The primary health care team began serving clients in April of 2010 out of our temporary location at 55 Water Street lead by Beccy Fournier, our first Nurse Practitioner. In August, when the new building on Pitt Street was completed, the second Nurse Practitioner Leanne MacDonald and Registered Nurse Leanne Clouthier were hired and registration of new clients began in earnest. By January 2011, Dr. Ioannis Felemegos (the first of two physicians), joined our team and the full scope of one-stop primary health care was offered at SVCHC. By the fall, Registered Nurse Nathalie Lamothe joined our team on a part-time basis.

The primary care team remains focused on registering 2,500 new clients, although for the first year or two this continues to be a lengthy process. We are on a learning curve with a new Team, a new environment, learning to use the electronic client record system and finding efficiencies in our service delivery methodology: all of this requires patience and time. Special Thanks to our Volunteers We remain grateful to Dr. Amir Nanji for his on-going support and assistance in providing primary health care to our newly registered clients and consultation to our Nurses. Counselling and Support Program

We have placed an emphasis on improved accessibility to counselling support through timely access to appointments for existing clients and offering, on a trial basis, emergency walk-in support. This has resulted in the provision of counselling and support services for 121 unique clients in our first seven months of operation. Our client data indicates that our Social Worker is

6 serving a significant percentage of individuals and families with low incomes (one of our target 7

populations), and many clients are shared collaboratively with the SVCHC primary care team. As well, there is a focus on coordinating care of clients with concurrent disorders in collaboration with the Cornwall Community Hospital Mental Health programs and other local mental health agencies.

10 11 Nancy Herrington, our Health Educator/Promoter obtained certification from the Training Enhancement in Applied Cessation Counselling and Health (TEACH) Project, Centre for Addiction and Mental Health (CAMH), integral to the ongoing provision of smoking cessation groups and individual counselling. SVCHC is now offering Smoke Cessation sessions and helping people reduce significantly or quit smoking altogether.

Our seniors’ wellness program – Livin’ It Up! – continues to be a great success as per the participants testimonials. With the recruitment of Madelaine Waller, the competent new Volunteer Assistant, Livin’ It Up Program is now completely coordinated and delivered by community volunteers.
