Information 1.1 Tender shall be submitted to The Chairman, Store Committee , LIC of India Divisional Office , Nagpur Road ,Jabalpur in sealed envelope clearly mentioned that “Tender for Printing of Club Stationery” containing three sealed envelope 1. Technical Bid (Annexure-B) with Annexure-A 2. Financial Bid (Annexure-C) 3. Tender Fees & EMD. 1.2 The Tender form should be properly filled in along with the name, address, telephone Number, email-id, signature and rubber stamp of the authorized person and should be sealed in cover/envelope as per Sr 1.1 above. 1.3 Tender should be submitted on or before Last date of submission i.e. 22.03.2017 till 3.00PM. Tender received after Last date will not be entertained. 1.4 Technical Bid is liable to rejected if rate is quoted or any indication is given there in. 1.5 Each and every page should be signed by authorized person of the firm and seal. 1.6 Size and specification of the Club Stationery will be as Corporate Identity Program Part 5 and it will be the part of tender. ( The program has been appended) 1.7 Quantity/Strength of the club member agents as mentioned above may vary +/- 25%. Actual Quantity/Strength of the club member agents will be communicated by the concerned Branch offices at the time of final order. 2 Conditions : 2.1 No deviation from CIP norms such as size, quality, and color of printing on any count will be accepted and in case of deviation, action against firm including forfeiting of security money and action of black listing will be taken and payment will also not be made. 2.2 Vendor has to collect data i.e. name address mobile no etc. of all club members from each branch and after the proof finally been checked by the concerned branch, supply Printed stationery to the branch in a separate pack for each agent with in two month(60 Days) from the data of collection of data from branch . No extra carriage and fright charges will be paid. 2.3 Rate quoted shall be inclusive of all taxes. No separate letter or any condition will not be accepted. Cont-2 -2- 2.4 The tender shall be approved to only one vendor on the basis of the total amount of all the Six category ( Galaxy, CM,ZM,DM,BM,DC) quoting the lowest rate. 2.5 After supply of goods, if it is found that the material supplied is not according to the approved samples and specifications, the same will be rejected at the supplier’s cost and no payment will be made for the rejected goods. 2.6 No advance or part payment will be made. 2.7 One original paper sample of each type of quoted paper stating clearly thereon full specifications like make/name of Paper Mill, Size, GSM, quality and item/form no. should be submitted with quotation. 2.8 Payment will be made by concerned branch after full completion of work for the branch and TDS, Penalty and any other deduction, if any, will be deducted as per rules. 2.9 Rates quoted by you shall be valid for the period of ONE YEAR only. Tender period can be extended for one more year with the prior permission of Competent Authority and mutual consent. At the same time Competent Authority may lessen the tender period during extra ordinary circumstances also. 2.10 If the Work not completed in specific time schedule, penalty will be imposed as under Within One week - 2% of Bill amount pertaining to the outstanding job. Within Two week - 3% of Bill amount pertaining to the outstanding job. Within Three week - 4% of Bill amount pertaining to the outstanding job.     Ministry of Health and Family Welfare National Family Health Survey - 4 2015 -16 District Fact Sheet Jabalpur Madhya Pradesh International Institute for Population Sciences (Deemed University) Mumbai 1 Introduction The National Family Health Survey 2015-16 (NFHS-4), the fourth in the NFHS series, provides information on population, health and nutrition for India and each State / Union territory. NFHS-4, for the first time, provides district-level estimates for many important indicators. The contents of previous rounds of NFHS are generally retained and additional components are added from one round to another. In this round, information on malaria prevention, migration in the context of HIV, abortion, violence during pregnancy etc. have been added. The scope of clinical, anthropometric, and biochemical testing (CAB) or Biomarker component has been expanded to include measurement of blood pressure and blood glucose levels. NFHS-4 sample has been designed to provide district and higher level estimates of various indicators covered in the survey. However, estimates of indicators of sexual behaviour, husband’s background and woman’s work, HIV/AIDS knowledge, attitudes and behaviour, and, domestic violence will be available at State and national level only. As in the earlier rounds, the Ministry of Health and Family Welfare, Government of India designated International Institute for Population Sciences, Mumbai as the nodal agency to conduct NFHS-4. The main objective of each successive round of the NFHS has been to provide essential data on health and family welfare and emerging issues in this area. NFHS-4 data will be useful in setting benchmarks and examining the progress in health sector the country has made over time.

The primary objective of the farm was to supply high quality pure seeds of recommended paddy varieties to the farmers of the Balaghat region. Later, because of inclusion of Balaghat district in the Chhattisgarh plain agro-climatic zone, the RARS was attached to the then newly established Indira Gandhi Krishi Vishwa Vidyalya, Raipur in 1987-88. However, with the formation of Chhattisgarh as a new state in year 2000, RARS, Waraseoni was again transferred back to JNKVV, Jabalpur on 6th November 2000. Since, then the RARS Murjhad Farm is an inherent part of JNKVV, Jabalpur. In the year 2019, College of Agriculture, Balaghat was accreditated by the ICAR peer review committee with a prestigious score of 3.01.

The project site is approx at a distance of 08 km from Jabalpur Railway Station and 07 km from HQ MB Area on the fringe of cantonment area. The Project site is a beautiful hillock with steep slopes. The water shed area of river Gour and dense plantation from three sides creates a panoramic view. The complex being developed by AWHO comprises of a total of 136 dwelling units which include Independent Bungalows and mix of Duplex, Deluxe, Modern and SFA apartments in multistoried configuration.
